Lin (Right): The Ying-blood has been deficient for a long time, causing the Liver Wood to lose nourishment. Wind-Yang is stirring violently, invading the meridians and collaterals, resulting in soreness and pain throughout the body. Now that the Wind-Wood season is in command, Yang Qi is弛张 (relaxed and expanded). Repeated episodes of Jing-Jue (spasm and syncope) have occurred; after the Jue subsides, consciousness remains confused. The pulse is fine, choppy, and thread-like. There is deep concern about the separation of Yin and Yang, and this cannot be regarded as a habitual condition. It is proposed to employ the method of Protecting Spirit and Subduing Yang. Please consult and decide.

Second visit: The Jing-Jue has stabilized, and the expression is clear. However, there is palpitation in the heart, low voice, and weak Qi. The deficiency is extreme; this prescription is merely applied tentatively.

Third visit: After the Jing-Jue, white rashes appeared on the body, indicating extreme deficiency of Middle Qi due to prolonged illness. Recurrent fever, with a deficient and minute pulse, shows insufficient Yin and excessive Yang. Both Qi and Yin should be addressed.

Xu (Left): Internal Wind carrying Phlegm diffuses into the Heart orifices, causing dullness of spirit and expression. There is still fear of internal closure leading to coma and spasm.

Certain Patient: Occasional Jing-Jue occurs; during Jue, the four limbs convulse and contract, and consciousness is lost. This is Liver Wind carrying Phlegm. It is suitable to dispel Wind and resolve Phlegm.

Jiang (Right): The constitution has always been deficient. At the time of Spring ascent, Wind-Yang stirs violently, causing sudden Jing-Jue. Jia Wood fails to descend, and the Gallbladder loses its power of decision-making, leading to palpitations, timidity, and fear. Tangible Phlegm is stirred up, hence the wiry and slippery pulse, red tongue, and yellow turbid coating. It is proposed to resolve Phlegm, calm the Spirit, subdue Yang, and extinguish Liver Wind.

Second visit: Gradually able to sleep peacefully, but the expression still seems dull. The coating is yellow, greasy, and turbid, with a moldy black center. This indicates that Liver Fire and Phlegm-Heat remain unresolved. Again, resolve Phlegm, extinguish Liver Wind, calm the Spirit, and stabilize the Will.

Certain Patient: Qi rushes upward, causing obstruction in the epigastrium, and Phlegm surges triggering Jue. This is Jue Qi carrying Phlegm disturbing the Stomach; quick success cannot be sought.

Tortoise Plastron Tribulus Terrestris Fruit Softshell Turtle Carapace Tangerina Peel Poria with Hosta Root Moutan Cortex Celosia Seed Oyster Shell Pinellia Rhizome Gold Artifact
Second visit: Since subduing Yang and calming the Liver, the Jing-Jue resembles epilepsy, showing that Phlegm rises with Wind, Wind moves due to Fire, Fire generates from Wood. Wood is dry, Water is deficient, Fire and Wind arise frequently. Since the medicine is effective, it is appropriate to expand further.
Raw Softshell Turtle Carapace Roasted Tortoise Plastron Tribulus Terrestris Fruit Moutan Cortex Raw and Cooked Licorice Root Raw Oyster Shell Black Soybean Hull Celosia Seed Gold Artifact
Third visit: Although Jing-Jue has recurred, the momentum has decreased by seven-tenths. Again, subdue Yang and extinguish Liver Wind.

Wu (Left): Wood Qi is depressed and unsmooth, Liver Qi is stagnant and un-dredged, causing discomfort in the abdomen and a feeling of stagnation and heaviness below the umbilicus. The Gallbladder is the outer mansion of the Liver, governing the opening and closing of Yin and Yang. When the Liver is diseased, the pivot of opening and closing in the Gallbladder meridian fails, hence Jue precedes Heat. Qi depression transforms into Fire, which harasses Lung Metal, causing hemoptysis of two mouthfuls recently. It is proposed to combine clearing and nourishing with soothing depression.
Dendrobium Officinale (Jin Shi Hu, 4 qian) Raw White Peony Root (1.5 qian) Corydalis Rhizome (Yan Hu Suo, wine-fried, 1.5 qian) Chaenomeles Fruit (Jin Ling Zi, 1.5 qian) Dried Tangerine Leaf (1 qian) Privet Fruit (3 qian) Asparagus Tuber (Da Tian Dong, 2 qian) Curcuma Aromatic Rhizome (1.5 qian) Fried Bitter Orange (0.7 qian) Xiao Yao Wan (Free and Easy Wanderer Pill, 3 qian, taken in two doses)
Tang (Right): Whenever palpitations occur, Qi rushes to the throat, causing choking cough and vomiting, immediately followed by pallor and sweating, resembling impending Jue. After Jue, tinnitus and dizziness occur, with a choppy pulse at the Chi position and wiry pulse at the Guan position. This is excessive rising of Jue-Yang. It is proposed to regulate Qi and submerge it.
Prepared Cyperus Rhizome Fried Bitter Orange Magnetite Earth-fried White Peony Root Fried Ziziphus Seed Cinnabar-coated Poria with Hosta Root Left-side Oyster Shell Chaenomeles Fruit Top-grade Broad-leaved Mandarin Peel Fried Bamboo Shavings
Follow-up visit: Previously, there was again impending Jue, choking cough and Qi rush, only settling after vomiting saliva. It is undoubtedly Liver Yang counter-attacking and harassing the Stomach. When wind flies, waves surge; this is the origin of vomiting. Although the medicine advances and Jue still occurs, the intensity is slightly lighter, which may be considered an improvement. Again, submerge the Yang and transform the fluid retention.
Prepared Cyperus Rhizome Poria with Hosta Root Prepared Pinellia Rhizome Top-grade Broad-leaved Mandarin Peel Amomum Fruit Powder Magnetite Dragon Bone Fried Bitter Orange Left-side Oyster Shell (salt-fried)
Certain Patient: The nature of alcohol is ascending and hot. Excessive drunkenness, combined with the period when Ministerial Fire is in command, stirs up Heart Fire and Liver Yang. Fire, Wind, and Heat become internally connected and closed, causing coma, locked jaws, silence, and even convulsions and spasm. Although the spasm settles, coma and closure persist with loss of consciousness, tossing of limbs, red eyes, malar flush, and constipation. The pulse is rapid, wiry, and large. Fire, Wind, and Heat are blazing internally; this is a Jue syndrome, extremely urgent and dangerous. Urgently, clear heat and descend fire, while opening the collaterals and orifices.
