痛瘳。
The son of Mr. Wan Su'an suffered from fever for eleven days, with thirst, the center of the tongue colored like agarwood and extremely dry, extreme heat on the forehead, heat in both hypochondriac regions, slight deafness in the ears, no sweat had been produced previously, although he had defecated twice, the heat was not significantly reduced, urine was extremely scanty, consciousness was clouded, feet were cold, and there was no desire for food. Local physicians exhausted their skills and again sought my help upon urgent request. Upon examination, the pulse at the middle position on the left hand was rapid and forceful, while the right hand pulse was weak and soft. Based on the symptoms, this was diagnosed as a concurrent disease of Yangming (Yangming Syndrome) and Shaoyang (Shaoyang Syndrome). It was appropriate to use Chai Ge Tang (Bupleurum and Pueraria Decoction) to clear the heat, observe whether sweating occurs, and then adjust the prescription according to the changing condition. Prescription: Bupleuri Radix (Chaihu) 5 qian, Puerariae Lobatae Radix (Gegen) 3 qian, Paeoniae Radix Alba (Baishao), Gypsum Fibrosum (Shigao) each 2 qian, Ginseng Radix (Renshen), Cimicifugae Rhizoma (Shengma), Trichosanthis Radix (Tianhuafen) each 1 qian, Glycyrrhizae Radix et Rhizoma (Fencao) 7 fen, decocted in water and taken orally. The next morning, upon examination, the rapid pulse had slightly slowed, the heat had slightly subsided, but the tongue and teeth remained dry, urination was difficult, and mental clarity was still clouded. Therefore, the formula was changed to include Bupleuri Radix (Chaihu), Glycyrrhizae Radix et Rhizoma (Fencao), Trichosanthis Radix (Tianhuafen), Scutellariae Radix (Huangqin), Ginseng Radix (Renshen), Atractylodis Macrocephalae Rhizoma (Baizhu), Poria (Fuling), Talcum (Huashi), Akebiae Caulis (Mutong), Alismatis Rhizoma (Zexie). In the afternoon, slight fever returned, the face became red, and the forehead was painful and heavy. Immediately, Yi Yuan San (Yiyuan Powder) 3 qian was administered. One bowel movement occurred, which was loose and yellow in color; the heat remained severe, and the face remained red. After taking the previous medicine, urination occurred twice. Upon re-examination, the Cun (Inch) and Guan (Carpal) positions of the pulse were approaching harmony, but the Chi (Cubit) positions on both sides were still surging and large. It was then understood that the pathogenic heat was in the Lower Jiao (Lower Burner), and only by promoting urination could it be resolved. Again, Chen Sha Yi Yuan San (Cinnabar Yi Yuan Powder) 5 qian was given. Thirst slightly stopped, the lower lip became moist, but the upper lip remained dry; consciousness was clouded, sleeping but not waking, and eye luster did not respond to external stimuli. It was further determined that the heat was in the Pericardium Collateral (Xin Bao Luo). It was suitable to use Tao Jian'an's Dao Chi San (Guide Out Red Powder). After two doses, mental clarity returned, although urination was still not smooth. Si Ling San (Four-Poria Powder) was added with wine-processed Coptidis Rhizoma (Jiu Lian), Hordeum Vulgare Sprout (Maiya), and Akebiae Caulis (Mutong), decocted and taken. After another two doses, all symptoms subsided, and dietary intake improved.
The wife of Grand Secretary Mr. Wan Lu'an experienced pain under the right hypochondrium, coughing with a dry throat, occasional hematemesis (vomiting blood), sometimes a bowlful, sometimes half a bowlful, borborygmus (intestinal rumbling) with diarrhea, aged over sixty. The cause was attributed to head wind disease damaging the eyes, with an irritable temperament. The left pulse was wiry and rapid, the right pulse was slippery and rapid. She was prescribed Trichosanthis Semen (Gualou Ren) 2 qian, Coptidis Rhizoma (Huanglian), Peucedani Radix (Qianhu), Platycodonis Radix (Jiegeng), Aurantii Fructus Immaturus (Zhike), Citri Reticulatae Pericarpium (Juhong), Fritillariae Bulbus (Beimu), Poria (Baifuling) each 8 fen, and Glycyrrhizae Radix et Rhizoma (Gancao) 5 fen. The next day, bleeding continued, though hypochondriac pain halved. The prescription was changed to Gardeniae Fructus (Shanzhiren), Moutan Cortex (Mudanpi), Cyperi Rhizoma (Xiangfu), Fritillariae Bulbus (Beimu), Glycyrrhizae Radix et Rhizoma (Gancao), Trichosanthis Fruit (Gualou), Asteris Radix (Ziwan), Talcum (Huashi), decocted in water. Just as I was trying to find true Curcuma Longa Rhizoma (Yujin) but could not obtain it, Mr. Tang Ning'an, who was the lady's son-in-law, happened to arrive and heard of this, responding immediately: "I have it." He took Curcuma Longa Rhizoma (Yujin), ground three fen, and mixed it with the herbal decoction to drink. After two doses, bleeding stopped and pain alleviated.
