Lung heat causes distension and fullness, attacking the chest and diaphragm, resulting in *Guixiong* (Tortoise Chest). Additionally, if the nursing mother consumes excessive amounts of "Five Pungents" (five pungent spices: garlic, chives, leeks, scallions, and asafoetida), this condition can also arise. After birth, an infant may contract external wind pathogens that enter the spine and penetrate into the bone marrow, leading to *Guibei* (Tortoise Back).
Kidney heat transmits to the Bladder; when Bladder heat is excessive, it rebels upward affecting the Spleen and Stomach. If the Spleen and Stomach are deficient and cannot control Kidney water, water counter-attacks Earth (Spleen). The Spleen Qi follows the downward movement of Water. Since the Spleen governs the four limbs and the Lung is controlled by Heart fire, wheezing occurs. One might ask: If Heart fire penalizes Lung metal, and the Lung is originally deficient, why does wheezing manifest as an excess syndrome? The answer is twofold: first, severe Lung wheezing indicates rebellion of the Five Zang organs; second, Kidney water qi ascends and infiltrates the Lung laterally, causing severe wheezing. These conditions are difficult to treat.
Liver disease appearing in Autumn: Wood is prosperous and the Liver is strong, overcoming Lung Metal. Treatment should tonify the Lung and drain the Liver. In mild cases, the Liver disease recedes; in severe cases, the lips turn white and death ensues.
Lung disease appearing in Spring: Metal is prosperous and the Lung overcomes Liver Wood. Treatment should drain the Lung. In mild cases, the Lung disease recedes; in severe cases, the eyes appear pale cyan, indicating impending *Jingfeng* (Convulsive Disease). If redness is present, pulling therapies (*Chou*) should be applied, as this indicates Liver deficiency, which typically presents with pale cyan eyes.
Heart disease appearing in Winter: Fire is prosperous and the Heart is strong, overcoming Kidney Water. Treatment should tonify the Kidney and address the Heart. In mild cases, the disease recedes; in severe cases, the body curls downward and speech is lost, indicating Kidney deficiency and weakness.
Kidney disease appearing in Summer: Water overcomes Fire, meaning the Kidney overcomes the Heart. Treatment should address the Kidney. In mild cases, the disease recedes; in severe cases, palpitations and convulsions occur.
Spleen disease appearing in the remaining seasons: Treat according to the above principles. Smooth patterns are easy to treat; inverted patterns are difficult. Spleen deficiency manifests as a reddish-yellow complexion. When the Five Zang colors are inverted, treat according to the specific syndrome.
Eyes red with cyan tint indicate impending convulsions.
Eyes staring straight with cyan color, and body arched backward and stiff, indicate *Jingfeng*.
Severe teeth grinding indicates onset of *Jingfeng*.
Spitting foam and saliva indicates subsequent worm pain.
Drowsiness with frequent sneezing and palpitations indicates impending *Chuangzhen* (Poisonous Rash/Smallpox/Measles).
Vomiting, diarrhea, drowsiness, and eyes half-open (exposing the pupil) indicate Stomach deficiency heat.
Vomiting, diarrhea, drowsiness, and eyes tightly closed indicate Stomach excess heat.
Vomiting/diarrhea with undigested milk indicates food stagnation. Use purgative methods.
Spitting foam and phlegm, white or green, indicates Stomach deficiency cold.
Spitting thick saliva and blood indicates Lung heat; prolonged duration leads to deficiency.
Stool yellow, red, crimson, or black all indicate heat; crimson also indicates toxicity.
Stool green or white with undigested food particles indicates Stomach cold.
Body heat without thirst indicates heat on the exterior; body heat with thirst indicates heat on the interior.
Inability to open the mouth (*Jin*) leads to loss of voice. Sluggish speech is similar.
Failure to walk upon growing up, or thin legs when walking.
Delayed tooth eruption or loose teeth after eruption.
Delayed hair growth or black hair failing to grow dark.
Blood deficiency and weakness allowing cold pathogens to invade result in cyan lips.
Deep yellow urine, if prolonged, leads to hematuria.
Urinary retention, if prolonged, leads to abdominal distension; promote urination.
Umbilicus not drying after bathing allows wind pathogens to enter, causing sores and *Cuekou* (Opisthotonos/Tetanus); severe cases indicate Spleen deficiency.
Spitting phlegm-saliva with heat signs: use purgatives. Spitting phlegm-saliva with cold signs: use warming methods.
Pustules appearing before macules is an inverted pattern.
Pustules appearing before papules/rash is a smooth pattern.
Vesicles appearing before papules/rash is an inverted pattern.
Pustules appearing before vesicles: many is smooth, few is inverted.
Vesicles appearing before macules: many is inverted, few is smooth.
Papules/rash appearing before macules is a smooth pattern.
If pox/rash presents in only one type, prognosis is good.
Fetal essence substantial: red face, large black pupils, frequent laughter.
Fetal essence deficient: yellow face, small black pupils, large white sclera, frequent crying.
For diseases initially presenting as deficiency, or requiring purgation: if the pattern requires purging, first tonify the "Mother" organ (e.g., tonify Spleen to support Lung), then purge. For example, Lung deficiency with Phlegm excess: first benefit the Spleen, then drain the Lung.
Excessive joy followed by feeding often leads to *Jingxian* (Convulsive Epilepsy).
Excessive crying followed by feeding often leads to vomiting and diarrhea.
Heart pain with spitting water indicates worm pain.
Heart pain without spitting water indicates Cold Heart Pain.
Spitting water without heart pain indicates Stomach cold.
Severe illness with five-colored facial changes lacking luster indicates death.
Yawning with red face indicates Wind-Heat.
Yawning with cyan face indicates Convulsive Wind.
Yawning with yellow face indicates Spleen deficiency with fright.
Yawning with drowsiness indicates internal heat.
Yawning with hot breath indicates Wind-Cold injury.
After treating heat syndromes with draining, releasing, or resolving methods, if no deficiency signs remain, do not use warming tonification, or heat will inevitably re-emerge.
