The wife of Mr. Cheng Liangfeng from Youxi suffered from bloody dysentery (blood in stool with tenesmus), accompanied by tenesmus (a feeling of incomplete defecation with urgency and rectal heaviness), vomiting, and inability to retain food. The pulses at both cun positions were surging and large. She was administered Zhuru Wendan Tang (Bamboo Shavings and Pinellia Decoction for Harmonizing the Gallbladder) with added Jiang Lian (Ginger-processed Coptis Rhizome), Hua Shi (Talcum), Ju Hong (Aged Tangerina Peel), and Jiu Qin (Wine-processed Scutellaria Root). After two doses, the stool became formed, pus and blood ceased, but tenesmus remained. Subsequently, Zhi Qiao (Bitter Orange Fruit), Dang Gui (Angelica Sinensis), Shao Yao (Peony Root), Sheng Di Huang (Rehmannia Root), Tao Ren (Peach Seed), Tiao Qin (Scutellaria Root), Gan Cao (Licorice Root), Hua Shi (Talcum), and Jiu Lian (Ginger-processed Coptis Rhizome) were given. After four doses, she fully recovered.
Mr. Cheng Qi Hu's elder brother from Youxi experienced pain on the right side of his umbilical and abdominal region, scanty urine, no bowel movement for four days, vomiting preventing food intake, a white coating on the tongue, cyanotic complexion, and cold hands and feet; the condition was critical. The left pulse was deep and hidden, while the right pulse was slippery, large, and forceful. I stated: This is phlegm obstructing the middle jiao and qi closure in the lower jiao, causing constipation and difficult urination, along with qi rebellion leading to vomiting. If not treated urgently, it will be incurable. He was given Er Dong Xing Gen Pi (Root Bark of Eucommia Ulmoides growing on the east side) 2 qian, Hua Shi (Talcum) 3 qian, Tao Ren (Peach Seed), Qing Pi (Citrus Aurantium Peel), Zhi Shi (Immature Bitter Orange Fruit), and Bing Lang (Areca Seed) each 1 qian, decocted in water and taken orally. At midnight, he vomited over a bowl of viscous phlegm, but the bowels remained unopened and pain persisted. The next day, Yan Hu Suo (Corydalis Rhizome) 5 qian was used, decocted in water, with Xuan Ming Fen (Mirabilite) 3 qian mixed in just before ingestion. Taken at Chen hour (7-9 AM), by Wu hour (11 AM-1 PM), the pain decreased significantly. By Wei hour (1-3 PM), the bowels finally moved, the right pulse normalized, and the left pulse emerged. Feeling weak and fatigued, he was regulated with Sheng Mai San (Generate the Pulse Powder) added with Gan Cao (Licorice Root), Shan Zhi Ren (Gardenia Seed), Huang Bo (Phellodendron Bark), Shao Yao (Peony Root), Yi Yi Ren (Coix Seed), and Chen Pi (Aged Tangerina Peel), after which he returned to normal.
Mr. Cheng She Gui from Youxi first became drunk, then engaged in sexual intercourse. Early the next morning, his limbs were cold, and he suffered extreme epigastric pain. His pulse was only four beats per cycle (approx. 40-50 bpm). The previous physician treated him for constrained fire, administering cold-natured herbs, which exacerbated the pain significantly. The watermelon eaten three days prior was still being vomited undigested. His father suspected Yin-type Cold Damage (Shanghan) and wished to use Fuzi Lizhong Tang (Aconite Decoction to Regulate the Middle), but hesitated. Seeking my advice instead, I observed his cyanotic complexion, loud screaming cries due to pain but with a suppressed voice, and restlessness in sitting or lying down. I diagnosed this as Huoluan (Cholera-like syndrome) combined with Hui Jue (Roundworm-induced syncope/cold extremities). First, pain relief and roundworm calming are necessary, followed by managing Huoluan to avoid death; delay would be fatal. Urgently, Wu Ling Zhi (Trogopterus Dung) stir-fried with vinegar 3 qian, Cang Zhu (Atractylodes Rhizome) 1.5 qian, Wu Mei (Mume Fruit) 3 pieces, Chuan Jiao (Sichuan Pepper), Pao Jiang (Prepared Ginger), and Gui Xin (Cinnamon Twig Heart) each 0.5 fen were decocted in water and drunk. The pain decreased significantly. In the afternoon, Da Fu Pi (Areca Pericarp), Huo Xiang (Patchouli), Ban Xia (Pinellia Tuber), Chen Pi (Aged Tangerina Peel), Shan Zha (Hawthorn Fruit), Wu Ling Zhi (Trogopterus Dung), and Fu Ling (Poria) were prescribed. After two doses, he fully recovered.
