[Various Diseases of Cough] Divided into eleven categories.
*The Treatise on Pathogenesis and Symptomatology of Diseases* (Chao Shi Bing Yuan) on the syndrome of pediatric cough: Cough arises from wind-cold damaging the Lung. The Lung governs Qi, corresponds to the skin and hair externally, and its Back-Shu point is located on the back. If infants are undressed and exposed to cold, wind-cold invades the skin and hair, subsequently entering through the Lung Back-Shu point to damage the Lung. When the Lung is affected by cold, cough ensues. Therefore, after birth, an infant's back must be kept warm constantly; even in summer, a single-layer garment covering the back should be worn. If cough is induced by a cold back, it is untreatable in infants under one month old. For infants within one hundred days, only one or two out of ten may recover.
*Ying Ru Shu* discusses: The origin of cough disease lies in the infant being separated from the mother, living alone and crying. At this time, the child's Qi is unsettled. Feeding during crying causes both Yin and Yang energies to surge simultaneously, alternating between deficiency and excess, leading to pathogenic invasion of the Lung and Stomach, thus causing infant cough.
Mr. Wang of Handong's *Jia Bao* records treatments for cough syndromes: Infantile cough is caused by wind-cold damaging the Lung; it is untreatable in infants under one month. For those within one hundred days, only one or two out of ten survive completely. It is advisable to first administer Mai Tang San (Wheat Decoction Powder) two or three doses (formula see section on Cold Damage with Macules), followed by Jin Hua San (Golden Flower Powder) two or three doses (formula see this section), interspersed with Guanyin San (Guanyin Powder) two or three doses (formula see section on Disharmony of Stomach Qi). Generally, infants should be particularly cautious about avoiding wind to prevent inducing cough.
Qian Yi discusses cough: Cough results from the Lung being affected by slight cold. Between the eighth and ninth months, Lung Qi is vigorous; coughs occurring then are likely excess patterns, not chronic conditions. Symptoms include red face, profuse phlegm, and fever. The method is to purge using Ting Li Wan (Descurainia Seed Pill) (formula not seen); if chronic, purging is contraindicated. Coughs occurring in the eleventh and twelfth months are wind-invasion coughs, where wind enters via the Feishu (BL13) acupoint at the third thoracic vertebra on the back; these should be treated with Ma Huang Tang (Ephedra Decoction) to induce sweating (formula not seen). For cases with heat signs such as red face, hot saliva after drinking water, and throat discomfort, Gan Jie Tang (Licorice and Platycodon Decoction) should be added (formula see section on Excess Heat). Within five to seven days, if symptoms include fever, profuse phlegm, and sticky sputum, Pian Yin Wan (Pianyin Pill) should be used to purge (formula see section on Phlegm-Fluids). In cases of Lung excess, characterized by cough followed by wheezing, facial edema, desire for water or lack thereof, and body heat, Xie Bai San (Bai Du San/Bark-Purging Powder) should be used to drain (formula see section on Wheezing and Cough). For wind-invasion cough without heat signs after five to seven days, Ting Li Wan may also be used to purge (formula not seen), followed by phlegm-resolving medicines. In cases of Lung deficiency, characterized by cough with choking sensations, frequent deep sighs, and sounds in the throat—indicating chronic illness—E Jiao San (Donkey-Hide Gelatin Powder) should be used to tonify (formula see section on Wheezing and Cough). For profuse phlegm, strengthen the Spleen first, then mildly purge with Pian Yin Wan (formula see above). Once saliva subsides, tonify the Lung as described above. For cough accompanied by vomiting water or green-blue fluid, Baixiang Wan (Baixiang Pill) is used to purge (formula see section on Smallpox/Chickenpox Inversion). For cough with vomiting of phlegm-fluids or milk/food, Bai Bing Zi (White Cake Pill) is used to purge (formula see section on Convulsions). For cough with slight pus and blood, indicating Lung heat, Gan Jie Tang is taken after meals (formula see above). For chronic cough where Lung fluids are depleted, E Jiao San is used to tonify (formula see above). For cough with moderate phlegm-solidity, wheezing, red face, and frequent thirst, Pian Yin Wan may be used to purge (formula see above). The general principle for treating cough is: purge excess, tonify chronic deficiency, and adjust based on deficiency/excess judgment.
Qian Yi discusses fatal prognoses for cough: A five-year-old son of the Du family in Luoyang suffered from cough from November until March without cessation. Initially coughing up phlegm, it was due to external wind-cold invading the Lung meridian. Now, with lung disease causing cough and phlegm, the wind remains in the lungs. Treatment should have involved dispersing with Ma Huang-type herbs, followed by cooling medicines. However, contemporary physicians used purgatives like Tie Fen Wan (Iron Powder Pill), Ban Xia Wan (Pinellia Pill), and Pian Yin Wan, which depleted Lung Qi and worsened the cough. By spring, it remained unresolved. Qian Yi examined the patient and observed a blue, lustrous complexion, cough with rapid breathing, choking, and frequent deep sighs. Qian Yi stated: "The condition is critical, nine-tenths gone. The reason is the blue, lustrous face indicates hyperactive Liver Qi. Spring is the season of the Liver and the time of Lung decline. Cough is a Lung disease; prolonged illness from Nov to Mar leads to Lung deficiency and flaccidity. Furthermore, purging has damaged the Mother (Spleen) and Child (Lung) relationship. The Liver now overacts upon the Lung, creating a counterproductive pattern. Hence the cough, wheezing, choking, and sighing." Qian Yi urgently prescribed Xie Qing Wan (Drain Green Pill) to drain the Liver (formula see section on Wind-Fire/Convulsions), followed by E Jiao San to solidify the Lung (formula see above). The next day, the face was blue but not lustrous. Qian Yi tonified the Lung again, but cough persisted. He drained the Liver again; before the effect was complete, Lung deficiency worsened, and lips became white as silk. Qian Yi said: "This case is fatal and untreatable. Why? Liver Qi is excessively strong while Lung Qi is exhausted. The Lung disease occurs when it is weak, yet the Liver overacts. Having drained the Liver three times without resolving the Lung pathology, and tonified the Lung three times without improving Lung signs, death is imminent." He noted that autumn-onset cases had a 30-40% survival rate, whereas spring/summer cases had less than 10%. The child indeed died of severe dyspnea.
*Tong Ying Bao Jian* lists fatal signs of cough: Cough with eyes rolling upward and black-green stool.
*Wu Guan Guan Zhen Zhu Nang* on pediatric cough syndrome: Caused by irregular feeding damaging the Lung, or by speaking while feeding, causing Qi and milk to clash in the Lung, leading to rebellious Qi and cough.
*Hui Yan Guan Zheng*: Cough in infants from one month to one year, or older, is categorized into types:
1. Milk-induced cough: Caused by hot milk damaging the Lung. Treat only with Wen Fei San (Warm Lung Powder) (formula see section on Malnutrition Cough) and apply Ren Shen Gao (Ginseng Paste) to the lips (formula see this section).
2. Ah-Jia cough (wheezing cough): Rapid breathing and cough, morning eyelid edema. Administer Hun Jin Dan (Complete Golden Pill) (formula see section on Cough with Wheezing Sounds); dose according to size. After vomiting/phlegm discharge, administer Yun Qi Hua Xian (Regulate Qi and Transform Phlegm) and cool Heart/Lung medicines.
3. Malnutrition cough (Gan Sou): Due to chronic diarrhea causing Spleen/Stomach deficiency, upper cough with fever attacking Heart Qi. First use Gan Yao Lu Hui Wan (Malnutrition Aloe Pill) (commonly taken formula) combined with Wu Xi Wan (Black Rhinoceros Horn Pill) (formula see section on Fatty Malnutrition), followed by Heart-heat clearing and Fang Ji Wan (formula see section on Malnutrition Cough).
4. Cold Damage cough: Due to exogenous wind in the Lung with latent heat; frequent vomiting after cough. Treat with Jin Fen San (Gold Powder Powder) (formula see section on Wind-Cold Cough).
For all above types, if Spleen/Stomach Qi is excess and chest/diaphragm is severely congested, first use Xian Tang Wan (Fresh Decoction Pill) to promote drainage (formula see section on Acute/Chronic Convulsions).
Mr. Mao's Song on Fatal Signs of Pediatric Cough:
Cough with high chest and coarse breathing,
Eyes fixed looking upward, never returning.
Frequent passage of green and black stool,
Refusal to eat signals imminent death.
Ge Hong's *Zhou Hou Bei Ji Fang* (Emergency Prescriptions Kept Up the Sleeve) for pediatric cough:
Zi Wan (Asteris Radix) 6 fen, Bei Mu (Fritillariae Bulbus) 2 fen, Kuan Dong Hua (Farflos) 1 fen.
