The nephew of Jiu De suffered from tinnitus (tinnitus), with a sensation of Qi rushing and blocking without free flow, nasal congestion and discomfort, loss of taste in the mouth, excessive phlegm, and a feeling of heat and turbidity in the chest and diaphragm. The pulse was floating and wiry-large on the left, slippery-large on the right, and rapid in both positions. The *Neijing* states: "Headache, tinnitus, and dysfunction of the nine orifices are generated by the stomach and intestines." This condition arises from the upward obstruction of Phlegm-Fire in the stomach, where extreme heat generates wind. Therefore, Jing Miao Zi (Vitex Fruit), Sheng Ma (Cimicifugae Rhizoma), Mu Tong (Akebiae Caulis), Chi Fu Ling (Poria with Red Core), Sang Bai Pi (Mori Cortex), Mai Men Dong (Ophiopogonis Radix), Sheng Di Huang (Rehmanniae Radix), Qian Hu (Peucedani Radix), Gan Ju Hua (Chrysanthemi Flos), Chi Shao Yao (Paeoniae Radix Rubra), Gan Cao (Glycyrrhizae Radix et Rhizoma) were prescribed, along with three slices of Sheng Jiang (Zingiberis Rhizoma Recens) and one Zao Ren (Jujubae Fructus), decocted in water and taken orally for four doses. Although the wiry quality of the left pulse decreased by half, the symptoms remained as before. Subsequently, Gan Ju Hua, Ju Hong (Citri Reticulatae Pericarpium Rubrum), Ban Xia Qu (Pinelliae Fermentatum), Fu Ling (Poria), Gan Cao, Zhi Mu (Anemarrhenae Rhizoma), Bai Shao Yao (Paeoniae Radix Alba), Jiu Huang Qin (Scutellariae Radix prepared with wine), Ma Huang (Ephedrae Herba), Shi Gao (Gypsum Fibrosum), Sang Bai Pi, Jie Geng (Platycodonis Radix) were added to Sheng Jiang and Zao Ren, and taken for another four doses, after which all symptoms subsided completely. Later, Liu Jun Zi Tang (Six Gentlemen Decoction) was modified with Jiu Lian Lian (Coptidis Rhizoma prepared with wine), Chai Hu (Bupleuri Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Bai Shao Yao, Mai Men Dong, and Sheng Ma, taken for two doses, restoring the ability to enjoy sweet tastes in food.
Mr. Chen Jueyu, a native of Changshan County, aged forty-three, was obese and had suffered from Phlegm-Fire syndrome for over ten years. It occurred once every month, or twice if mental exertion was excessive. Symptoms included expectoration of phlegm, fever, roaring asthma, anorexia, inability to lie flat to sleep, and delirious speech when closing the eyes. He presented with a red face, headache, wandering Phlegm-Qi throughout the body causing pulling pain, and only after expectorating phlegm would his tinnitus cease and tearing stop. Previously, he took wind-phlegm resolving herbs such as Nan Xing (Arisaematis Rhizoma Preparatum) and Ban Xia (Pinelliae Rhizoma) without effect. Later, taking Ren Shen (Ginseng Radix et Rhizoma) only provided relief for four to five months. His pulse was flooding and slippery at both Cun (inch) positions, and deep and faint at both Chi (cubit) positions, indicating a pattern of Excess above and Deficiency below. The treatment principle should be to clear the upper and tonify the lower. Ju Hong, Bei Mu (Fritillariae Bulbus), Fu Ling, Gan Cao, Jie Geng, Xing Ren (Armeniacae Semen Amarum), Qian Hu, Gou Teng (Uncariae Ramulus cum Uncis), Tian Ma (Gastrodiae Rhizoma), Jiu Huang Qin, Zhi Ke (Aurantii Fructus Immaturus) were decocted in water and taken orally. At night, Qi Zhi Hua Tan Wan (Seven-Processed Phlegm-Resolving Pill) was administered. Additionally, Ba Wei Wan (Eight-Ingredient Pill with Rehmannia) was modified with Ren Shen, Mai Men Dong, and Wu Wei Zi (Schisandrae Chinensis Fructus), taken on an empty stomach. After half a year, he recovered.
