The fu organs function normally, and diet is unchanged, indicating that the fire toxin is mild. If the skin and flesh are broken, external application of detoxifying powder will also be administered; this is a cautious method, followed by the external application of Hupo Gao (Amber Plaster).
Fifty-eight: For cases where arms, fingers, feet, legs, or shoulders have been severed, it is crucial to address the condition while blood heat rises. Whether the injury involves the hand or arm, the entire body's qi and blood must be unblocked. The wound should then be sealed with Jinqiang Yao (Golden Gun Medicine), internally administered Tuoli Zhitong San (Support Interior and Stop Pain Powder), followed by conditioning agents and medicinal wine for stabilization.
Fifty-nine: In cases where bones and joints are crushed by falling bridges, walls, or city ramparts, if the head is injured, one must check for bone fragments. If present, remove them with copper forceps; failure to remove fragments may lead to complications and prevent wound closure. The primary concern is tetanus infection; thus, Hufeng Liqi Tang (Protect Wind and Regulate Qi Decoction) must be administered first, followed by Jiangu Dan (Bone-Connecting Pill). If both Taiyang acupoints are injured causing coma, inability to eat, speak, or swallow liquids, the case is incurable. Extrusion of brain matter is also incurable. Fracture of the Tianjing bone is incurable. If chest and back injuries damage the five zang organs, speech and appetite will cease, but rescue is still possible because qi stagnation is in the heart; urgently administer Jili San (Auspicious Powder) mixed with Amomum villosum (Sharen, 砂仁) decoction. If the patient can tolerate this, rescue is possible. If chills and fever develop, urgently administer Hufeng Liqi Tang. If the patient cannot tolerate it, observe for two days and attempt Jili San again as before. If still intolerant, the case is incurable. For injuries to both soft ribs, assess severity: if diet is normal and no chills/fever occur, treat with Jili San. If chills/fever occur, administer Sufeng Liqi Tang (Soothe Wind and Regulate Qi Decoction). For kidney (lower back) injuries, severe cases are incurable; mild cases without skin breakage should have external application of Sunshang Gao (Trauma Plaster) and internal administration of Bushen Hexue Tang (Tonify Kidney and Harmonize Blood Decoction) along with medicinal wine for recovery.
Sixty: For blunt trauma causing inversion or purple discoloration, affecting vital acupoints, with locked jaw and eyes unable to open, first prepare fine powder from roasted Amomum villosum (Sharen, 砂仁), brew in water, and let the patient drink to regulate qi. Then, administer Jili San mixed with light ginger decoction.
Sixty-one: For head injuries with frontal/temporal lacerations and coma, roast Radish seeds (Hongluobozi, 红萝卜子), grind into powder, brew in water, and drink. Subsequently, administer Jili San mixed with light ginger decoction. For severe injuries using the aforementioned powder, use 2 qian; for mild injuries, 1 qian is optimal. Avoid wind exposure, as tetanus infection is a risk. After drinking the Amomum villosum decoction, immediately administer one dose of Sufeng Liqi Tang to protect against wind.
Sixty-two: For kicks or falls injuring the small intestine, causing painful stiffness, urinary obstruction, and inability to walk, there is definitely internal blood stasis. Urgently administer Guitong Poxue Tang (Regulate Intestines and Break Blood Stasis Decoction) for stabilization. If urination is partial (two or three times), rescue is possible; otherwise, defecation will delay, making treatment difficult after prolonged duration, though early intervention remains effective. The area behind the anus and scrotum is named Haidi Xue (Sea Bottom Acupoint). If kicked or severely impacted, assess severity: swelling, cyanosis, blackness, purple hue, redness, or mixed red-purple colors with unbearable pain require internal administration of Xingqi Huoxue Zhitong Tang (Move Qi, Invigorate Blood, and Stop Pain Decoction), external application of trauma medicine, followed by Jili San. If swollen, cyanotic, black, accompanied by chills/fever, urinary retention, testicular ascent/descent, mental confusion, and lower abdominal pain, internal blood stasis is confirmed. Administer Sufeng Xingqi Huoxue Tang, followed by Hupo San (Amber Powder), external application of Sunshang Gao, and Jili San. For anal swelling, urinary/fecal obstruction, day-night fever, anorexia, and restlessness, first administer Sufeng Shunqi Tang, followed by Hupo Wan (Amber Pill). For wheezing, coughing, emotional instability (laughing/crying), urinary difficulty, persistent red/swollen tissues, and paroxysmal pain, first administer Bushen Huoxue Zhitong Tang, followed by Jili San, and finally Bushen conditioning medicinal wine. Another critical sign is immediate loss of speech, unconsciousness, salivation, throat/nasal gasping, deep/thin pulse, and pale complexion; this is a ominous sign. If chest/axillary movement persists, treatment may be attempted: insufflate a small amount of Glycyrrhiza uralensis root powder (Zhuyazao, 猪牙皂) into the nose. If ineffective, use a rush pith stick dipped in powder to induce tearing, then administer via Amomum villosum decoction. Subsequently, continue with Jili San mixed in Amomum villosum decoction, followed by Sufeng Liqi Huoxue Zhitong Tang. If no fever or prior symptoms occur, long-term use of Bushen Liqi Shunqi medicinal wine ensures recovery. If symptoms recur, follow previous protocols. Apply Sunshang Gao to the injury site. For slight pain, only Jili San mixed in Amomum villosum decoction and plaster application are needed for cure; no other medicines are required.
