痘麻胎毒发于心，胃腑相连热毒侵。咳嗽鼻中清涕出，且观双目泪淋淋。
痘麻皆胎毒所为。毒者，火也。痘为少阳相火，阳道常饶，故痘大而 肿；麻乃少阴君火，阴道常乏，故麻
小而碎密。心火旺则肺受之，故治麻当以肺为主。凡咳嗽者，火炎于肺也；鼻流清涕者，以火铄金而液自流也；目
中泪出，乃肺热移于肝也；凡手掐眉目鼻面者，肺热证也。
凡遇冬温最不详，民多疫疠发疮疡。若逢斑疹相传染，可用汤丸预解良。
春温夏热，秋燥冬寒，此四时之主气也。冬应寒而反温，阳先暴泄，火令早行，人感其气，至于来春必生疮
疹。未出痘麻者，必感而发，虽曰胎毒，未有不由天行疠气，故一时传染，大小相似。但见麻疹之出，宜服代天
宣化丸以预解之，可使毒彻，不为已甚也。
麻出须明岁气先，忽轻汗下致颠连。察人虚实施方法，暗损天和寿不坚。
麻初发热，与伤寒相似，但麻疹则面颊赤，咳嗽喷嚏，鼻流清涕，目中泪出，呵欠喜睡，或吐泻，或手掐眉
目鼻面，宜升麻葛根汤，不可作伤寒，妄用汗下也。汗之则增其热，为衄血，为咳血，为疮咽痛，为目赤痛，为烦
躁，为大小便不通；下之则虚其里，为滑泄，为滞下。经曰∶必先岁气，毋伐天和。此之谓也。
麻喜清凉痘喜温，须知麻痘不同门。麻苗痘实无人解，寒热宜分未可浑。
麻喜清凉，痘喜温暖，此法人皆知之。然麻疹初发，亦宜和暖则易出，所以发苗之初，
只要发出得尽，则毒便解矣。若痘必苗而秀，秀而实，毒斯解也。然成实之时，若太温热，则反溃烂不收，是痘
后亦宜清凉。故治痘麻，无过热，无过寒，温凉得宜，阴阳自和，是为得之。
麻毒从来不可留，出完毒解便无忧。腹中胀痛邪犹伏，喘促昏迷命必休。
麻疹只怕不能得出，若出尽，毒便解矣。凡麻疹发热之时，当审时令寒暄，以药发之。如时令大寒，以桂枝
葛根汤发之；大热，以升麻葛根汤合人参白虎汤发之；不寒不热，以荆防败毒散发之。如兼疫疠时行，以人参
败毒散发之，外以胡荽酒，用苎麻蘸酒遍身戛之，务令亟出。若发而不出，反加腹中胀痛，气上喘促，昏闷谵妄
者，死证也。
过期不出势淹延，毒伏身中未得宣。急用透肌休怠玩，岂堪脏腑受熬煎！
发热六七日已后，明是麻证，却不见出。此皮肤坚浓，腠理闭塞，又或为风寒袭之，曾有吐泻，乃伏也，急
用发表之剂，麻黄汤去杏仁，加蝉蜕、升麻，外以胡荽酒散麻刮之。如前未更衣者，毒甚于里，伏而不出，凉膈
散加牛蒡子发而解之。再不出者，死证也。（更衣，谓大便也。）
肺为华盖脏称娇，毒火炎蒸津液消。喘嗽连声痰唾少，急须清润救枯焦。
麻疹初发热时，未见出现，咳嗽百十声不已，上气促急，面浮目胞肿，时卧时起。此火毒内蒸，肺叶焦举，
宜甘桔汤加石膏、知母、牛蒡子主之。
火热熏蒸汗不停，毒邪并迫血违经。汗多卫表邪从解，血去荣中毒少宁。
麻疹发热自汗，或鼻血出，不须止之，亦发散之义。故汗者毒从汗散，衄者毒从衄解，但不可太过。如汗太
多，人参白虎汤合黄连解毒汤清之；衄太甚，玄参地黄汤凉之。
发热乍然生吐泻，由他频出不须怕。肠胃停污自此清，胞胎之毒全消化。
麻疹发热吐泻，纯是热证，不可作寒论，
乃火邪内迫。毒在上焦则吐，毒在下焦则泻，毒在中焦则吐泻并作。单泻，黄芩汤；吐而兼泻，黄芩加半夏汤；
自利，里急后重，黄连解毒汤合天水散。
毒火熏蒸气上腾，咽喉自此痛烦增。从来麻痘多斯证，解毒清咽效自能。
麻痘咽痛，本为常候，乃火毒熏蒸而痛也，勿与喉痹同论，妄用针刺。盖喉痹之证，内作痈肿，故宜以针决
去恶血，麻痘只是咽干作痛，宜甘桔汤或鼠粘子汤，细细咽之自愈。
麻毒如焚饮水饶，炎邪未许一杯浇。咽喉急燥心家热，津液频枯胃脘焦。
麻疹渴喜饮水，纯是火邪，肺焦胃干，心火内亢故也。初发热作渴，升麻葛根汤（方见前）加天花粉、麦门冬；
渴甚，人参白虎汤合黄连解毒汤（方见前）
一齐涌出莫惊惶，顷刻浑身朱锦庄。似痘出时还又没，如 红处却成疮。
痘疹贵三四次出，谓出匀；麻疹贵一齐涌出，谓出尽。麻疹只要得出便轻减，以火照之，遍身如涂朱之状，
此将出之兆。出形细密，与痘疹密者相似，但麻疹粒粒成疮，非若 又皮红成片，如蚊咬之迹也。
痘疮赤艳痒来攻，麻见红鲜毒得松。白色血虚犹可疗，黑斑恶证莫相逢。
痘麻之色不可同论，大抵痘怕太红，皮嫩易破，必生瘙痒；麻喜通红，麻发于心。红者，火之正色。若麻
色淡白，心血不足，宜养血化斑汤主之；色太红艳，或微紫，或出太甚，并宜大青汤。黑者，死证也。
麻之出没合阴阳，出以温和没以凉。连出不收阳气盛，迟迟间出是阴强。
麻疹出没，常以六时为准。假如子后出，午时即收，午后出，子时即收，乃阳生阴成，阴生阳成，造化自
然之数。凡此旋出旋收者轻。若一出连绵三四日不收，乃阳毒太甚，大青汤（方见前）解之；逡巡不出，乃风寒外
束，皮肤闭密，宜荆防败毒散（方见前）。
麻出浑身似火烧，毒邪壅甚急难消。解肌只许皮肤暖，救里还期便溺调。
麻疹欲出，则遍身发热，或烦躁，或头眩，或身拘急；及既出，则身即清凉，诸病悉解，此一层麻疹随收
矣。如麻既出，热甚不减，此毒壅遏，宜大青汤（方见前）以解其表；小便涩，大连翘汤以解其里；大便秘，凉膈
散（方见前）加牛蒡。
麻疮出尽得安和，毒水清时奈若何。拂拂热烦邪尚炽，频频呕泻毒犹多。
凡麻疹只要出得尽，则毒邪解散，正气和平。如怫怫发热，烦闷不宁，如蛇在灰，如蚓在尘之状，或呕吐，
或泄泻，此毒邪壅遏，尚未出尽。烦热，黄连解毒汤；呕泻，黄连橘皮汤。二者并外用胡荽酒，以苎麻蘸酒搽
之。（方法见前。）待麻出尽，则烦热自除，呕泻自止矣。
麻毒流殃为伏邪，几经恶候致嗟呀。虽然疫疠由天降，也是因循人事差。
麻疹欲出未出之时，即当早为发散，以解其毒，庶无余灾。若不预解使之尽出，以致毒蓄于中，麻后必为
壮热，日久枯瘁，或成搐搦，或为痢疾，或咳血喘促，或作疳 而死。此虽一时疫疠之染，未有不由人事之
未尽。
麻后连日热不除，蒸蒸烙手发毛疏。骨消骨立成疳瘦，幸有良工药可茹。
麻疹收后，身有微热，此虚热也，不须施治，待气血和畅，自然清凉。若热太甚，或日久不减，以柴胡麦
冬散清之；如发枯毛竖，肉消骨立，渐渐羸瘦，柴胡四物汤主之。
发热无休神渐昏，忽然螈 乍惊魂。莫将痉病同调治，退热凉心命可存。
痘后热不除，忽作搐搦，不可称为惊风而用风药，宜导赤散加人参、麦冬，煎汤送安神丸（方见女人麻疹条。）
小便清者可治，短少者不可治。
麻毒流殃走马疳，牙龈溃烂药空含。穿喉漏颊声音哑，早赋归欤疾似骖。
凡麻后牙龈黑烂，肉腐血出，息臭冲人，曰走马疳。马鸣散主之。若面颊浮肿，环口青黑，颊漏齿脱，唇崩
鼻坏者，死证也。宜从卷四齿牙门参考。
麻后泄泻名休痢，昼夜无停真可异。勿轻劫涩图霸功，噤口毒深凶莫避。
麻后泄痢，日久不已，曰休息痢。不可妄用涩剂以图霸功，宜黄芩汤合六一散煎送香连丸。若呕吐不能食，
谓之噤口。更加肠滑不止，或下鲜血，或如烟尘水者，死证也。
炎炎胃火金遭迫 咳嗽百声痰阻隔。胸高肩息目虚浮，摆手摇头泉下客。
麻疹收后微咳，此肺气未平，不须调治。若咳转甚，喘气上逆，发则连绵不已，此肺中伏火，宜人参清膈
散主之；身热，门冬清肺汤主之。若咳久不止，面浮目胞肿，胸高而喘，息则耸肩，血自口鼻中出，面色或青
或赤，鼻燥昏闷，摇头摆手者，死证也。
麻收禁忌切须防，盐醋鸡鱼慎勿尝。欲莫从心终是福，物多爽口定为殃。
麻后通禁鸡鱼炙 盐醋之类，须过七七之后方可食之，惟宜食淡，不可纵口，以遗后患也。
麻收无恙将平复，饮食如常未纵欲。心胸绞痛忽倾亡，疫疠侵凌名中毒。
曾见痘麻收后，动止出入饮食如常，忽然心胸绞痛而死者，还是元气怯弱，疫疠之毒乘之。正不能胜，邪
伏于中，外若无病，内已亏损，故一中而死，谓之中恶。
婴孩初离胎壳中，遍身斑驳似朱红。胎中热毒于今现，莫作时行麻疹攻。
凡小儿初生未盈月者，遍身红点，俗呼奶麻子是也。此胎中受热，故生下即发，现于皮肤，不可作时行麻
毒论治，妄用汤剂。盖脏腑娇嫩，不能胜汤丸也，宜溯源解毒汤与乳母食之。

Both smallpox (Xiaochuan) and measles (Mazhen) originate from fetal toxins (Taidu) emanating from the Heart channel, connected to the Stomach organ, with heat-toxin invading. Coughing occurs, clear discharge flows from the nose, and observing the eyes reveals them teeming with tears.
Smallpox and measles are both caused by fetal toxins. The term "toxin" refers to Fire pathogen. Smallpox belongs to the Shaoyang Ministerial Fire (Shaoyang Xianghuo); since the Yang pathway is often abundant, smallpox lesions are large and swollen. Measles belongs to the Shaoyin Sovereign Fire (Shaoyin Junhuo); since the Yin pathway is often deficient, measles lesions are small, fine, and densely packed. When Heart Fire is excessive, it affects the Lung; therefore, treating measles should focus primarily on the Lung. Coughing indicates Fire flaming upward in the Lung; nasal discharge of clear fluid results from Fire scorching Metal (Lung), causing fluids to flow freely; tearing from the eyes signifies Lung heat transferring to the Liver; scratching the eyebrows, eyes, nose, and face are all signs of Lung heat.
Encountering winter warmth is most inauspicious; the populace often suffers from epidemics and develops sores and ulcers. If macules and papules are contagious, decoctions or pills can be used to preemptively resolve the toxin, yielding good results.
Spring warmth, summer heat, autumn dryness, and winter cold are the dominant Qi of the four seasons. Winter should be cold but becomes warm instead; Yang Qi leaks out violently, and the Fire command arrives early. Humans contracting this Qi will inevitably develop sores and rashes in the following spring. Those who have not yet manifested smallpox or measles will certainly contract and manifest the disease. Although termed fetal toxin, it invariably arises from heavenly epidemic pestilence Qi (Tianxing Liqi), causing mutual infection within a short time, with symptoms appearing similar across individuals. Upon seeing measles erupt, one should administer Dai Tian Xuan Hua Wan (Heaven's Envoy Transforming Pill) to preemptively detoxify, ensuring the toxin is thoroughly expelled without becoming overly severe.
The emergence of measles must align with the annual climatic conditions (Suiqi); rashly using sweating or purging methods leads to prolonged, critical conditions. One must assess deficiency and excess to apply appropriate treatments; secretly damaging the Harmony of Heaven (Tianhe) shortens life.
Early fever in measles resembles Cold Damage (Shanghan), but in measles, the cheeks are red, accompanied by coughing, sneezing, clear nasal discharge, tearing, yawning, drowsiness, vomiting/diarrhea, or scratching the face/eyes/nose. Shengma Gegen Tang (Ephedra and Pueraria Decoction) is indicated; do not treat as Cold Damage and recklessly use sweating or purging. Sweating increases heat, leading to epistaxis, hemoptysis, sore throat, red/painful eyes, irritability, and urinary/fecal retention. Purging empties the interior, leading to slippery diarrhea or dysentery. The Classic states: "First understand the seasonal Qi, do not violate the Harmony of Heaven." This refers to such cases.
Measles prefers coolness; smallpox prefers warmth. It is known that they belong to different categories. Measles is like a seedling; smallpox is like a fruit. Few understand this distinction; cold and heat must be differentiated, not confused.
It is known that measles prefers coolness and smallpox prefers warmth. However, during the initial eruption of measles, mild warmth facilitates eruption. Thus, at the beginning, ensuring complete eruption resolves the toxin. Smallpox must progress through stages (seedling, flowering, fruiting) for the toxin to resolve. However, if too warm during the fruiting stage, it causes ulceration and failure to heal; thus, post-smallpox care also requires coolness. Therefore, treating smallpox and measles requires neither excessive heat nor cold; appropriate warmth and coolness allow Yin and Yang to harmonize naturally, which is correct.
Measles toxin should never remain; once erupted, the toxin resolves, bringing peace. Abdominal distension and pain indicate latent pathogens; panting, coma, and confusion signal death.
Measles only fears failure to erupt; if fully erupted, the toxin resolves. During the febrile stage of measles, observe the seasonal climate and use herbs to induce eruption. In extreme cold, use Guizhi Gegen Tang (Cinnamon Twig and Pueraria Decoction); in extreme heat, use Shengma Gegen Tang combined with Renshen Baihu Tang (Ginseng White Tiger Decoction); in neutral conditions, use Jingfang Baidu San (Schizonepeta and Saposhnikovia Powder to Overcome Toxicity). If concurrent with epidemic spread, use Renshen Baidu San (Ginseng Powder to Overcome Toxicity). Externally, apply Huixu Jiu (Coriander Wine) rubbed over the body with Ramie fibers to hasten eruption. If eruption fails and abdominal pain worsens, accompanied by panting, coma, and delirium, it is a fatal sign.
Delayed eruption prolongs the condition; toxin remains latent in the body. Urgently use skin-penetrating, exterior-releasing agents; do not delay, lest the Zang-Fu organs suffer roasting!
After six to seven days of fever, clearly indicating measles, yet no eruption occurs. This is due to thick skin and closed pores, or invasion by wind-cold, or prior vomiting/diarrhea indicating internal latency. Urgently use exterior-releasing formulas: Mahuang Tang (Ephedra Decoction) minus Xingren, plus Chantui (Cicada Slough) and Shengma (Cimicifuga Rhizome). Externally, scrape with Huixu Jiu powder. If constipation persists, toxin is severe in the interior; use Liangge San (Cooling the Chest Powder) plus Niubangzi (Arctii Fructus) to release and resolve. Failure to erupt again indicates death. ("Changing clothes" refers to defecation.)
The Lung is the canopy (Huagai) and is called the delicate organ. Toxin-fire steams, consuming body fluids. Continuous panting and coughing with scant sputum require urgent clearing and moistening to rescue desiccation.
During early measles fever, before eruption, continuous coughing (hundreds of times), upper airway urgency, facial edema, eyelid swelling, alternating lying and sitting indicate internal steaming of fire-toxin and scorched lung lobes. Ganjie Tang (Licorice and Platycodon Decoction) plus Shigao (Gypsum), Zhimu (Anemarrhenae Rhizoma), and Niubangzi is indicated.
Fire-heat steaming causes incessant sweating; toxin-pathogens force blood to deviate from vessels. Profuse sweating releases pathogenic Qi from the Wei (Defensive) surface; blood loss reduces Ying (Nutritive) toxin burden.
Spontaneous sweating or epistaxis during measles fever need not be stopped; this is part of the dispersing principle. Sweating allows toxin to disperse via sweat; epistaxis allows toxin to resolve via bleeding. However, excess must be avoided. If sweating is excessive, use Renshen Baihu Tang combined with Huanglian Jiedu Tang (Coptis Decoction to Resolve Toxicity) to clear. If epistaxis is severe, use Xuanshen Dihuang Tang (Scrophulariae and Rehmanniae Decoction) to cool.
Sudden vomiting and diarrhea during fever are not alarming. They clear intestinal stagnation and digest fetal toxins.
Vomiting and diarrhea during measles fever are purely heat syndromes, not cold. This is Fire-pathogen forcing internally. Toxin in the Upper Jiao causes vomiting; in the Lower Jiao, diarrhea; in the Middle Jiao, both. For pure diarrhea, use Huangqin Tang (Scutellaria Decoction); for vomiting with diarrhea, Huangqin Jia Banxia Tang (Scutellaria and Pinellia Decoction); for spontaneous diarrhea with tenesmus, Huanglian Jiedu Tang combined with Tiansan San (Sky Water Powder).
Toxin-fire steams, vapor rising; throat pain and irritation increase. This symptom is common in measles and smallpox; resolving toxicity and clearing the throat is effective.
Sore throat in measles/smallpox is normal, caused by fire-toxin steaming; do not confuse with Hou Bi (Sore Throat/Glandular Swelling) or use needle pricking. Hou Bi involves internal abscesses requiring needle drainage of bad blood; measles/smallpox involve dry, painful throats, treated with Ganjie Tang or Shi Niu Zi Tang (Arctii Fructus Decoction) swallowed slowly for self-healing.
Measles toxin burns like fire, increasing thirst. Heat-pathogens cannot be quenched with a single cup. Dry throat indicates Heart heat; frequent dehydration scorches the Stomach.
Thirst and preference for drinking water in measles are purely due to Fire-pathogen, scorched Lung, dry Stomach, and hyperactive Heart Fire. For initial fever-thirst, add Huatufen (Trichosanthis Radula) and Maidong (Ophiopogonis Radix) to Shengma Gegen Tang. For severe thirst, use Renshen Baihu Tang combined with Huanglian Jiedu Tang.
Do not panic when they erupt together in a rush; suddenly the whole body looks like vermilion brocade. Like smallpox, they may appear and disappear; where red spots form, sores arise.
Smallpox/rashes are prized for emerging in three or four waves (uniform eruption); measles are prized for simultaneous eruption (complete eruption). Measles lightens if fully erupted. Illuminating with fire shows the body coated in vermilion, a sign of impending eruption. The shape is fine and dense, resembling dense smallpox, but measles lesions become individual sores, unlike smallpox which forms red patches like mosquito bites.
Smallpox lesions are bright red and itchy; measles show fresh redness, loosening the toxin. Pale white indicates Blood deficiency, treatable; black spots are malignant signs, avoid contact.
Colors of smallpox and measles differ. Smallpox fears excessive redness (tender skin breaks easily, causing itching); measles prefer uniform redness (originating from Heart). Red is the true color of Fire. If measles are pale white (Heart-Blood deficiency), use Yangxue Huaban Tang (Nourishing Blood and Resolving Macules Decoction). If too bright red, slightly purple, or excessive eruption, use Daqing Tang (Isatidis Decoction). Black indicates death.
Measles emergence and disappearance follow Yin-Yang principles: emergence with mild warmth, disappearance with coolness. Continuous eruption without receding indicates excessive Yang; slow, intermittent emergence indicates strong Yin.
Measles emergence/disappearance follows a six-period cycle. Eruption after Zi hour recedes at Wu hour; afternoon eruption recedes at Zi hour, reflecting natural generation cycles. Rapid emergence/recession is mild. Continuous eruption for 3-4 days without receding indicates excessive Yang toxin; resolve with Daqing Tang. Hesitant non-emergence indicates external wind-cold binding tight skin; use Jingfang Baidu San.
Whole-body burning sensation upon eruption; toxin-pathogen accumulation is difficult to dissipate. Muscle-releasing allows only warm skin; interior-rescuing expects regulated urination/defecation.
Before eruption, general fever, irritability, dizziness, or stiffness occur. After eruption, the body cools, diseases resolve, and measles recede. If fever persists post-eruption, toxin is blocked; use Daqing Tang to release the exterior. If urine is turbid/hard to pass, use Dalianqiao Tang (Large Forsythia Decoction) to release the interior. If constipated, use Liangge San plus Niubangzi.
After complete eruption, peace returns. What to do when toxic fluids clear? Lingering heat/irritation indicates still-active pathogens; frequent vomiting/diarrhea indicates remaining toxin.
If measles erupt completely, toxin disperses, and healthy Qi harmonizes. Lingering fever, restlessness (like a snake in ash or worm in dust), vomiting, or diarrhea indicate blocked toxin, incomplete eruption. For fever/irritation, use Huanglian Jiedu Tang; for vomiting/diarrhea, use Huanglian Jupi Tang (Coptis and Tangerine Peel Decoction). Externally, rub Huixu Jiu with Ramie. Once fully erupted, fever and vomiting cease.
Measles toxin flowing as latent pathogen causes repeated malignant signs. Although epidemics come from Heaven, human negligence contributes.
When measles are about to erupt, promptly use dispersing agents to resolve toxin, avoiding residual disaster. If not preemptively resolved, toxin accumulates internally, leading to high fever, emaciation, convulsions, dysentery, hemoptysis/panting, or malnutrition syndrome (Gan syndrome) death. Though contracted from epidemic Qi, it stems from inadequate human management.
Persistent fever after measles resolution feels hot to touch; hair becomes sparse. Bone wasting leads to malnutrition syndrome; fortunately, skilled physicians can prescribe medicine.
After measles recede, slight fever indicates deficiency-heat; no treatment needed. Wait for Qi/Blood harmony for natural cooling. If fever is severe or persistent, use Chaihu Maidong San (Bupleurum and Ophiopogon Powder). With dry hair, standing bristles, muscle wasting, and emaciation, use Chaihu Siwu Tang (Bupleurum Four Substances Decoction).
Unceasing fever leads to gradual mental clouding; sudden convulsions startle the spirit. Do not treat as Spasm disease; clearing heat and cooling the Heart preserves life.
Post-smallpox fever with sudden convulsions should not be treated as Wind-stroke with wind herbs. Use Daochi San (Guide Out Red Powder) plus Renshen and Maidong, decocted to deliver Anshen Wan (Calming Spirit Pill). Clear urine is treatable; scant urine is untreatable.
Measles toxin causes Gangrene Running Like a Horse (Zouma Gan); gum ulceration renders medicine useless. Throat perforation, cheek leakage, hoarseness signal early death.
Black, rotten gums with rotting flesh, bleeding, and foul breath after measles is Zouma Gan. Treat with Maming San (Horse Cry Powder). Facial swelling, blue-black ring around mouth, cheek leakage, tooth loss, lip collapse, or nasal destruction are fatal. Refer to Volume 4, Teeth Chapter.
Post-measles diarrhea named Resting Dysentery (Xiuli) is strange day and night. Do not lightly use astringents for quick success; anorexic type indicates deep toxin and凶险 (danger).
Persistent diarrhea/dysentery after measles is Xiuli. Do not recklessly use astringents; use Huangqin Tang combined with Liuyi San (Six-to-One Powder) to deliver Xianglian Wan (Amomum and Coptis Pill). Vomiting with inability to eat is Jin Kou (Clamped Mouth). Plus uncontrolled intestinal slipping, fresh blood, or smoke-like water stool is fatal.
Blazing Stomach Fire presses Metal; coughing hundreds of times blocks phlegm. High chest, shoulder breathing, floating eyes, waving hands/shaking head indicate a guest from the underworld (death).
Slight cough after measles recedes indicates uncalmed Lung Qi; no treatment needed.
