(Sound hoarseness, Mumps)
When the Cun (Inch) pulses of both hands are floating, surging, and overflowing, it is Hou Bi (Throat Obstruction). If the pulse is faint and sunken, the prognosis is fatal.
The *Nei Jing* (Inner Canon) states: "When one Yin and one Yang knot, it is called Hou Bi." The term "one Yin" refers to the Hand Shaoyin Jun Huo (Ministerial Fire), which is the pulse qi of the Pericardium. The term "one Yang" refers to the Hand Shaoyang Xiang Huo (Ministerial Fire), which is the pulse qi of the San Jiao (Triple Burner). These two meridians both connect to the throat. If Qi becomes hot, it knots internally; severe knotting leads to swelling; severe swelling leads to obstruction; severe obstruction leads to death. Investigating the twelve meridians, only the Foot Taiyang Bladder Meridian descends to the nape, while the others converge at the throat. Why does the *Nei Jing* specifically state that the knotting of one Yin and one Yang causes Hou Bi? This is because Jun Huo and Xiang Huo prevail exclusively with heat, attacking the collaterals, so pain arises rapidly. I believe this can be summarized in one word: Fire. Therefore, among the twelve meridians, symptoms such as dry throat, sore throat, swollen throat, swollen jaw, and stiff tongue base are all caused by Jun Huo. Only the rapid onset of throat obstruction is caused by Xiang Huo. Jun Huo is like human fire; Xiang Huo is like dragon fire. Human fire burning wood has a slow momentum; dragon fire burning wood has a rapid momentum. The Hou Bi mentioned in the *Nei Jing* lies between the pharynx and the tongue. Since the pathology is the same regarding fire, no distinction is made. Later physicians detailed various conditions and arbitrarily established eight names: Dan Ru E (Unilateral Tonsillitis), Shuang Ru E (Bilateral Tonsillitis), Zi She Zhang (Sublingual Swelling), Mu She Zhang (Wooden Tongue), Chan Hou Bi (Entangling Throat Obstruction), and Zou Ma Hou Bi (Racing Horse Throat Obstruction). Rising heat transmits to both sides of the throat, causing external swelling. Due to its shape resembling a swallow's breast, it is called Ru E. One side is Dan (Single); both sides are Shuang (Double). Those slightly smaller than Ru E are called Hou Bi. Heat knotting under the tongue generates a small secondary tongue, called Zi She Zhang. Heat knotting in the middle of the tongue causes it to swell, called Mu She Zhang, which is stiff and not flexible. Heat knotting in the throat causes swelling around the exterior, accompanied by numbness and itching; if the swelling is large, it is called Chan Hou Feng (Entangling Throat Wind). Sudden onset and death in Hou Bi is called Zou Ma Hou Feng (Racing Horse Throat Wind). Although these eight names are detailed, if they are not attributed to Fire, they deviate significantly from the root cause. Mild cases can be treated with salty substances to soften hardness; severe cases require acrid substances to disperse. Current physicians possess herbs such as Bo He (Menthae Haplocalycis Herba), Wu Tou (Aconiti Radix), Jiang Can (Bombyx Batryticatus), Bai Fan (Alumen), Pu Xiao (Natrii Sulfas), and Tong Lv (Chalcanthum). However, for Zou Ma Hou Bi, reliance on these is insufficient. Life and death hang in the balance. The method least likely to cause error is Bian Zhen (Bloodletting with a stone needle or three-edged needle). When blood flows, the disease resolves. The *I Ching* (Book of Changes) says, "When blood leaves, fear departs," which likely refers to this principle. Once, I treated a woman with Mu She Zhang whose tongue filled her mouth. Other physicians failed. I used a small, sharp silver needle to puncture the area five to seven times. The swelling subsided and resolved completely in three days. The blood loss amounted to nearly a dou (a unit of volume). Previously, I treated a man with Chan Hou Feng swelling, where both interior and exterior were affected, and medicine could not be swallowed. I administered cold medicine via nasal instillation, resulting in more than ten bowel movements, and applied Ba Du San (Detoxication Powder) externally. Yang Qi Shi (Stibnite) was calcined and quenched, mixed with an equal amount of Fu Long Gan (Dragon's House Earth), ground into fine powder, and stirred one hundred times with fresh water. After three days, the heat receded and swelling subsided. I also treated a noblewoman with Hou Bi, which was Dragon Fire. Although cold formulas were used, they must not be taken cold. Dragon Fire should be driven out by fire. Human fire refers to cooking fire. I had her stand in the scorching sun and ascend to a high hall. Family members carried a brazier, placing the medicine pot on top to keep the decoction extremely hot but not boiling. She sipped it continuously through the mouth over one hundred times. The fire dispersed naturally. This method uses heat to move cold, avoiding rejection by heat. Generally, using needles to bleed is the best strategy for treating Hou Bi. However, people fear needles and seek alternative methods, leading to sudden death. For any needle wound, crushed ginger block mixed with boiled water should be sipped frequently to promote healing. The *Tong Ren* (Bronze Man) texts also describe moxibustion, but it is suitable only for mild pain; for rapid-onset cases, delay may be fatal. Treating Hou Bi is akin to saving lives; there is no room for hesitation. The *Nei Jing* states, "For constrained fire, release it." "Release" refers to sweating. How can there be sweat in the throat? Thus, bleeding is a form of releasing sweat. Future scholars should not cling to minor prescriptions claiming they do not affect the viscera and excel at bleeding. If fortunate to encounter minor illnesses and achieve success, but unfortunate to encounter major illnesses and die, do not leave regrets.
Discussion on critical Hou Bi, where death occurs in an instant, with locked jaws. On the outer side of the patient's thumb, at the root under the nail, regardless of gender (left for males, right for females), use a cloth-wrapped needle to prick until blood flows for immediate effect. In critical cases, prick both thumbs for greater efficacy.

If not Hou Bi, it will dissipate naturally. For various tongue swellings, use 0.5 qian, dip finger in powder, rub on tongue, and swallow saliva. For children, divide 1 qian into four or five doses, following the same method, without regard to timing.
Bo (also known as Ma Bo), a fungus. It is soft like purple cotton; when bounced, purple-gray dust emerges. It resembles a dog's liver and grows in damp, decaying garden areas. It treats malignant sores and horse mange. Apply to sores, mix with honey, and sip to treat throat obstruction and pain.
