Cough, as described in the *Neijing* (Classic of Internal Medicine), is caused by the Lung channel being affected by slight cold. If coughing occurs between the seventh and eighth months, a time when Lung Qi is flourishing, the disease must be excess type, not a chronic condition. The symptoms include red face, abundant phlegm, and fever; treatment should involve purgation. Qian Yi used Tingli Wan (Descurainia Seed Pill). For chronic cough, purgation is contraindicated. If coughing occurs in the eleventh or twelfth month, it is due to injury by wind-cold. Wind-cold enters through the Feishu (BL13) acupoint at the third thoracic vertebra on the back. Symptoms include restlessness, aversion to wind, and chills, with symptoms lighter during the day and heavier at night; treatment should involve diaphoresis (sweating). Qian Yi used Mahuang Tang (Ephedra Decoction). If accompanied by heat signs, there is a red face, desire for water, and phlegm-saliva obstructing the throat causing difficulty; Qian Yi combined this with Ganjie Tang (Licorice and Platycodon Decoction). If coughing occurs in the fifth or sixth month, symptoms include fever, abundant sticky saliva/phlegm, or abundant phlegm without severe wheezing, red face, or occasional desire for water. Qian Yi used Pianyin Wan (Bitter Silver Pill) for purgation in all these cases. If Lung Qi is excessive, coughing is followed by wheezing, fever, restlessness, and edema, regardless of desire for water, Qian Yi used Xiebai San (White Herb Powder) to drain the Lung.
Previously, Qian Yi treated the nine-year-old grandson of Mr. Zhang, who suffered from Lung-heat cough. Previous physicians had treated him with cooling medicines such as Pearl, Rhinoceros Horn, Dragon Bone, Musk, and Realgar for one month without success. Symptoms included coughing, rapid wheezing, restlessness, incessant drinking, and complete inability to eat. Qian Yi prescribed Shijunzi Wan (Quince Seed Pill) and Yihuang San (Yellow Protecting Powder). Mr. Zhang asked, "There was originally heat; why use warming medicines?" Other doctors had used cooling medicines to attack the pathogen for a month with no effect. Qian Yi said, "Long-term use of cooling medicines causes Stomach cold and inability to eat. Since the child is deficient and cannot eat, we must tonify the Spleen first. Once eating returns to normal, we will drain the Lung meridian, and the illness will surely heal." After two days of taking Spleen-tonifying medicine, the child desired food. Then, using Xiebai San to drain Lung heat, he recovered. Mr. Zhang asked, "Why did this not cause deficiency?" Qian Yi replied, "By first solidifying the Spleen and then draining the Lung, there is no deficiency."
In cases of wind-cold injury causing cough without heat signs, only coughing, Qian Yi used Mahuang Tang (Ephedra Decoction) along with phlegm-resolving medicines. If Lung Qi is deficient, and coughing is accompanied by gasping and frequent deep sighs with sounds in the throat, this is a chronic condition requiring tonification. Qian Yi used Ejiao San (Donkey-Hide Gelatin Powder). Previously, Qian Yi treated the five-year-old son of Mr. Du, who had been coughing from November to March without cessation. Initially, coughing with expectoration indicated wind-cold entering the Lung. With wind in the Lung, coughing and expectoration occurred; dispersal with Ephedra-type medicines was appropriate. Later, cooling medicines suppressed it, and it healed. However, other doctors used various purgatives, causing Lung deficiency and worsening cough. By the third month of spring, it had not healed. Upon consultation, the pulse showed a bright green complexion, coughing with rapid wheezing, gasping, and frequent deep sighs. Qian Yi said, "The condition is critical; eight or nine out of ten parts of the disease are already lost. This is because a bright green complexion indicates flourishing Liver Qi. The three months of spring correspond to the Liver's domain and the time of Lung decline. Cough is a Lung disease; lasting from November to March leads to Lung deficiency and flaccidity. Having been purged, both Spleen and Lung (mother and child organs) are deficient, and they are overcome by Wood (Liver). This is a counter-indication (Ni), hence the coughing, rapid wheezing, gasping, and deep sighs." Qian Yi urgently administered Xieqing Wan (Draining Green Pill) to drain the Liver, followed by Ejiao San to solidify the Lung. The next day, the complexion was green but not bright. Qian Yi then tonified the Lung, but the cough persisted. He drained the Liver again, but the disease did not cease, and Lung deficiency worsened, with lips white as silk. Qian Yi said, "This disease is fatal and incurable. Why? The Liver is excessively flourishing while the Lung is exhausted. The Lung disease does not occur at its proper time, yet the Liver overcomes it. Now, after draining the Liver three times, the Liver pathology remains; after tonifying the Lung three times, the Lung pathology becomes even more deficient. Life will not last long, hence I say death." For this syndrome, if contracted in autumn, three or four out of ten can be saved; if contracted in spring or summer, fewer than one out of ten can be saved. Indeed, the patient died of severe asthma.
Regarding insufficient Lung Qi, affected by cold evil, manifesting as cough, wheezing, chest fullness, and shortness of breath, treatment should tonify the Lung. Previously, Qian Yi treated the eight-year-old grandson of Transport Commissioner Li, who suffered from cough, wheezing, chest fullness, and shortness of breath. Other doctors believed there was heat in the Lung meridian and treated him with Zhuye Tang (Bamboo Leaf Decoction) and Niuhuang Gao (Calculus Bovis Ointment). After three days, wheezing worsened. Qian Yi diagnosed: "This is insufficient Lung Qi with cold evil, immediately causing wheezing and chest fullness. Tonify Lung and Spleen; do not take cooling medicines." Mr. Li said, "We have already taken Zhuye Tang and Niuhuang Gao." Qian Yi asked, "How were they treated?" The previous doctor said, "To reduce heat and resolve saliva." Qian Yi asked, "What heat?" The doctor said, "Heat in the Lung meridian generates cough; prolonged cough generates saliva." Qian Yi said, "It is originally deficiency caused by wind-cold, what heat is there? If it were Lung heat, why not treat the Lung, but instead regulate the Heart? Zhuye Tang and Niuhuang Gao are both Heart-treating medicines." The doctor looked ashamed. Qian Yi cured the disease based on his discussion.
For coughing up pus and blood, it is Lung heat. Qian Yi used Ganjie Tang (Licorice and Platycodon Decoction). For coughing with phlegm-saliva and wheezing, first solidify the Spleen (Qian Yi used Yihuang San), then mildly purge (Qian Yi used Pianyin Wan). After saliva recedes, tonify the Lung (Qian Yi used Ejiao San). Previously, Qian Yi treated the four-year-old son of Duan Zhaolang, who had cough, fever, expectoration for several days, and hemoptysis. Previous doctors used Jiegeng Tang (Platycodon Decoction) and Fangji Wan (Stephania Pill) without success. Saliva attacked upward, causing incessant vomiting and wheezing. Qian Yi administered a large dose of Pianyin Wan to purge, then treated with Spleen-tonifying medicine. Someone asked, "This child has hemoptysis and Lung deficiency; why purge?" Qian Yi said, "Although the Lung hemoptysis is due to heat, prolonged heat leads to deficiency and flaccidity. Now saliva surges upward with vomiting; we must purge the saliva. If we do not allow vomiting of saliva, the condition will worsen. Vomiting saliva causes deficiency and may generate fright. Phlegm-excess attacking upward can also cause convulsions. Therefore, according to the method, it is appropriate to first purge the phlegm. This treatment of 'purge first, tonify later' contrasts with the earlier discussion of 'tonify first, purge later.' The Classics state that diseases have urgency, and treatments have sequence. Treating like Qian Yi allows one to grasp the intent of the Sages; students should emulate this." Coughing up green-white watery fluid indicates Cold Stomach with retained fluid retention. Qian Yi used Baixiang Wan (Hundred Auspicious Pills) to purge. Coughing up phlegm-saliva and milk/food indicates undigested old stagnation. Qian Yi used Baibing Zi (White Cake Pill) to purge.
For chronic cough with Lung deficiency and loss of Jin-Ye (Body Fluids), Qian Yi used Ejiao San (Donkey-Hide Gelatin Powder) to tonify. The general principle for treating cough: Excess requires purging; chronic conditions require tonifying; Wind requires dispersing. One must assess age, size, and deficiency/excess to apply treatment wisely. Therefore, care for children must be cautious; they should always wear padded vests. Even in hot summer months, add an extra layer to the back of the single vest. Since Feishu (BL13) is on the back, we fear wind-cold injuring it and causing cough. Prolonged cough may also induce fright. For infants under one hundred days with cough, only one or two out of ten survive; it is not a minor illness.
Another type is retained fluid transforming into phlegm. Due to improper feeding, Spleen and Stomach harmony is disrupted, retained fluids fail to disperse, and remain to form phlegm. If this rises with Qi to disturb the Lung causing cough, it is called Phlegm-Cough. If there is no cough, the Lung is congested and unsmooth, causing throat obstruction with saliva, and stagnation in the hypochondrium, abdomen, and diaphragm.
If Spleen and Stomach cold and heat are imbalanced, saliva fails to return to the Stomach, flowing from the mouth corner ceaselessly. Cold type presents with blue color on the nose and green-white stool; Heat type presents with red color on the nose and red-yellow stool. Besides the formulas used by Qian Yi recorded elsewhere in this collection, the following formulas are listed.
