According to *Chao Shi Bing Yuan* (Source of Diseases by Master Chao) regarding the condition of double tongue in infants: The occurrence of double tongue (a swelling resembling a second tongue under the main tongue) in children is caused by heat in the Heart and Spleen. The Heart opens into the tongue and governs blood; the collateral vessels of the Spleen also emerge from beneath the tongue. The Heart and the Spleen (Earth element) have a mother-son relationship. When heat is present, both Qi and Blood become hyperactive. The manifestation involves growth attached beneath the tongue, extending to the root, forming a structure similar to a tongue but shorter, hence termed "double tongue."
The *Qian Jin Fang* (Essential Prescriptions Worth a Thousand Gold) states: Six or seven days after birth, an infant's Qi and Blood converge to form flesh, at which point the oral cavity, tongue, throat, and cheeks should remain clear. If there are foreign substances in the throat or on the tongue, resembling water held within a reed sheath, with swelling protruding beneath the tongue, this is called double tongue. Such cases require pricking to release blood stasis.
*Hui Yan Guan Zheng* records: Conditions such as thrush (oral candidiasis), double tongue, double palate, and mouth sores in newborns are all caused by heat in the Upper Jiao (Upper Burner). This results from excessive heat exposure during fetal development. Immediately powder Ji Nei Jin (Gallus Gallus Domesticus Brisson Gizzard Lining) and dry-dust it into the mouth. Apply Zhusha Gao (Cinnabar Ointment) (formula seen in the Convulsion Section) and Dihuang Gao (Rehmannia Ointment) (formula seen in this section) alternately. Additionally, administer Tianzhu Huang San (Bambusa Textilis Calamus Powder) internally.
*Dihuang Gao* from *Hui Yan Guan Zheng*: A formula for treating thrush, double tongue, and double palate in newborns.
Jin Yu (Curcuma aromatica Rhizome), boiled with Gleditsia Sinensis Fruit juice until dry, cut finely, and dried (0.5 liang); Dou Fen (Starch) (0.5 liang); Gan Cao (Licorice Root), roasted (1 fen); Ma Ya Xiao (Natrii Sulfas) (1 qian, ground).
Mix these with fresh Sheng Di Huang Juice (Rehmanniae Radix Fresh Juice) and bee honey. Place approximately 2 fen of liquid mixture in a cup and boil down to an ointment consistency. Take a dose the size of a soap bean, dissolve in warm cooked water; or apply with a goose feather quill inside the mouth.
*Jin Zhu Yin Zi* from *Hui Yan Guan Zheng*: Treats convulsions, high fever, latent heat from cold damage, deficiency heat in the Upper Jiao; as well as double tongue, mouth/nose sores/rashes causing red eyes. (Originally named Tianzhu Huang San.)
Chuan Yu Jin (Curcuma aromatica Rhizome), crushed, boiled with Gleditsia Sinensis Fruit juice until dry (fine texture like gall bladder lining is best); Tian Zhu Huang (Bambusa Textilis Calamus); Gan Cao (Licorice Root), roasted; Ya Xiao (Natrii Sulfas), separately ground (each 0.5 liang); Zhu Sha (Cinnabaris) (1 fen, ground); Chan Ke (Cicada Slough) (14 pieces, soaked in water to remove dirt); She Xiang (Moschus) (a small amount).
