Delirium during pregnancy indicates an extreme heat condition of the Zang-Fu organs. It is urgent to frequently administer child's urine (Tongbian). If ineffective, use Shengdi Huang Huanglian Tang (Rehmannia and Coptis Decoction) to clear the fire in the blood, thereby stabilizing the fetus. If the pulse is excess (Shi), add Jiu Dahuang (Wine-processed Rhubarb) to purge; delay in purging will damage the fetus. There are cases where the fetus is damaged, leading to bleeding after miscarriage, causing the heart spirit to lose its master and resulting in delirium. Even with strong tonification, efficacy is difficult to achieve. The treatment methods and formulas for such cases are recorded in the section on "Postpartum Delirium and Hallucinations." If a pregnant woman presents with a cyanotic tongue, the fetus has died in the uterus, and immediate purgation is required. In cases of twin pregnancy where one is alive and one is dead, there must be coldness on one half of the abdomen and heat on the other. If the mother suffers from a heat syndrome with excessive internal heat steaming the fetus, causing fetal death, administering Heishen San (Black Divine Powder) with fresh Puhuang (Polygoni Typhonii Pollen) can warm the fetus, facilitating its expulsion. If the fetus is not yet dead, applying well mud to the umbilicus can protect the fetal Qi, often preserving the pregnancy.
Shengdi Huang Huanglian Tang (Rehmannia and Coptis Decoction): Treats dryness-heat, thirst, and rapid, full pulse following blood loss.
Ingredients: Sheng Di Huang (Rehmanniae Radix, 2 qian), Fang Feng (Saposhnikoviae Radix, 0.8 qian), Chuan Xiong (Ligusticii Chuanxiong Rhizoma, 0.8 qian), Dang Gui (Angelicae Sinensis Radix, 1.5 qian), Huang Lian (Coptidis Rhizoma, 0.7 qian), Huang Qin (Scutellariae Radix, stir-fried, 1 qian), Hei Shan Zhi (Gardeniae Fructus charred, 1 qian), Chi Shao (Paeoniae Radix Rubra, 1 qian).
Xuezheng Heishen San (Blood Disorder Black Divine Powder): Treats persistent hematemesis (vomiting blood) and epistaxis (nosebleed).
Ingredients: Pao Jiang (Zingiberis Rhizoma Preparatum, 1 liang), Rou Gui (Cinnamomi Cortex, 1 liang), Shu Di Huang (Rehmanniae Radix Preparata, 4 liang), Dang Gui (Angelicae Sinensis Radix, 3 liang), Pu Huang (Polygonii Typhonii Pollen, sifted and charred, 3 liang), Bai Shao (Paeoniae Radix Alba, wine-processed, 2 liang), Zhi Gan Cao (Glycyrrhizae Radix et Rhizoma preparata, 2 liang).
Usage: Grind into powder. Take 4 qian per dose. Stir-fry half he of Hei Dou (Glycines Soja Seed) until fragrant, pour half zhan of wine over it, add half zhan of water, decoct until half remains, mix with half cup of Tongbian (child's urine) and take. For Qi deficiency, add Renshen (Ginseng Radix, 3 liang) and Zhi Huangqi (Astragali Radix preparata, 6 liang) to secure Wei Qi and prevent Ying Qi collapse.
When women suffer from Qi and Blood deficiency, spiritual weakness, and accumulation of evil thoughts, causing stagnation and reversal in the Chong and Ren meridians, with obstructed and stagnant pulses, symptoms may resemble pregnancy; thus it is called "Ghost Fetus" (Guotai). However, upon detailed examination, Guotai is a pseudo-fetus. When illicit sexual thoughts arise, the Ministerial Fire of the Liver and Kidney activates. Dreams of intercourse with spirits do not imply actual conception by spirits. As stated in the Classics, "thoughts endless, desires unfulfilled," lead to Baiyin (white discharge) and Baizhuo (turbid discharge) flowing into the uterus, condensing into Guotai. This is actually the woman's own blood and essence gathering into a mass, where blood knots with Qi and does not disperse, causing chest and abdominal distension resembling pregnancy. Hence, it is a pseudo-fetus. The pulse of Guotai is deep, fine, wiry, and choppy, or sometimes floating, sometimes tight, fluctuating in size, indicating insufficient Yang Qi. Although the abdomen gradually enlarges with a heavy bearing sensation, it differs fundamentally from a normal pregnancy. Slight movement below or to the left of the umbilicus also differs distinctly from a true fetus. Therefore, upon detecting irregular menstruation, immediate regulation and tonification should be applied to avoid this condition. If caused by amenorrhea and stasis, belonging to Xuzha (blood mass) or Qizha (Qi mass), the disease is established. Treatment should primarily tonify Yuan Qi, followed by drugs to resolve accumulations. To combine tonification with unblocking, Juejin Jian (Juejin Decoction) is superior; to remove stasis without being too harsh, Tongyu Jian (Tongyu Decoction) is best. If caused by Yang Qi deficiency or Liver-Spleen depression anger damaging the ability to generate, leading to Yin Blood non-transformation and amenorrhea with abdominal enlargement, use Jiawei Guipi Tang (Modified Guipi Decoction) or Jiawei Xiaoyao San (Modified Xiaoyao Powder) plus Chenpi (Citri Reticulatae Pericarpium, peeled, 0.8 fen). For dry cough, honey-processed treatment leads to self-healing. Cases involving supernatural encounters are real infections but not Guotai; treat as Zhengjia (masses) and purge. Often, women conceive with stopped menses, but the Chi pulse is choppy or weak without other diseases. This indicates incomplete Uterus Zheng Qi. Although Jing and Blood congeal, Yang deficiency prevents Yin transformation, so the fetus does not form properly. At term, blood clots and placental membranes are discharged. Large doses of warming and tonifying herbs must be used for pre-conditioning to ensure successful conception.
Juejin Jian (Juejin Decoction): Treats female Blood deficiency with menstrual stasis, causing severe pain due to lack of flow. It acts like water supplementing water; when a river breaks its banks, accumulated dirt is washed away. This is a miraculous formula using tonification as purgation.
Ingredients: Dang Gui (Angelicae Sinensis Radix, 3-5 qian or 1 liang), Ze Xie (Alismatis Rhizoma, 1.5 qian), Niu Xi (Achyranthis Bidentatae Radix, 2 qian), Rou Gui (Cinnamomi Cortex, 1-3 qian), Shu Di Huang (Rehmanniae Radix Preparata, 2-3 qian or 5-7 qian, or omit), Wu Yao (Linderae Radix, 1 qian).
Usage: Decoct 2 zhong of water to 0.7 portion. Take before meals. For nausea/vomiting, add Jiao Jiang (Charred Ginger, 1-2 qian); for Yin stagnation, Aconitum (Fuzi) is essential; for Qi stagnation with pain/distension, add Xiang Fu (Cyperi Rhizoma, 1-2 qian) or Mu Xiang (Aucklandiae Radix, 0.8 fen); for Blood stasis/astringency, add Hong Hua (Carthami Flos, wine-fried, 1-2 qian).
Tongyu Jian (Tongyu Decoction): Treats female Qi stagnation and Blood accumulation, meridian obstruction, severe pain with refusal to press, postpartum Blood stasis solid pain, and male/female Blood rebellion or Blood syncope.
Ingredients: Dang Gui Wei (Angelicae Sinensis Radix caulis, 3-5 qian), Shan Zha (Crataegi Fructus), Zhi Xiang Fu (Cyperi Rhizoma processed), Hong Hua (Carthami Flos, new ones fried yellow, 2 qian each), Wu Yao (Linderae Radix, 1-2 qian), Qing Pi (Citri Reticulatae Pericarpium viride), Ze Xie (Alismatis Rhizoma, 1.5 qian each), Mu Xiang (Aucklandiae Radix, 0.7 fen).
