Pediatric Sha Syndrome presents in Yin and Yang types. Yin Sha manifests as abdominal pain with cold hands and feet; Yang Sha manifests as abdominal pain with warm hands and feet.
Some symptoms resemble cold but are not true cold, or resemble heat but are not true heat, accompanied by limb fatigue, loss of appetite, and a miserable facial expression. The manifestations vary, commonly referred to in folklore as Sha disease.
However, it is not true Sha disease; it is mostly caused by contracting wind-cold from exposure and dampness from playing in water. The treatment method involves applying hot water-soaked cloths to the arms and frequently scraping the skin with ramie fiber (Zhumá).
Scraping should continue until red petechiae appear on the skin. In severe cases, pricking the dorsal aspect of the ten fingers near the nail beds (approximately one fen) is appropriate. One may first stroke the child's arm from the elbow towards the hand to gather blood at the fingertips, thereby restoring the smooth flow of Qi and Blood.
Regarding Pediatric Intestinal Twisting Sha (severe pain in the heart, abdomen, and waist), use Huolong Dan (Fire Dragon Pill).
Ingredients: Xionghuang (Realgar), Yanxiao (Saltpeter/Niter) (each 2 qian).
Grind together into a fine powder. For each application, use a hairpin to pick up a small amount of powder and apply it to the inner canthus of the eye. For males, apply to the left eye; for females, to the right eye. Recovery is immediate.
Shenxian Wanli Wan (Immortal Ten Thousand Pill Pill)
Ingredients: Zhusha (Cinnabar), Badou (Croton Seed, husked), Hanshi Mian (Cold Food Flour).
First, grind Zhusha until smooth. Then grind Badou together with Zhusha until extremely fine. Next, mix Hanshi Mian with good wine to form a dough, incorporate the medicinal powder, and grind over a hundred times more.
Knead and roll into pills the size of millet seeds. The dosage is only three, five, or seven pills. Adjust according to deficiency or excess conditions, and use the following decoctions as guides for administration:
For contraction of wind-cold, administer with Ginger-Scallion Decoction to induce sweating.
For internal injury due to diet, administer with clear tea.
For heart pain, administer with Mugwort Decoction mixed with a little vinegar.
For latent summer-heat, administer with cold water.
For chest distension and qi stagnation, administer with light Ginger Decoction.
For cholera with vomiting and diarrhea, administer with Ginger Decoction.
For dysentery, administer on an empty stomach with clear tea.
For abdominal pain, administer with hot tea.
For malaria, administer with Ginger Decoction.
For acute or chronic convulsions (Jing Feng), administer with Mint Decoction.
For miscellaneous diseases, administer with clear tea.
Taiyi Hunyuan Dan (Great Unity Primordial Pill)
Ingredients: Ziheche (Placenta Humanum, dried, 3 qian), Baimihua (White Plum Blossom, 3 qian), Chensha (Vermilion Cinnabar, 1 liang, boiled with Licorice Root 1 liang for half a day, discard Licorice), Huashi (Talcum, 6 liang, boiled with Moutan Cortex 2 liang until the water evaporates and the bark is dry, discard Moutan), Xiangfumi (Cyperi Rhizoma, boiled through with honey, 1 liang), Fencǎo (Licorice Root, 2 liang), Gansong (Nardostachys Radix, 4 qian), Ezhu (Curcuma Zedoaria Rhizome, fire-roasted, 3 qian), Sharen (Amomi Fructus, peeled, 3 qian), Yizhi (Alpinia Oxyphylla Fruit, husked, 6 qian), Shanyao (Dioscoreae Rhizoma, stir-fried with ginger juice, 2.5 qian), Renshen (Ginseng Radix et Rhizoma, de-stumped, 1 qian), Huangqi (Astragali Radix, honey-fried, 1 qian), Baifuling (Poria, 3 qian), Baifushen (Poria with Pine Root, de-barked, 2.5 qian), Yuanzhi (Polygalae Radix, soaked in Licorice water, de-heart, 1.5 qian), Jiegeng (Platycodonis Radix, de-stumped, 1 qian), Muxiang (Aucklandiae Radix, 1 qian), Shexiang (Moschus, 3 fen), Niuhuang (Bovis Calculus, 2 fen), Tianzhuhuang (Sinarundinariae Nodulus, 1 qian). Another version lacks Hunyuan Yi (Gold Leaf Coating) and Meihua (Plum Blossom).
Grind all ingredients into a fine powder. Bind with refined honey into pills the size of longan seeds. Coat with gold leaf. Dosage varies by age. This formula treats various diseases in children, adults, and women.
For wind-stroke with phlegm syncope and unconsciousness, dissolve and administer with Ginger Decoction.
For cold damage complicated by fright and fever, dissolve and administer with Ginger-Scallion Decoction; induce sweating.
For food stagnation with vomiting, abdominal distension, and sour-smelling stool, administer with Ginger Decoction.
For cholera, administer with Perilla and Papaya Decoction; for diarrhea, administer with Rice Decoction.
For red-and-white dysentery, administer with aged rice decoction.
For cough and asthma, administer with Ephedra and Apricot Kernel Decoction.
For accumulation and abdominal pain, administer with Ginger Decoction.
For worm pain, administer with Toosendan Root Decoction.
For hernia with unilateral testicular swelling, administer with Small and Large Fennel Decoction.
For incessant night crying, administer with Rush Ash Decoction.

For chronic convulsions, administer with Ginseng and Atractylodes Macrocephala Decoction.
For hematochezia (blood in stool), administer with Sophora Flower and aged rice decoction.
For urinary retention, administer with Plantago Seed Decoction.
For nocturnal spontaneous sweating, administer with Floating Wheat Decoction.
For fever, administer with Golden Coin Mint Decoction.
For measles/rubella that fail to erupt, administer with Cimicifuga Decoction.
For heatstroke with irritability and thirst, administer with Centella Decoction.
For infantile malnutrition (Gan syndrome) with emaciation, large abdomen, thin limbs, or dribbling, diarrhea, swelling, distension, asthma, or cough, administer with aged rice decoction.
Discussion on pediatric diseases: For children with surplus strength and robust constitution, Wanli Pill is suitable. For children with deficiency and weak constitution, Xionghuang Jiedu Wan (Realgar Detoxification Pill) is suitable.
Ingredients: Xionghuang (Realgar, 2.5 qian), Yujin (Curcumae Longae Radix, 2.5 qian), Badou (Croton Seed, 24 kernels, oil removed).
Grind into powder. Bind with vinegar paste into pills the size of mung beans. Take seven pills per dose, swallowed with hot clear tea.
For sudden wind-stroke with collapse, locked jaw, opisthotonos, and unconsciousness, swallow with clear tea.
For pediatric acute wind-stroke with phlegm-heat, add Bovis Calculus (0.5 fen) and Borax (1 qian), bind with water paste, and administer with Mint Decoction.
For strangling throat wind (acute laryngeal obstruction), take seven pills, dissolve them, and administer mixed with white plum blossom using hot water, or with hot tea. Awakening occurs immediately. This utilizes the principle that heat promotes circulation. If death occurs suddenly from strangling throat wind but the heart remains warm,灌服 (force-feed) this medicine to restore consciousness.
For malaria with palpable masses (pi), administer with ground Agarwood water. Black blood like mud, extremely foul-smelling, will be expelled. Effect is immediate.
For various accumulations with diarrhea, administer with Wu Ling San (Five-Ingredient Powder with Poria) decoction.
For carbuncles/boils (Ding Chuang), add Scorpion (1 qian), Gleditsia Sinensis Fruit (1 qian), and Moschus (a little). Bind with drops of water into pills. Take twenty pills per dose with tea. If initially unrecognized as Ding Chuang and no sweating was induced, and after several days it is realized that chills, fever, and body pain have ceased, indicating toxins have entered the interior, purgatives must be used to drain internal toxins. This pill treats such cases.
For scrofula/ulcers, add Seven Blister Beetles (Banmao), wings and feet removed, stir-fried with glutinous rice. Administer with cold tea. It clears all heat-toxin congestion, resolves accumulations, aids digestion, and clears heat. Administer with tea.
