[Appendix: Head Wind]
Patient (Left): The headache stopped but recurred. Due to deficiency of Liver and Kidney Yin, deficient wind rises and harasses the head. It is reasonable to tonify the deficiency and drain the excess.
Prescription: Charred Rehmanniae Radix (Shengdi Tan), Chrysanthemi Flos (Chuju Hual), Angelicae Sinensis Radix (Fen Guishen), Ligustici Chuanxiong Rhizoma (Chuanxiong), Calcined Apatitum (Wei Jueming), Paeoniae Radix Alba (Dong Baishao), Bombyx Batryticatus (Bai Jiangcan), Ligustici Wallichii Rhizoma (Ben), Moutan Cortex (Fen Danpi), Gardeniae Fructus (Hei Shanzhi).
Patient (Right): The headache persists; at times severe, causing mental restlessness and oppression in the chest and heart. This is due to Liver Fire and Wind obstructing the Yang collaterals. There is fear of blindness resulting.
Prescription: Mori Folium (Sangye), Gardeniae Fructus (Hei Shanzhi), Saposhnikoviae Radix (Fangfeng), Scutellariae Radix (Dan Zixin), Notopterygii Rhizoma et Radix (Qianghuo), Moutan Cortex (Danpi), Chrysanthemi Indici Flos (Gan Juhua), Ligustici Wallichii Rhizoma (Ben), Halitum (Shijueming), Bombyx Batryticatus (Jiangcan).
Patient (Right): The headache is extremely severe, with opacity in the right eye. This is due to Liver Fire and Wind rotating upward. Vision will inevitably be damaged.
Prescription: Ligustici Chuanxiong Rhizoma (Chuanxiong), Bombyx Batryticatus (Bai Jiangsan), Forsythiae Fructus (Lianqiao), Antelopis Cornu (Lingyang Pian), Dried Lotus Leaf (Gan Hebian), Angelicae Dahuricae Radix (Baizhi), Chrysanthemi Indici Flos (Gan Juhua), Moutan Cortex (Danpi), Lethariella Viridis (Songluocha), Scorched Gardeniae Fructus (Jiao Shanzhi).
Patient (Right): The headache is on the right side, with blood occasionally present in the phlegm. Although symptoms are temporarily relieved, there is still vertigo and palpitations, caused by Fire Qi counterflowing upward. The pulse is wiry and slippery. This is essentially insufficient Yin essence of the Liver and Kidney, and excessive Phlegm-Dampness of the Spleen and Stomach. The Qi of the Gallbladder and Stomach fails to descend, causing the Yang of the Liver to rise excessively. The proposed method is to extinguish Liver Wind and harmonize Yang.
Prescription: Tribuli Fructus (Bai Jili), Scutellariae Radix (Huangqin), Saposhnikoviae Radix (Qing Fangfeng), Fried Ziziphus Seed (Chao Zaoren), Halitum (Shijueming), Alisma Rhizoma with Cinnabar (Zhu Fushen), Notopterygii Rhizoma et Radix (Qianghuo), Angelicae Sinensis Radix (Bai Guishen), Soybean Coat (Douyi), Processed Pinellia Tuber (Zhi Banxia).
Patient Shao (Right): Pain on one side of the head, accompanied by palpitations, fright, insomnia, and occasional sensation of heat intolerance. This is due to excessive rising of Yang, preventing Heart Fire from descending. The proposed method is to calm the Spirit and harmonize Yang.
Prescription: Fried Ziziphus Seed (2 qian), Alisma Rhizoma (3 qian), Moutan Cortex (1.5 qian), Hang Bai Shao fried with wine (1.5 qian), Halitum (5 qian), Black Soybean Coat (3 qian), Biota Seed (3 qian), Dragon Teeth (3 qian), Fried Anemarrhenae Rhizoma (1.5 qian), Chaenomelis Fructus (1.5 qian), Tianwang Buxin Wan (3 qian, taken first).
Second visit: Sleep has slightly improved, and the sensation of heat has decreased, but pain on one side of the head persists. The left Guan pulse is large; this indicates insufficient Yin nourishment and excessive rising of Yang. The previous method should be combined with Yin-nourishing methods.
Prescription: Raw Tortoise Shell (4 qian), Sour Jujube Seed (2 qian, fried with decoction of 0.2 qian Coptidis Rhizoma and ground), Female Date Fruit steamed with wine (3 qian), White Peony Root fried with wine (1.5 qian), Pearl Mother fried with vinegar (4 qian), Chrysanthemi Flos (1.5 qian), Dragon Teeth (3 qian), Black Soybean Coat (3 qian), Moutan Cortex (2 qian), Chen Dengxin (3 chi).
Third visit: Able to sleep slightly, but heat rushes upward intermittently. The pulse is fine and wiry. Insufficiency of Yin division, failure of Yang to submerge. Advance the previous method further.
Prescription: Gelatinous Donkey-hide Hide Bead (3 qian), Alisma Rhizoma (3 qian), Dragon Teeth (3 qian), White Peony Root fried with wine (1.5 qian), Sour Jujube Seed (2 qian, fried with decoction of 0.3 qian Coptidis Rhizoma), Caulis Trichosanthis (4 qian), Female Date Fruit fried with wine (3 qian), Pearl Mother fried with vinegar (4 qian), Chen Dengxin (3 chi), Lianzhu Fen (0.02 qian, taken first).
Patient Zhang (Left): Headache with vertigo. Tongue coating is white, thick, and greasy; pulse is soggy, slow, and slightly slippery. This is Liver Yang carrying Phlegm upward. The proposed method is to extinguish Liver Wind and resolve Phlegm.
Prescription: Processed Pinellia Tuber (1.5 qian), Tribuli Fructus (3 qian), Fried Bamboo Shavings (1.5 qian), Calcined Gastrodia Tuber (1.5 qian), Chrysanthemi Indici Flos (2 qian), Thin Tangerine Peel (1 qian), Uncaria Ramulus cum Uncis (3 qian), Halitum (4 qian), Poria (3 qian), Baijin Wan (0.07 qian, taken in two doses).
Second visit: After resolving Phlegm and draining Heat, vertigo has slightly decreased but not stopped. The pulse is fine and wiry. As stated in the Classic: "Headache and cranial diseases indicate deficiency below and excess above." The cause is internal deficiency of Kidney Water and rising of Yang Qi. Re-propose the method of nurturing Yin and submerging Yang.
Prescription: Turtle Shell (6 qian, decocted first), Oyster Shell (8 qian), White Chrysanthemum Flower (1.5 qian), Tribuli Fructus (3 qian), Goji Berry (3 qian), Rehmanniae Radix (4 qian), Black Soybean Coat (3 qian), Moutan Cortex (2 qian), Calcined Gastrodia Tuber (1.5 qian).
Patient Shao (Right): Dizziness gradually turns into pain, radiating to behind the ear, with nausea and desire to vomit. Both Guan pulses are wiry. This is failure of Shaoyang and Yangming Qi to descend.
Prescription: Bupleuri Radix (0.4 qian), Fried Bamboo Shavings (1 qian), Processed Pinellia Tuber (1.5 qian), White Peony Root fried with wine (1.5 qian), Moutan Cortex (1 qian), Gardeniae Fructus (2 qian), White Poria (3 qian), Ligustici Chuanxiong Rhizoma (0.5 qian), Viticis Fructus (0.8 qian).
Second visit: Headache greatly reduced, swelling behind the ear, confirming excessive rising of Jia Wood (Liver/Gallbladder).
Prescription: Mori Folium (1.5 qian), Gardeniae Fructus (3 qian), Tribuli Fructus (3 qian), Chrysanthemi Flos (1.5 qian), Uncaria Ramulus cum Uncis (3 qian), Moutan Cortex (1.5 qian), Viticis Fructus (1 qian), Halitum (3 qian), Forsythiae Fructus (3 qian), Dried Lotus Leaf (3 qian).
Patient Liu (Right): The Classic states: "True headache involves severe pain throughout the brain, with cold limbs reaching the joints; it is incurable." For the symptom of headache connecting to the brain, there have rarely been specific formulas. Previous diagnosis showed a fine, deep pulse, becoming wiry upon prolonged pressing. Described as: when the disease is severe, the brain suffers intense pain, causing twitching of meridians throughout the body, which subsides after several days. The brain is the Sea of Marrow; if the disease enters the marrow, it is incurable. How much more so if the painful area is within the Sea of Marrow? If the disease reaches the Sea of Marrow, even with treatment, there is no effective formula, let alone gradual subsidence in a few days. The reason for this must lie in profound principles that clinicians have not deeply considered. Examining the twelve meridians, the Foot-Taiyang Bladder Meridian is the residence of Water, and its channels connect to the brain. Phlegm and Dampness belong to the category of Water. When Phlegm-Dampness is suppressed, Cold Water obstructs the vessel pathways, which is the origin of brain pain. Currently, although headaches do not occur frequently, every morning there is a feeling of nausea and uneasiness in the chest until noon when it normalizes. This is because lying down closes the Qi mechanism; closed Qi prevents Phlegm-Dampness from moving. In the early morning, Phlegm-Dampness is about to move but has not yet done so, while Yang Qi floats upward, causing physical weakness and chest discomfort. Ying Qi originates from the Middle Jiao. The Middle Jiao receives Qi, extracts juices, transforms them, and turns them red, becoming Blood. Now, the Qi of food and drink received by the Middle Jiao does not transform into Blood but brews into Phlegm. Thus, before reaching the age of forty-nine (seven sevens), menstruation ceases. Proposed prescription follows, for correction by experts.
Prescription: Salt-fried Astragalus Complanatus Seed (2 liang), Tangerine Peel (8 qian), Alisma Rhizoma (1 liang), Roasted Astragalus Membranaceus Root (2 liang), Poria (2 liang), Processed Pinellia Tuber (2 liang), Fried Atractylodes Macrocephala Rhizoma (2.5 liang), Salt-fried Phellodendri Cortex (1 liang), Scorched Atractylodes Rhizoma (1.5 liang), Fried Lycium Chinense Fruit (3 liang), Calcined Gastrodia Tuber (1 liang), Eucommia Bark (3 liang), Fan Zhi Qu (1.5 liang), Charred Angelicae Sinensis Radix (2 liang), Drynaria Rhizome (2 liang, fried), White Peony Root (1 liang), Fried Ziziphus Seed (2 liang), Fried Hordeum Vulgare (2 liang), Fried Zingiberis Rhizoma (7 qian).
Grind the above medicines finely according to method, make pills with water like mung beans. Take 3 qian every morning with boiled water. Mix in 1.5 qian of ginseng whiskers powder separately.
Patient Sun (Right): Headache decreased then worsened. Yesterday, Qingzhen Tang was administered to drain Wood-Fire, and the pain subsided, bowel movements also occurred. However, stabbing pain occurs everywhere between the feet, knees, legs, and thighs. The pulse is slow, and the Guan section remains wiry. Fire and Wind have not been extinguished; they are wandering through the meridians, fearing upward movement leading to complications. Propose clearing and draining to blunt their edge.
Prescription: Gardeniae Fructus, Scutellariae Radix, Fresh Bamboo Shavings, Kudingcha, Forsythiae Fructus, Prunella Spica, Biyu San, Fresh Chrysanthemum Leaves, Moutan Cortex, Haematitum, Fresh Lotus Leaf Edge.
Patient Wang (Left): Initially, latent Cold Evil in the Taiyang meridian moved with the release of Yang Qi, causing headache as if breaking, even leading to confusion of spirit. Fortunately, Mr. Songyun treated according to symptoms, and the major condition stabilized. Since then, only the Stomach Qi has not revived; nothing is eaten. Various methods of harmonizing the middle, resolving dampness, warming and regulating Middle Yang, and guiding turbidity downward were tried. Regarding stomach intake, care was meticulous, yet Stomach Qi remained unrevived. Now carefully observing the condition, besides anorexia, there is dizziness preventing turning the head to the left, acid regurgitation, nausea, Qi attacking and distending the middle epigastrium, and abdominal pain. The pulse is slightly rapid, the right Guan is wiry, the Chi is softer and slightly larger, and the tongue coating is yellow and turbid. This is likely due to the aftermath of headache, where Liver Wood is not pacified, insulting Stomach Earth, causing Yangming to lose its function of descending. Here, discussed with Mr. Songyun, based on previous methods, incorporating Qi-regulating and Liver-pacifying herbs. Please correct if appropriate.
Prescription: Processed Pinellia Tuber, Cloud Poria, Coptidis Rhizoma, Cyperi Rhizoma, New Tangerine Peel, Chaenomelis Fructus, Fried Aurantium Fructum, Earth-fried White Peony Root, Ground Agarwood, Tribuli Fructus (thorns removed, fried), Zhu Er Qing (salt-water fried).
Patient (Right): Headache as if breaking. The first turn was resolved by eliminating Wind, the second by ascending and dispersing. What is dispersed? The depression of Fire. Why is it attributed to Wind? Swollen gums are the evidence. Is it because Wind-eliminating agents worked initially but failed later, thus guessing it is Fire? No. Initially, the mind was clear; later, pain became severe, accompanied by delirious speech. Only Fire can disturb the Divine Mind; even strong Wind cannot disturb the Heart. The Classic says, "For Fire depression, disperse it," hence the daring attempt with Bupleuri Radix and Cimicifugae Rhizoma. Fortunately, all responded well, worthy of celebration. Specifically, although the headache is settled, wandering pain throughout the body suggests that if it were Blood failing to nourish the vessels, the pain would be fixed and slightly slower. Now the wandering is fast, indicating the residual power of Wind-Fire窜入 (intruding) into the collaterals. The pulse is rapid, fine and wiry upon heavy pressure, slightly floating upon light pressure, consistent with the evidence. Propose draining Heat and dispelling Wind to eliminate remaining embers.
Prescription: Gentianae Macrophyllae Radix, Bombyx Batryticatus, Taxilli Herba, Angelicae Pubescentis Radix, Saposhnikoviae Radix, Moutan Cortex, Scutellariae Radix, Gardeniae Fructus, Forsythiae Fructus, Qin Guo, Reed Rhizome.
Left: Neck and nape pulling into head pain, ear orifice swelling. Liver Fire-Wind depressed in Shaoyang and Yangming.
Prescription: Mori Folium (1.5 qian), Gardeniae Fructus (3 qian), Schizonepetae Spica (1 qian), Scutellariae Radix fried with wine (1.5 qian), Chrysanthemum (2 qian), Moutan Cortex (2 qian), Kudingcha (3 qian), Scrophulariae Radix (3 qian), Forsythiae Fructus (4 qian), Lotus Leaf Edge (3 qian).
Patient Qian (Right): History of stomach pain, recurring intermittently, hardness in the left abdomen, headache worse on the right, sometimes radiating to eye pain. Pulse shape is choppy at Chi. Liver Fire-Wind rotates upward. Should clear and drain.
Prescription: Mulberry Leaf (1 qian), Gardeniae Fructus (3 qian), Chrysanthemum (1.5 qian), White Peony Root fried with wine (1.5 qian), Moutan Cortex (2 qian), Fine Rehmanniae Radix (4 qian), Eyebright Seed fried with wine (3 qian), Viticis Fructus (1 qian), Large Yams (3 qian), Lotus Leaf Edge (3 qian).
Second visit: Pulse wiry, Chi choppy. Right-sided headache radiating to eyes, dry and painful throat after slight exertion. Liver Fire-Wind not extinguished. Nourish insufficient Yin, drain excessive Heat.
Prescription: Fine Rehmanniae Radix (4 qian), Hang White Peony Root fried with wine (1.5 qian), Chrysanthemum (2 qian), Moutan Cortex (2 qian), Viticis Fructus (1.5 qian), Eyebright Seed fried with wine (3 qian), Scutellariae Radix fried with wine (1.5 qian), Polygonatum Odoratum (3 qian), Gardeniae Fructus (3 qian), Wild Black Soybean (3 qian), Lotus Leaf Edge (3 qian).
Patient Zhang (Left): Earth depression slightly relieved, headache comes and goes. "Below the position of Earth, Dryness Qi takes over."
Prescription: Curcumae Radix, Notopterygii Rhizoma et Radix, Atractylodes Macrocephala Rhizoma, Alisma Rhizoma, Processed Pinellia Tuber, Upper Guang Pi, Fried Coix Seed, Red Polyporus, Late Silkworm Sand (wrapped), Fan Zhi Qu, Tribuli Fructus.
Right: After throat ulcer, Wind-Fire not cleared. Wind connects to Liver, causing Fire-Wind to wander meridians, headache as if breaking, sometimes forming lumps anywhere, as stated "severe heat causes swelling."
Prescription: Ligustici Chuanxiong Rhizoma, Antelopis Cornu, Moutan Cortex, Viticis Fructus, Gentianae Macrophyllae Radix, Gardeniae Fructus, White Bombyx Batryticatus, Saposhnikoviae Radix, Angelicae Dahuricae Radix, Chrysanthemum.
Second visit: Headache reduced, but Qi rushes from lower abdomen to throat, difficulty sleeping. Pulse surging and large but slightly收敛 (converging), Guan still wiry. Liver Fire-Wind not fully pacified, Jue Qi follows suit. Combine previous method with Qi-regulating.
Prescription: Angelicae Dahuricae Radix, White Peony Root, Moutan Cortex, Ligustici Wallichii Rhizoma, Chaenomelis Fructus, Fresh Chrysanthemum, Gardeniae Fructus, Angelicae Sinensis Radix, Cyperi Rhizoma, Citrus Reticulata Green Fruit, Loquat Leaf.
Right: Guide Fire downward. Sleep slightly improved, but headache remains severe, vomiting and coughing. Pulse fine and choppy, left section wiry. Essentially Yang failing to submerge. Add reverse adjuvant to advance.
Prescription: Antelopis Cornu (1 qian, decocted first), Broad Tangerine Peel (1 qian), Leucophaeite (3 qian), Old Phellodendri Cortex processed with bile (0.5 qian), Left Oyster Shell salt-fried (8 qian), Poria and Alisma Rhizoma (3 qian each), Fried Trichosanthis Pericarpium (3 qian), Halitum (5 qian), Bamboo Juice (1 liang), Ginger Juice (small amount).
Patient (Right): Elderly patient with left-sided headache. Excessive childbirth leads to Blood deficiency, Liver lacks nourishment, Yang Qi usurps upward.
Prescription: Angelicae Sinensis Radix fried with wine, Angelicae Dahuricae Radix honey-fried, Chrysanthemum, White Bombyx Batryticatus, Ligustici Chuanxiong Rhizoma honey-fried, White Peony Root fried with wine, Viticis Fructus, Turtle Shell Heart, Charred Rehmanniae Radix.
Patient Sun (In): Forehead headache is severe, nasal passages blocked. Right pulse wiry and large. Constitutionally deficient Yin, external Wind stirs internal Wind. Use Xuanqi Tang with modifications.
Prescription: Light Fermented Soybean (3 qian), Scutellariae Radix (1.5 qian), Black Soybean Coat (3 qian), Dendrobii Caulis (4 qian), Saposhnikoviae Radix (1 qian), Chrysanthemum (2 qian), Ligustici Wallichii Rhizoma (1 qian), Bamboo Shavings water-fried (1 qian), Dried Lotus Leaf Edge (3 qian), Scallion White (2 pieces).
