(Author's commentary to clarify confusion)

It is said: Acute convulsion (Ji Jing) presents with fever (Fa Re), red eyes (Mu Chi), coarse breathing from mouth and nose (Kou Bi Qi Cu), and tidal accumulation of phlegm-saliva (Tan Xian Chao Yong). It occurs suddenly, and after the episode, the complexion returns to normal. (This does not occur without external contraction or internal injury; there are no cases of sudden fever, red eyes, coarse breathing, and phlegm obstruction arising from nowhere. This description of sudden onset is merely a simulated expression.) Some cases arise from fright (Jing), while others do not. Generally, this is due to Fire dryness and Liver Wood urgency. (In fact, cases caused by Cold damaging the tendons leading to tendon spasm are more common; one cannot attribute all cases to Fire dryness.) Therefore, the body must have heat prior to onset; there are no cases where the body is cool yet the condition erupts. (These two sentences are good, but one should investigate the source of the heat to establish solid evidence. Merely stating "the body has heat first" without distinguishing whether the heat is in the Exterior (Biao) or Interior (Li) is unconvincing.) The syndrome belongs to Yang, and it is appropriate to use cooling formulas (Liang Ji). (Is it not dangerous to use cooling agents without first dispersing the pathogen, risking drawing the pathogen deeper into the Interior?) Clearing Heat (Re) and resolving Phlegm (Tan) will naturally subside the Fright/Convulsion. Ancients stated that Phlegm generates Heat, Heat generates Wind (Feng), and Wind generates Fright. In reality, these are all diseases originating from the body's own Qi (Ben Qi). (On what basis is it known to be a disease of the body's own Qi?) People fail to understand that Wind arises internally (How can one know it does not come from the Exterior but arises entirely internally? This is not accurate). They stubbornly administer a mixture of herbs for Wind and Phlegm such as Tianma (Gastrodiae Rhizoma), Danxing (Arisaematis Cum Bile), Gualou (Trichosanthis Fructus), Beimu (Fritillariae Bulbus), Jiangcan (Bombyx Batryticatus), and Quanxie (Scorpio), but the treatment fails. (This is due to transmission through teaching, causing harm to the people—a regrettable matter throughout history.) Furthermore, using Niuhuang (Calculus Bovis) wax pills from Guangdong or Zixue (Purple Snow) damages the Yuan Qi (Primal Qi). When Yuan Qi is damaged and Deficient Phlegm forces upward, causing chest and diaphragm distension, they claim the syndrome has transformed into Jie Xiong (Chest Binding). Is there any logic in this? (This is like opening the door to invite thieves, drawing the pathogen inward, and failing due to misdiagnosis!)

It is said: Fright (Jing) is being startled. (Fright results from being startled.) Because the child previously had internal injury (Nei Shang) and then suffered external contraction (Wai Gan), the Lung orifices are confused by Phlegm, and the Heart Spirit (Xin Shen) has no master. Once startled, the condition immediately erupts. (Since it is known that previous internal injury and subsequent external contraction led to Lung orifice confusion by Phlegm and Heart Spirit lack of mastery, this is actually due to illness, not due to Fright. Now saying "once startled, it erupts" places emphasis on Fright. The practitioner treats Fright rather than Phlegm, ignoring internal injury and external contraction, engaging in treating an unfounded "Fright," abandoning the root for the branch. How can the harm be overstated?)

It is said: Fright arises from the Heart, Phlegm from the Spleen, Wind from the Liver, and Heat from the Lungs. This is a fixed principle. (Half true, half false; difficult to call it definite.) Excessive Heat generates Wind, excessive Wind generates Phlegm, and excessive Phlegm generates Fright. This is the inevitable trend of "Thief Pathogens" (Ze Xie). (Above it was said that Fright arises from the Heart, Phlegm from the Spleen, Wind from the Liver, and Heat from the Lungs, implying these arise from their respective Zang organs. Why are they now called "Thief Pathogens"? Is this not mistaking the host for the thief?) To treat Fright, one must first resolve Phlegm (Huo Tan); to resolve Phlegm, one must first dispel Wind; to dispel Wind, one must first clear Heat. But what comes first in clearing Heat? The Lungs govern the skin and hair (Pi Mao), which are the portals for Heat pathogens to enter and exit. (Here, external pathogens are explicitly mentioned, so they do not arise from the Heart, Liver, Spleen, or Lungs themselves.) When the Six Excesses (Liu Yin: Wind, Cold, Summer-heat, Dampness, Dryness, Fire) arrive, they affect the skin and hair and immediately invade the Lungs. (Previously it was said Wind arises internally; now it is explicitly stated that the Six Excesses come from outside. Who is right and who is wrong?) Are the Lungs originally a place that produces Heat? (The Lungs are a place of clear and descending function; how can they be seen as producing Heat? The Classics state: "Cold exposure and cold drinks damage the Lungs." If this portal allows Heat pathogens to enter and exit, why not Cold pathogens?) When Dryness, Fire, or Summer-heat pathogens enter, Heat is generated, and combined with existing Heat, the deficiency becomes apparent. When Cold, Wind, or Dampness pathogens enter, the Lung orifices become blocked. (When the Six Excesses first arrive, they only affect the skin and hair, which are governed by the Taiyang channel. If one were to disperse them, how could they easily enter the Lung orifices and cause blockage?) Thus, Heat has no outlet to vent, and Heat becomes excessive. If clearing Heat requires first removing the pathogen... (Above it was said: Treat Fright by resolving Phlegm, resolve Phlegm by dispelling Wind, dispel Wind by clearing Heat, clear Heat by removing the pathogen. Did we not fail to remove the pathogen initially, leading to Heat generation, Wind generation, Phlegm generation, and Fright? Now starting again from releasing the Exterior and removing the pathogen, then clearing Heat, dispelling Wind, resolving Phlegm, and calming Fright—why not have released the Exterior and removed the pathogen at the beginning? Would that not have resolved everything instantly, avoiding such complexity? The errors of pediatric sedation and diarrhea cessation are fully evident here.)

It is said: Acute convulsion is due to Blood deficiency in the Liver Channel, with Fire agitation generating Wind. (This changes the subject completely.) Generally, when Wind arises, Yin Blood scatters further, and Yin Fire blazes more fiercely. (Yin deficiency leads to blazing Yang Fire; but what exactly is this "Yin Fire" referring to?) Fire agitation makes Lung Metal more deficient, and Liver Evil becomes more rampant. One should nourish Liver Blood and tonify Spleen Qi. If repeatedly taking Wind-dispelling, Phlegm-resolving, and Fire-draining agents yields no effect... (If it were truly Fire excess generating Wind, these agents would not be misuse. The fact that they fail suggests the syndrome is inaccurate and the medicine does not match the disease, leading to futile speculation. Ultimately, it remains ineffective.), one should readily recognize it as Spleen deficiency and Blood damage. (Readily recognizing implies a lack of firm insight; the previous descriptions were ambiguous.) Urgently tonify Spleen Earth. (Urgently tonifying Spleen Earth indicates that misdiagnosis has led to transformation, requiring immediate action. Otherwise, why the urgency?)

It is said: Acute convulsion involves excess in Wood, Fire, and Earth. (Repeatedly mentioning Wood evil insulting Earth, Wood flourishing and Earth declining, how can there be upper excess syndromes?) When Wood is in excess, spasms are forceful, eyes look upward, and blinking is frequent. When Earth is in excess, there is fever, red face, no vomiting or diarrhea, lying supine with closed eyes. Treatment should involve cooling and purging. There are also cases triggered by fright... (Triggered by fright indicates Spirit deficiency. This is Fire deficiency, not Fire excess.), leading to locked jaw, high fever, etc. This is internal Excess Heat combined with external Wind pathogen. (Is this Wind internal or external?) One should stop Wind and calm spasms. ("Stopping Wind and Calming Spasms" is far superior to legendary swords like Ganjiang and Moxie. Today, all pediatricians use this sharp tool.)

It is said: Acute convulsion presents with high fever, phlegm obstruction, wandering gaze, opisthotonos, spasms, locked jaw, hot breath, red cheeks, red lips, and floating, surging, rapid pulse. This is Liver Evil Wind-Heat, a syndrome of Yang excess and Yin deficiency. (Floating, surging, rapid pulse, drinking cold water, constipation clearly indicate a Shaoyang/Taiyang Cold Damage syndrome. Without proper resolution, Heat pathogens entered the Interior, causing tension in both Exterior and Interior, manifesting the above symptoms. Without clearing the Interior, the Exterior cannot be resolved. How can one still call this Acute Convulsion?)

It is said: Pediatric convulsion is a Liver disease, but also a Kidney, Heart, Spleen, and Lung disease. (Most texts state that since children's Tian Gui is insufficient, the Kidney does not primarily govern disease; only Heart, Liver, Spleen, and Lungs do. Suddenly mentioning the Kidney is indeed groundbreaking.) Since children's True Yin is insufficient, Softness cannot balance Rigidity, so Liver Evil moves easily. (The body's Ying and Wei pulses begin in the Hand Taiyin Lung at Yin hour, circulate fifty times, ending in Foot Jueyin Liver at Chou hour, and reconnect at Yin hour. It is the meeting place of Yin and Yang, the origin of True and Evil. Yet pediatrics labels it as Liver Evil, implying no organ in humans is free from Evil.) Wood generates Fire, Fire generates Wind. Wind and Heat battling lead to Blood deficiency, and Blood deficiency leads to tendon spasm. (Cold damaging Yin Ying often causes tendon spasm.) Tendon spasm leads to dizziness, tremors, opisthotonos, and stiffness, which are fundamental diseases of Liver Wood. (This originates from the *Neijing*'s discussion of Taiyang tendon diseases, yet is used to explain the facial expressions of convulsion, which is laughable.) Regarding transmission: Wood evil insulting Earth causes Spleen disease; Wood flourishing and Metal declining causes Lung disease; Liver Fire flaming upward causes Heart disease; Liver and Fire damaging Yin causes Kidney disease. (Liver is Yi Wood, a Yin-soft body, the root of growth. Without this Wind-Wood in heaven and earth, what facilitates spring growth and summer expansion? Without Ding-Ren in the Five Movements, what coordinates the colors in the five heavens? Without Si-Hai in the Six Qi, what balances the ruling and ruled qi? Without liver and gallbladder in humans, what initiates conception in the first month? The foundation of transformation is greatest here. Yet pediatrics echoes this, calling it a hundred evils and fierce pathogenic qi. All organs suffering from it are devastated, leading to Wind generation, Fire blazing, insulting Earth, harming Metal, damaging Yin, and harming Water—all evils complete. Even if attacked relentlessly, it is not enough to punish its guilt; unless this infant is killed so Liver Wood has no place to exist, can its ferocity be removed? This goes against reason and conscience, how can it be endured! Someone says: Mostly, Kidney Water is insufficient, and Liver Qi is excessive, thus harming other organs. Perhaps this is true, but denying it entirely may be hard to believe. I reply: Ha! This statement is even more contradictory. Knowing Kidney Water is insufficient and Liver Qi is excessive, this rootless Wood and biased Qi are unstable and uncertain. One should urgently save the root, nourish Water to generate it, nourish Blood to harmonize it, and diligently tonify, yet regretting slowness. Instead, identifying it as a Thief Pathogen and attacking, insulting, and abusing it, letting it wither and break, with no way to rescue—is this not truly tragic?) This is the general overview of Five-Zang convulsions. (Explicitly stating Five-Zang convulsion confirms convulsion belongs to the Five Zang organs.) The methods of treatment have key points: 1. Wind, 2. Fire, 3. Phlegm, 4. Yang deficiency, 5. Yin deficiency. (Suddenly discussing methods for Yang and Yin deficiency.)

It is said: Acute and chronic convulsions are what ancients called Yin and Yang Epilepsy (Dian). (Epilepsy is a chronic disease, not an acute one, and should not be grouped here.) Acute convulsion belongs to Yang, chronic to Yin. If Fright Evil enters the Heart, it causes red face and cheeks, nightly crying with fear. If it enters the Liver, the face and eyes turn blue, and eyes wander. If it enters the Kidney, the face turns black, screaming is aversive, teeth gnash, and biting occurs. If it enters the Lungs, the face is pale white, with panting and急促 breathing. If it enters the Spleen, there is vomiting, refusal to eat, and pale yellow face. (Earlier it was said Fright is being startled, and only the Heart receives it. Here it suddenly says Fright Evil enters Heart, Liver, Kidney, Lungs, Spleen. What is this Fright Evil? And why discuss it incessantly?) However, Wind does not move without Fire, and Fire does not erupt without Wind. Wind and Fire battling form convulsion, so Heart and Liver govern it. (Earlier it said Fright Evil is in the Five Zang, so Liver Wind was irrelevant. Now pulling in Liver Wind to prove the fallacy of Wood-Fire generating Wind is nauseating.) However, if Fire is deficient, Metal is damaged, Water loses its mother, Fire has nothing to fear, Wood has nothing to restrain, and Spleen Earth is injured. (Why not say Water excess damages Fire, Earth loses its mother, Water has nothing to fear, Metal has nothing to restrain, and Liver Wood is injured? Dragging branches and leaves, making excuses is easy. It is merely trying to substantiate the concept of Liver Wind, implicating the Five Zang. Losing fire burns fish, killing the ape harms the tree—this greed is worse.)

It is said: Acute convulsion is a Yang syndrome. Children usually have excess Yang and deficient Yin. (The Classics say: "Yang Qi is like the sky and sun; if it loses its position, lifespan is shortened and brilliance fades." Also: "If Yang Essence descends, the person dies early." The *I Ching* regards Yang as the Gentleman and Yin as the Petty Person. This is not valuing Yang and despising Yin, but because Yang governs ascending and generating, while Yin governs肃杀 (descending and killing). Pediatricians oppose this. Sages support Yang and suppress Yin; pediatricians protect Yin and attack Yang, frequently using "Yang excess, Yin deficiency" as a formula. Though spoken earnestly, they do not know what they mean. If Blood is Yin and Qi is Yang, the Classics say: "Infants have fragile flesh, little Blood, and weak Qi." Never "little Blood and much Qi." This means True Qi is not yet born, relying only on the thread of respiration. Now considering it "excess," one must make this thread of Qi cease entering and exiting to call it "Yang deficiency"! If Cold is Yin and Heat is Yang, Cold governs convergence, Heat governs generation. Humans rely on this Yang Harmony. With this Yang Harmony, the hundred bones and five viscera can move. Now considering it "excess," one must extinguish this warm Qi until limbs are stiff and the body is cold water to call it "Yang deficiency"! If Water is Yin and Fire is Yang, Tianyi is not Yin, Di'er is not Yang, Kan-Xu is not Yin, Li-Si is not Yang—this is even less relevant. If Liver is Yin and Lung is Yang, Metal does not control Wood, Liver Qi is excessive. But Liver belongs to Jueyin, governing Blood storage. Blood is Yin Ying. Yi and Gui share the same source. Mistaking it for Yang is doubly erroneous. If Liver is Yin and Gallbladder is Yang, Gallbladder has no entry/exit, though belonging to Shaoyang, it governs Half-Exterior Half-Interior. Disease here requires harmonizing Exterior and Interior, strictly prohibiting sweating, vomiting, and purging. Now mistakenly identifying Shaoyang Gallbladder Heat as Yang Fire excess, lightly using cooling, draining, and purging treatments—does this not change the constant principles and willingly accept the ridicule of misleading the world? If True Yang is excess, children's Tian Gui is insufficient, and True Yang has not yet established itself—this is baseless. If Five Zang are Yin and Six Fu are Yang, one should specifically drain the Yang of the Six Fu, not regard Heart Fire and Liver Wind as Thief Pathogens, nor use large-cold, large-toxic substances like Huanglian (Coptidis Rhizoma) and Gansui (Kansui Radix) to drain the deficient Yin. I do not know what they refer to when speaking incessantly. Do children have some other "Yang excess, Yin deficiency"? My insight is shallow and insufficient to fathom their depth.) Prone to generating Heat; excessive Heat generates Wind, Phlegm, and Fright. (Popular tunes echoed nationwide?) Moreover, diet is hard to regulate (suddenly blaming diet), emotions are unpredictable (suddenly blaming temperament). Violent anger damages Yin, violent joy damages Yang. Damaging Yin causes diarrhea; damaging Yang causes Fright. (Above it said Yang is usually excess; if excess, damage is not harmful. Now saying damage to Yang causes Fright implies Yang should not be damaged, which is beyond words. Holding both ends, difficult to serve as a model.) Violent joy damages milk in children. (Above said violent joy damages Yang; here it says violent joy damages milk. Milk is blood-like, not Yang. Why does violent joy damage it?) Milk is sweet and slow, lingering in the diaphragm (blaming milk, specifically discussing milk and food). Combined with external Cold contraction, Phlegm condenses and obstructs, stagnating and steaming. (Milk lingers in the diaphragm, Cold invades externally, leading to Phlegm condensation and obstruction, stagnation and steaming. The best treatment is to cut off milk and food; otherwise, when will the root of disease be broken?) Internally there is Food Heat, externally Wind pathogen. (Above said milk stasis with Cold; here says Food Heat with Wind—a confusing dream.) Heart Heat excess generates Fright. (Earlier said Fright from startling; here says Heart Heat generates Fright.) Liver Wind excess causes spasms. Liver Wind and Heart Fire contend. (Earlier said Wind does not move without Fire, Fire does not erupt without Wind. They work together. Here says they contend, but what is contending?) Thus, Phlegm arises from Spleen, Wind from Liver, Fright from Heart, Heat from Lungs. If the four syndromes of Fright, Convulsion, Phlegm, and Heat are present, eight throat manifestations arise. (This article starts from Yang excess and Yin deficiency generating Heat and Wind, extending to diet, emotions, breastfeeding, cold/heat/harmful pathogens, internal injury/external contraction, decorated in many ways, yet ending with the four characters "Fright, Convulsion, Phlegm, Heat." Wasting so much thought only to provide a foil for "Fright Convulsion." Ignoring that baseless actions and erroneous talks, however eloquently spread, never follow reason. It is just pulling things together to deceive ordinary people. Those with clear eyes will not be deceived.)

It is said: Acute or chronic convulsion, triggered by loud noises or great fright, causing spasms, returning to normal after. This is "No Yin," so one should purge. (*Neijing* states: "Great fright and sudden fear cause Qi and Blood separation." This is precisely spirit/mind injury and Yin-Yang dispersion. Rescue is barely sufficient; how can one see "No Yin" and purge, worrying not about purging causing Yin loss?) This syndrome is originally due to Heat arising from the Heart. (Given great fright and sudden fear, where does Heart Heat come from?) Fever, red face, thirst, hot breath, yellow-red urine and stool. Severe cases involve intense Heat. Since excessive Heat generates Wind, it belongs to the Liver, a syndrome of Yang excess and Yin deficiency. Thus, purging is used to remove Phlegm. (Above said "returning to normal after"; below describes fever, red face, etc. If these signs appear before spasms, it is due to illness, not Fright. If they appear after spasms, the previous "returning to normal" is nonsense.) Pediatric Phlegm-Heat lodges in the Heart space. (No wonder Ba Dou (Crotonis Semen) and Gansui are suddenly used to purge Phlegm from the Heart space. Does one not know that when children spasm, even if there is Phlegm, it only blocks the great collaterals of the Spleen and clogs the airway. How can it enter the Heart space? Using large toxic substances to harm the innocent and damage the Spirit-containing Zang organ—how can one avoid becoming epileptic?) Due to hearing unusual sounds, movement triggers spasms. If Heat is extreme, not hearing sound and fright, spontaneous spasms also occur. (Hearing sound and trembling exists in those with deficient Spirit. Cultivators say Essence generates Qi, Qi generates Spirit. Spirit relies on Qi like fish rely on water. Children with thin endowment lack congenital essence before pregnancy and Gui Water is not formed currently, so Yuan Jing is insufficient. Yuan Qi has no root, so Yuan Shen is most timid, often trembling at sounds. This is truly a syndrome of deficient Spirit and fearful Gallbladder. Yet pediatrics not only pities the insufficient Yuan Shen but points to Heart Fire excess. Not only Heart Fire excess, but drags in Liver Wind, Spleen Phlegm, and Lung Heat to prove the fallacy of "Fright, Convulsion, Phlegm, Heat" four syndromes and eight signs. Again, hearing unusual sounds, seeing strange objects, sudden surprise and trembling—this is precisely Spirit and Soul lacking mastery, Ying and Wei chaotic, wavering and unable to settle. Not protecting and rescuing, yet saying "Heat arises from Heart," insisting on purging to remove Phlegm—adding insult to injury, snow on frost. These people, what are their intentions? Truly astonishing.)

It is said: A child normally fine, suddenly high fever, hand-foot spasms, eyes rolling up, tidal phlegm obstruction, locked jaw, fever, red eyes. (Saying "normally fine" means no disease is known. Seeing these bad signs suddenly, laypeople would say it is a ghost disease. Otherwise, how can one be normally fine yet suddenly so dangerous? Does one not know children easily contract Wind-Cold and injure milk/food? Half a day prior, the pathogen already entered inside. The child cannot speak, parents do not notice. Pathogen stagnates and cannot extend, so spasms occur suddenly. Observing the words "sudden high fever" reveals this. Whether Wind-Cold entering Interior causing high fever, or food stagnation causing high fever, this is truly caused by illness, not "normally fine." Treating this is not difficult; one should observe the source of illness. External causes: reach and release them. Internal causes: guide and purge them. Once pathogen is gone, Spirit nature calms itself. If one insists on "no cause," laypeople suspect ghosts and seek shamans; mediocre doctors label it convulsion and treat wildly. Both delay, what can be done?) This acute convulsion belongs to Yang, disease in the Fu organs. (Often said convulsion is governed by Heart and Liver. Here suddenly says belongs to Fu. Are Heart and Liver Fu or Zang? Must ask the founding ancestor of pediatrics to clarify. I am dazzled and cannot recognize.)

It is said: Acute convulsion is a Yang syndrome, all Fu organs receiving the disease. Children have guest Phlegm-Heat in the Heart and Diaphragm, indicating Shaoyang Minister Fire is prosperous. (Usually speaks of Heart Fire; here says Minister Fire—novel.)
