(Chi Li, also known as Bloody Dysentery)
*Chao Shi Bing Yuan* (Source of Diseases by Chao Yuanfang) on the syndrome of pediatric hot dysentery: Children inherently possess deficient heat and are invaded by wind pathogens. When both wind and heat enter the large intestine causing diarrhea, it is classified as a hot-type diarrhea, specifically where water and grain are not separated (Shui Gu Li). If the stool color is yellow, it is termed Hot Dysentery.
*Chao Shi Bing Yuan* on the syndrome of pediatric bloody dysentery: Children harbor external pathogenic heat entering the meridians. Blood, affected by heat, flows and scatters into the large intestine. Due to intestinal deficiency leading to leakage, bloody dysentery occurs.
Song from *Shi Bi Jing* regarding the thirty-six types of internal bloody dysentery:
In cases of bloody dysentery, first observe if the eyes are open; this confirms that the heat has been developing for some time. Red lips, red face, and mouth ulcers (or alternatively, white tongue, red lips, and mouth full of sores) indicate more than a hundred bowel movements in a single day. Dripping sounds in the abdomen accompanied by stabbing pain, loss of appetite, and complete disruption of food intake suggest that spleen heat (or alternatively, poisoning) has reached its extreme. Carefully select effective medicinal materials.
This condition is caused by the accumulation of heat. One must remove the accumulation, cool the stomach, regulate qi, and restore diet. When treating dysentery, strictly avoid using strongly heating medicines, as these will induce sweating, swelling, and severe fatigue.
From *Xiao Er Xing Zheng Lun* (Discussion on Infant Forms and Syndromes), the song for the forty-eight syndromes regarding bloody dysentery is identical to the above, followed by: In this syndrome, if physicians mistakenly administer heating medicines, the child may suffer from closed eyes, red lips, inability to eat or drink, or fatal bloody dysentery. Heating medicines must absolutely be avoided. Instead, one should administer medicines that open the stomach and resolve spleen toxicity. With such treatment, peace can be restored within three to five days.
