[Water Phlegm and Ghost Stagnation] Total of nine sections
Song of the Infantile Phlegm-Fluid Syndrome from *Yingtong Baojian*:
Drinking water excessively or damaging milk (leading to indigestion) causes phlegm-fluids to knot in the chest. This prevents the infant from nursing smoothly; vomiting froth resembles epilepsy. In excess patterns, the body often has fever, and delirious speech occurs during sleep. Early medical treatment is necessary; if prolonged, it transforms into convulsive disease (pediatric condition characterized by convulsions and altered consciousness).
Recorded in *Waitai Mi Yao*: Fan Wang stated that patients suffering from phlegm-fluids should be treated with warm herbs to harmonize them. This treats deficiency-cold in the heart and abdomen, wandering phlegm-qi rising upward, fullness in the chest and hypochondrium, poor appetite, vomiting and hiccups, and coldness in the chest. Banxia Decoction
Formula Composition:
Banxia (Pinelliae Rhizoma, one sheng, washed) Shengjiang (Zingiberis Rhizoma Recens, one jin) Jupi (Citri Reticulatae Pericarpium, four liang)
Cut the above three herbs, boil in ten dou of water until three sheng remain, and divide into three doses. If there is urgency in the heart and heart pain, add Rouguixin (Cinnamomi Ramulus et Cortex internal bark) four liang. If there is abdominal pain, add Danggui (Angelicae Sinensis Radix) four liang. It is excellent for emaciated individuals, the elderly, and children. Avoid mutton and maltose. Reduce the dose for infants.
Beishu Pill from *Wangshi Shouji*: Treats dampness invading the spleen and stomach, fluid retention under the heart (epigastrium), irritability, thirst, vomiting, gurgling sounds in the intestines, chest and diaphragm stuffiness and fullness, shortness of breath, distending pain in the abdomen and hypochondrium, difficulty in urination, puffy edema of the body and face, and complete loss of appetite.
Formula Composition:
Guigui (Cinnamomi Ramulus, a type of cinnamon) Ganjiang (Zingiberis Rhizoma, one liang each) Baizhu (Atractylodis Macrocephalae Rhizoma, two liang)
Grind the above herbs into powder, make pills with refined honey the size of mung beans. Take fifteen to twenty pills per dose, down with rice water. Reduce the number of pills for infants.
Dingxiang Kaiwei Pill from *Wangshi Shouji*: Treats dysfunction of the spleen and stomach, accumulation of phlegm-fluids, vomiting and acid regurgitation, and chest and diaphragm stuffiness and oppression.
Formula Composition:
Banxia (Pinelliae Rhizoma, made into coarse powder, soaked in Shengjiang juice, stir-fried until yellow) Gancao (Glycyrrhizae Radix et Rhizoma, honey-fried) Jingsanleng (Curcumae Radix, processed, one liang each) Dingxiang (Caryophylli Flos, three fen) Gan Mugua (Chaenomelis Fructus, half liang) Shengjiang (Zingiberis Rhizoma Recens, twelve liang, sliced, ground, mixed with one liang of Qingyan (Green Salt), stir-fried moist, dried, and ground into powder)
Grind the above herbs into fine powder, make pills with refined honey the size of a chicken head (similar to Euryale Seed). Take one pill per dose, dissolve in boiling water, take at any time. Reduce the dose for infants.
Infantile Edema and Fullness Syndrome from *Chao Shi Bing Yuan*: The syndrome of edema and fullness in infants is caused by improper care, leading to deficiency in both the Kidney and Spleen. The Kidney governs water, its qi connecting downward to the yin organs. The Spleen governs earth, controls muscles, and restrains water. Kidney deficiency fails to transport fluids, and Spleen deficiency fails to restrain water, causing water flow to overflow into the skin, resulting in edema and fullness. Those accompanied by edematous swelling have thin skin resembling a ripe plum. If the skin is affected by wind pathogen, where wind binds and qi stagnates causing swelling, it is all deficiency-swelling like being blown up, called Wind Qi Swelling.
Mr. Mao discusses: Infants may present with Middle Qi Swelling syndrome from birth. The whole body is yellow and swollen, full of edema, abdominal discomfort, and rapid breathing. This syndrome results from immoderate eating or chronic diarrhea leading to severe deficiency. Treatment involves first using Yunqi Powder (see section on Stomach Qi Disharmony) and Xingpi Powder to regulate for one day (there are two formulas: one see Stomach Qi Disharmony section, one see Slow Spleen Wind section), then administer Gansui Powder to purge five sheng or more of yellow water (see this section), followed by taking Yunqi Powder combined with Xingpi Powder. After five days, frequently use Taki Powder (see this section). Salt and vinegar must be abstained from for one month for recovery. If large panting, refusal to eat, scrotal swelling, and retracted lips appear, it is a death sign and untreatable.
Discussion from Han Dong Mr. Wang's *Jiabao*: Edema of the infant's head, face, hands, and feet, caused by Gan accumulation forming wind over a long period. This disease may result from chronic illness where medication fails to resolve symptoms, leading to thirst and preference for drinking water, transforming into two deficiency-floating syndromes. It is appropriate to first tonify with herbs regulating stomach qi, such as Guanyin San (see Stomach Qi Disharmony section) and Renshen San (see Food Loss and Cold Damage section), which will naturally resolve. If stomach-regulating herbs fail, proceed to Neixiao Wan. If still ineffective, administer Hongfen San to expel pathogens (both formulas see this section), then tonify with medication again.
Qian Yi's discussion: Edema disease involves Kidney heat transmitting to the Bladder; Bladder heat excess reverses to affect the Spleen and Stomach. Spleen and Stomach deficiency fails to control Kidney water, so water counter-restrains Earth, and the Spleen follows the water. The Spleen governs the four limbs, so water flows causing swelling of the entire body and face. Severe panting indicates a heavy condition. Why? Because Kidney water is excessive, restraining Spleen Earth, and overcoming Heart Fire upwards; Heart then restrains Lung Metal, and since Lung is restrained by Heart, panting occurs.
Question: If Heart restricts Lung, and Lung is originally deficient, why is the panting severe and presenting as an excess pattern? Answer: There are two cases: one is severe Lung panting due to reversal of qi in the five zang organs; the second is Kidney water qi ascending and flooding the Lung, causing severe panting. Both are difficult to treat.
Zhang Huan's discussion: After various diseases in infants, if dysentery does not heal or body edema appears, it is due to Spleen and Kidney weakness. The Kidney governs water, its qi not connecting to the yin; the Spleen governs earth, controls muscles, and restrains water. Spleen and Kidney deficiency fails to restrain water, causing water to overflow into the skin, leading to edema and fullness.
Hanlin Attendant Yang Daye asked: What is the cause of edema in the four limbs and panting/coughing in infants? Answer: Fluctuations of cold and heat in infants are normal. Once dietary regulation fails, hundred diseases gather. How much more so with excessive heat obstruction and various toxins? This accumulates small to large, blocking throughout the stomach tubes. Over time, accumulation turns deep. With constant mixed food intake, the Qi of the Triple Burner becomes blocked, limbs gradually swell, urine may be red-yellow, stool may be clear or constipated, leading to panting, bloating, and reduced appetite turning into emaciation. First, harmonize the Spleen and Stomach; second, promote urination via the Small Intestine to avoid injuring healthy qi.
Song of Infantile Qi-Swelling and Water-Edema from *Yingtong Baojian*:
Infant edema looks like being blown up, solely due to wind pathogen and qi disorder. If skin resembles a ripe plum, this is named Gan Water without doubt. Spleen deficiency and Earth failure cannot hold water; edema always stays in the skin. If swelling is red, it is caused by heat, all due to blood and qi having no return. Seeing such signs requires early differentiation; do not let errors lead to danger.
*Liu Shi's Formula from Waitai Mi Yao* for treating infant body fullness, rapid breathing, and inability to lie down:
Take Yuliren (Prunus japonica Seed), one he, grind to powder, mix with flour to make cakes, feed to the infant according to method. Mild diarrhea indicates cure.
Another formula from Liu Shi in *Waitai Mi Yao*:
