邪热上冲于头面则生疮,结于皮肤间则成瘰 。气结于颈下则成瘿,病久不瘥则成 ,甚则成疳,本根一也。

[Carbuncles, Ulcers, and Scrofula] Divided into eleven categories.
*Zhubing Yuanhou Lun* (Source of All Diseases) on the syndrome of pediatric carbuncles and sores: When the functions of the six fu-organs are disharmonious, cold pathogenic factors invade the skin. Cold condenses with blood, causing stagnation and obstruction, lingering between the meridians and collaterals, knotting and swelling to form a carbuncle. Its presentation involves local swelling, with thin, shiny epidermis. If heat pathogenic factors take advantage of this, and heat exceeds cold, muscle and blood rot and transform into pus. After the pus ruptures, if the sore does not heal, it is called "carbuncle and sore."
*Yangsheng Biyong* (Essentials for Health Preservation) discusses prescriptions for treating carbuncles and ulcers: Whenever a carbuncle or ulcer first appears, one must use herbs such as *Rhei Radix et Rhizoma* (Dahuang) to strongly promote bowel movement. After bowel movement is achieved, the patient will naturally feel comfortable; do not worry about physical weakness or distress. One might say, "Rather than suffering abdominal and back ulceration, scorching of the internal organs, profuse flow of pus and blood, perforation of holes, and death from various malignant conditions, it is better to die through purgation, let alone have a chance of survival." The ancient sages established laws, generally using formulas such as *Wuxiang Lianqiao Decoction* and *Loulu Decoction*. For those in remote areas or living in poverty who cannot afford these drugs, one may decoct only *Rhei Radix et Rhizoma* (Dahuang) and *Glycyrrhizae Radix et Rhizoma* (Gancao) to promote purgation. This must be done daily without interruption until the pus ruptures and vitality gradually returns, at which point herbs like *Astragali Radix* (Huangqi) should be taken to expel pus and relieve pain. These are well documented in *Qianjin Fang* (Prescriptions Worth a Thousand Gold) and *Waitai Miyao* (Secret Essentials of the Outer Chamber). Modern physicians who do not study classics are blinded by subjective opinions and fail to address the root cause. They claim, "Sores manifest on the exterior; how can one use purgatives?" resulting in frequent deaths, which is truly lamentable. Sun Simiao said, "In emergencies, decocting a single herb of *Rhei Radix et Rhizoma* (Dahuang) to achieve rapid purgation is a crucial method."
Zhang Huan cautiously notes: Pediatric carbuncles, ulcers, toxic swellings, furuncles, scrofula, tubercles, goiter, various fistulas, and gan-sores are no different from those in adults. The classics state: "When the five zang-organs are disharmonious, the nine orifices become obstructed; when the six fu-organs are disharmonious, qi and blood flow and knot to form carbuncles." All are caused by the binding and struggling of cold and heat pathogens. Superficial involvement constitutes a carbuncle; severe involvement constitutes an ulcer. Toxic swelling involves concurrent wind pathogens; swelling extending up to one cun is called a furuncle. Evil heat rushing upward to the head and face causes sores; knotting between the skin forms scrofula. Qi knotting below the neck forms goiter; prolonged illness that fails to heal forms fistulas; severely, it forms gan-sores. The root cause is identical.
*Qianjin Fang* (Prescriptions Worth a Thousand Gold) Loulu Decoction: A prescription for treating pediatric red-and-white carbuncles and ulcers caused by heat-toxin, as well as various erysipelas, sores, and furuncles.
Herbs: *Euphorbiae Humifusae Herba* (Loulu), *Forsythiae Fructus* (Lianqiao - *Zhouhou* uses *Cynanchi Atrati Radix* [Baiwei]), *Ampelopsis Radix* (Bailian), *Natrii Sulfas* (Mangxiao - *Zhouhou* uses *Paeoniae Radix Alba* [Baishao]), *Glycyrrhizae Radix et Rhizoma* (Gancao, honey-fried. Each 6 zhu), *Scutellariae Radix* (Huangqin), *Cimicifugae Rhizoma* (Shengma), *Immaturae Fructus Citri Reticulatae* (Zhishi, bran-fried), *Ephedrae Herba* (Mahuang, remove roots and joints. Each 9 zhu), *Rhei Radix et Rhizoma* (Dahuang, 1 liang).
The above ten herbs are cut into pieces, decocted in 1.5 sheng of water to yield 5 he of liquid. For infants aged 1 to 7 days, administer 1 he divided into three doses. For 8 to 15 days, 1.5 he divided into three doses. For 16 to 20 days, 2 he divided into three doses. For 20 to 30 days, 3 he divided into three doses. For 30 to 40 days, 5 he divided into three doses. (*Zhouhou* treats adults using *Euphorbiae Humifusae Herba* (Loulu), *Ampelopsis Radix* (Bailian), *Scutellariae Radix* (Huangqin), *Cynanchi Atrati Radix* (Baiwei), *Immaturae Fructus Citri Reticulatae* (Zhishi), honey-fried *Glycyrrhizae Radix et Rhizoma* (Gancao), *Cimicifugae Rhizoma* (Shengma), honey-fried *Glycyrrhizae Radix et Rhizoma* (Gancao), *Paeoniae Radix Alba* (Baishao), *Ephedrae Herba* (Mahuang) without joints, each 2 liang, and *Rhei Radix et Rhizoma* (Dahuang) 3 liang. Ten herbs decocted in 1 dou of water to yield 3 sheng. If other herbs are unavailable, using *Rhei Radix et Rhizoma* (Dahuang) for purgation is excellent. Erysipelas requires needle-lancing to release blood. *Jingxin Lu* lacks *Forsythiae Fructus* (Lianqiao) but includes *Anemarrhenae Rhizoma* (Zhimu), *Paeoniae Radix Alba* (Baishao), and *Rhinocerotis Cornu* (Xijiao) in equal parts.)

The above fourteen herbs are cut into pieces, decocted in 4 sheng of water. When the volume is reduced by half, add *Bambusae Caulis Succus* (Zhuli), and continue decocting to yield 1.2 sheng. For infants aged 100 to 200 days, administer 3 he per dose. From 200 days to one year, administer 5 he per dose. Another version excludes *Ephedrae Herba* (Mahuang).
