Commentary: Mr. Haicang stated that slow eruption on the posterior body involves the Foot Taiyang Meridian; use schizonepeta herb (Jingjie, 荆芥), licorice (Gancao, 甘草), and saposhnikovia root (Fangfeng, 防风). Slow eruption on the anterior body involves the Hand Yangming Meridian; use Shengma Gegen Tang (Aucklandia and Kudzu Decoction). Slow eruption on the limbs involves the Foot Yangming Meridian; use Fangfeng Shaoyao Gancao Tang (Saposhnikovia, Peony, and Licorice Decoction). These are all agents for resolving toxins and promoting ascent and eruption. This knowledge is essential.

Take 3 qian per dose, decoct in water. For one-year-olds, take 1–2 spoonfuls; for three-to-five-year-olds, take several spoonfuls.
Commentary: The preceding formulas are bitter-cold and acrid-dispersing, acting to vent the exterior and unblock the internal organs. They are most suitable for excess heat in both interior and exterior, characterized by irritability, preference for cold drinks, constipation, and urinary retention. Do not overdose, lest stomach qi be further damaged and other syndromes arise. Improper use of diaphoresis may cause ulceration; improper use of purgatives may lead to collapse.
Huisu Jiu (Coriander Wine): Treats foul qi hindering the eruption of smallpox.
Take one bunch of coriander (Huisu, 胡荽), decoct in two cups of good wine until boiling once. Let the nursing mother hold one or two mouthfuls in her mouth and spray the infant’s entire body, evenly covering the head and face. Burn coriander incense in the room to dispel foul qi and facilitate rapid eruption. Hanging the decocted coriander on the door is optimal.
Commentary: This formula is highly appropriate. If the rash has erupted but appetite is low, use honey-roasted jujubes. The scent of dates opens the stomach qi and promotes eating. Regarding detoxification, if food stagnation has not been resolved, using this formula may exacerbate evil qi, generating damp-heat and forming smallpox toxin.
Ganlu Yinzi (Sweet Dew Drink): Treats accumulated heat, post-smallpox sore throat, mouth and tongue ulcers, and swollen gums.
Fried raw rehmannia root (Shengdihuang, 生地黄), heartless and roasted ophiopogon tuber (Mendong, 麦门冬), prepared rehmannia root (Shudihuang, 熟地黄), heartless asparagus tuber (Tiandong, 天门冬), fried scutellaria root (Huangqin, 黄芩), dendrobium stem (Shihu, 石斛), bran-fried bitter orange peel (Zhike, 枳壳), hirta loquat leaf (Pipaye, 枇杷叶), yinchen (Yinchen, 茵陈), honey-fried licorice (Gancao, 甘草) (equal parts).
Take 3 qian per dose, decoct in water. Administer 3–5 spoonfuls per dose; do not overconsume.
Commentary: This formula cools blood, resolves toxins, removes dampness, and clears heat; it is a cold-natured agent. Practitioners must exercise careful judgment.
Tuoli San (Support Interior Powder): Treats smallpox toxin with weak primordial qi, or when harsh treatments have failed to disperse it. Used to resolve unformed pus or rupture formed pus.
Ginseng (Renshen, 人参), fried astragalus root (Huangqi, 黄) (each 2 qian), wine-washed angelica sinensis root (Danggui, 当归), white atractylodes rhizome (Baizhu, 白术), tangerine peel (Chenpi, 陈皮), prepared rehmannia root (Shudihuang, 熟地黄), poria (Fuling, 茯苓), fried peony root (Shaoyao, 芍药) (each 1.5 qian), honey-fried licorice (Gancao, 甘草) (5 fen).

Shenxiao Ge Suan Jiu Fa (Miraculous Garlic-Separated Moxibustion Method): Treats intense smallpox toxin preventing eruption, suppuration, or crusting, or causing severe pain/numbness. Moisten pain until numbness, numbness until pain; toxin disperses with fire. In the capital, practitioners used thread needles to puncture and drain toxic blood, allowing suppuration. If puncture yields no pain/blood, it is hard to treat. With moxibustion, pain returns; puncture then yields purple blood, allowing suppuration and recovery. Method: Slice garlic 3 fen thick, place on lesion, apply small moxa cone on garlic. Change garlic every five moxa units. If purple blood appears and swelling/pain persists, moxibustion is especially indicated. Practitioners must judge carefully.

Take 3 qian per dose with one dried persimmon, decocted in rice wash water until dry. Eat the persimmon without core, three times daily, anytime.
Yu Taihe, a Daoist alchemist from Chengdu, treated critical smallpox with black depression using one house spider (Xi Zhi, 喜蜘) the size of a mung bean, pounded. Dosage of realgar (Xionghuang, 雄黄): 1 li for 1-year-old, 2 li for 2-year-old, 1 qian for 10-year-old. Mix with spider, blend with a cup of strong liquor, and administer slowly. I believe this formula resembles the previous twelve-ingredient formula and the Eleven-Ingredient Yigong San. For yang deficiency-cold causing cold limbs, abdominal distension, blue lips, and black depression, use strong liquor. For primordial qi deficiency with pale color hiding in skin, aged wine is also suitable; do not restrict to strong liquor. For infants under one year, adjust dosage appropriately; do not restrict to cup measures. For dual blood-qii deficiency with dull red color, failure to erupt, or fright/grinding teeth, boil arnebia root (Zicao, 紫草) and safflower (Honghua, 红花) in aged wine until concentrated, decoct, and administer to preserve life. Practitioners must exercise judgment.
Baolong Wan (Embracing Dragon Pill): Treats phlegm-heat wheezing/cough, fever, fright/restlessness, and can induce smallpox eruption.
Arisaema with bile (Danning, 胆星), bamboo slime (Tianzhuhuang, 天竺黄), realgar (Xionghuang, 雄黄), cinnabar (Zhusha, 朱砂) (each 5 qian), musk (Shexiang, 麝香) (small amount).
Powder finely. Boil 1 liang of licorice in water for juice. Make pills. Each liang makes twenty pills. Dissolve in mint or centella water before administration.
Method for Preparing Arisaema: Take any amount of arisaema (Nanxing, 南星), wash with winter water in the twelfth lunar month, slice, dry, and powder. Mix with bile of a yellow ox. Fill the empty gallbladder shell with the mixture, tie shut, hang in a windy place to dry. Use only after one year.
Commentary: The preceding formulas clear heat, resolve phlegm, and regulate qi. Overdose damages spleen and lungs, worsening the condition or causing chest/abdominal distension and vomiting. Use Renshen Baizhu San (Ginseng and Atractylodes Powder) to tonify middle qi.
Wufu Huadu Dan (Five Blessings Detoxifying Pill): Treats fetal toxin and post-smallpox sores on head/face, eye swelling/pain.
Raw rehmannia root (Shengdihuang, 生地黄), prepared rehmannia root (Shudihuang, 熟地黄), heartless asparagus tuber (Tiandong, 天门冬), heartless ophiopogon tuber (Mendong, 麦门冬), scrophularia root (Xuanshen, 玄参) (each 3 liang), licorice (Gancao, 甘草), saltpeter (Tianxiao, 甜硝) (each 3 liang), indigo naturalis (Qingdai, 青黛) (1.5 liang).
Powder the ingredients. Make pills with refined honey the size of Chinese bayberry. Take one pill per dose, dissolve in warm water.
Commentary: This formula generates and cools blood, resolves toxins; it is a cold-natured agent. When used appropriately, it is highly effective, requiring only one or two pills.
Xijiao Jiedu Wan (Rhinoceros Horn Detoxifying Pill): Treats accumulated heat and post-smallpox/rubella sores from residual toxin.
Raw rehmannia root (Shengdihuang, 生地黄), saposhnikovia root (Fangfeng, 防风), angelica sinensis root (Danggui, 当归), rhinoceros horn shavings (Xijiao Xie, 犀角屑), schizonepeta herb (Jingjie, 荆芥) (each 1 liang), pounded and fried burdock fruit (Niubangzi, 牛蒡子), red peony root (Chishaoyao, 赤芍药), forsythia fruit (Lianqiao, 连翘), platycodon root (Jiegeng, 桔梗) (each 7 qian), mint (Bohe, 薄荷), fried scutellaria root (Huangqin, 黄芩), licorice (Gancao, 甘草) (each 5 qian).
Powder the ingredients. Make pills with refined honey the size of Chinese bayberry. Take one pill per dose with mint water.
Commentary: This formula clears heat, vents exterior, cools blood, breaks stasis, and detoxifies; it damages stomach qi. Do not exceed one or two doses. Generally, for smallpox toxin, refer to previous methods for cold water ingestion errors or Shaofen San treatment. Follow this pattern.
Baicao San (Decayed Grass Powder): Treats sores caused by excessive scratching of smallpox, with dripping pus and blood, termed "ban lan" (ulceration). Use rotten roof grass or wall-covering grass aged for years; wild grass is best if unavailable. Powder and apply externally.
Commentary: Some cases fail to heal due to blood/qi deficiency-heat. For widespread cases, sprinkle generously on mats for the child to sit/lie on; sores will heal.
Yanggan San (Sheep Liver Powder): Treats smallpox entering the eyes or innocent malnutrition affecting the eyes.
Centella leaf (Mimenghua, 蜜蒙花), cornel fruit (Qingzixian, 青箱子), cassia seed (Juemingzi, 决明子), fried plantago seed (Cheqianzi, 车前子).
Powder the ingredients. Mix 3 qian of mimenghua powder with 1 qian each of the others. Slice a large sheep liver thinly, sprinkle mixture on it, wrap in wet material, bake/cook, and eat on an empty stomach.
Commentary: For liver meridian wind-heat damaging blood, use this formula. For liver blood deficiency with wind agitation, use Siwu Tang (Four Substance Decoction) plus gardenia fruit (Shanzhi, 山栀), Uncaria hook (Gouteng, 钓藤钩), and moutan bark (Mudanpi, 牡丹皮). For liver blood deficiency generating wind or kidney water failing to nourish liver wood, use Liuwei Wan (Six-Ingredient Pill). For liver malnutrition, use Liuhuang Dihuang Wan (Six-Ingredient Rehmannia Pill) to nourish liver/kidney, and Simei Fei'er Wan (Four-Ingredient Fattening Child Pill) plus ginseng and white atractylodes to tonify liver/spleen.
A child with smallpox developed eye opacity, fluctuating severity, unresponsive to opacity-clearing herbs. Pulse was wiry, thin, and rapid. This indicates liver-kidney malnutrition. I used...
