(Attached: Symptoms of weeping and laughing)
Patient [Surname omitted], experiencing vertigo and sudden collapse, with profuse salivation and limb convulsions. The onset is so sudden that there is no time to prepare; after the episode, the patient returns to normal. This indicates wind-phlegm entering the collaterals, presenting as epilepsy and syncope (Xian Jue, 痫厥). The condition is difficult to completely arrest.
Prescription: Zhi Banxia (Prepared Pinellia Rhizome), Fuling (Poria), Jiangcan (Bombyx Batryticatus), Baijili (Tribulus Terrestris), Gouteng (Uncaria Rhynchophylla), Yuanzhi (Polygala Root), Juhong (Citrus Reticulatae Pericarpium Rubrum), Chen Danning (Aged Arisaema Cum Bile), Tianma (Gastrodia Elata), Jiujie Changpu (Acorus Tatarinowii Rhizoma).
Patient Zheng, following an attack of infantile convulsion (Jing Feng, 惊风), wind-phlegm has entered the collaterals, resulting in a stiff tongue and inability to speak. Movements and gait resemble those of a puppet. Currently, spasmodic syncope occurs intermittently. During syncope, the malar region becomes red as if fire is rising, eyes deviate, mouth opens, hands relax, and limbs become cold (Si Zhi Jue Ni, 四肢厥逆). The pulse is fine, wiry, and weak. Within the collateral tunnels, although wind-phlegm obstructs internally, the Liver Yin and Kidney fluids are already deficient, leading to the stirring up of wind pathogen. The proposed treatment is to nourish Yin and subdue Yang.
Prescription: Sheng Guiban (Raw Turtle Shell, 6 qian), Baishao (White Peony Root, 2 qian), Chuan Beimu (Fritillaria Cirrhosa Bulb, 2 qian), Fuling (Poria, 3 qian), Da Dancai (Large Mytilus Edulis, wine-washed, 2 pieces), Sheng Muli (Raw Oyster Shell, 8 qian), Cishi (Magnetite, 3 qian), Juhong (Citrus Reticulatae Pericarpium Rubrum, 1 qian), Ejiao (Donkey-Hide Gelatin, 2 qian), Jinqi (Gold Artifact, 1 piece).
Second visit: Using shell-type minerals to subdue Yang Qi, but syncope and collapse persist. Wind-phlegm entering the collaterals is a stubborn disease. The prescription strategy should be "retreat to advance."
Prescription: Zhuli Banxia (Pinellia Rhizoma cum Bile et Phragmitis Rhizome, 1.5 qian), Chen Danning (Aged Arisaema Cum Bile, 0.7 qian), Yujin (Curcuma Longae Radix, 1.5 qian), Jiangcan (Bombyx Batryticatus, 3 qian), Zhuhuang (Tianzhu Huang/Artemisia Annua Honeydew, 3 qian), Wei Tianma (Calcined Gastrodia Elata, 1.5 qian), Bai Fuling (White Poria, 3 qian), Baijili (Tribulus Terrestris, 3 qian), Zhenxin Dan (Calming Heart Pill, 1 pill).
Third visit: The pulse is wiry and slippery; epileptic syncope still occurs. Wind-phlegm entering the collaterals is difficult to treat.
Prescription: Chen Danning (Aged Arisaema Cum Bile, 0.5 qian), Tianzhuhuang (Artemisia Annua Honeydew, 3 qian), Zhi Banxia (Prepared Pinellia Rhizome, 1.5 qian), Jiangcan (Bombyx Batryticatus, 3 qian), Baijili (Tribulus Terrestris, 3 qian), Wei Tianma (Calcined Gastrodia Elata, 1.5 qian), Guang Juhong (Broad-spectrum Citrus Reticulatae Pericarpium Rubrum, 1 qian), Fuling (Poria, 3 qian), Shichangpu (Acori Tatarinowii Rhizoma, 0.4 qian), Gouteng (Uncaria Rhynchophylla, 4 qian), Yuanzhi (Polygala Root, 0.5 qian).
Separately take powder: Zhi Nanxing (Processed Arisaema, 0.8 fen), Zhi Xiewei (Roasted Scorpion Tail, de-toxified, 2 tails), Chensha (Cinnabar, 0.2 fen), Jinbo (Gold Leaf, 2 sheets), Xihuang (Calculus Bovis, 0.04 li), Bashuang (Crotonis Fructus Resina, 0.03 li). Grind into extremely fine powder. Take 1 fen per dose, mixed with warm water.
Patient Tang (Left pulse): Slight anger causes Liver Yang to rebel upwards. Yang Qi fails to enter Yin, resulting in insomnia (Wu Bu Cheng Mei, 寤不成寐). The pulse is wiry; the coating is white with a yellow center. There is fear that turbid phlegm may reverse upwards at any time, leading to epilepsy.
Prescription: Zhi Banxia (Prepared Pinellia Rhizome, 3 qian), Chao Zhishi (Stir-fried Immature Bitter Orange, 1 qian), Qinglongchi (Green Dragon Tooth/Fossilized Bone, 4 qian), Chao Fei Zhimu (Stir-fried Large Fritillaria Bulb, 2 qian), Suanzaoren (Ziziphus Seed, 2 qian, stir-fried with Pig Bile), Juhong (Citrus Reticulatae Pericarpium Rubrum, 1 qian), Chen Danning (Aged Arisaema Cum Bile, 0.8 fen), Yejiaoteng (Polygonum Multiflorum Vine, 4 qian), Zhusha Anshen Wan (Cinnabar Calming Spirit Pill, 2 qian, taken with warm water).
Second visit: After reducing fire and resolving phlegm, sleep has slightly improved. However, the chest still feels stuffy, with occasional irritability. The pulse is wiry and slippery. The Yin aspect has been deficient since long ago, and Shaoyang Fire, carrying phlegm, disturbs internally. During the spring season when Yang Qi ascends, the condition is likely to be complicated.
Prescription: Zhuli Banxia (Pinellia Rhizoma cum Bile et Phragmitis Rhizome, 2 qian), Guang Juhong (Broad-spectrum Citrus Reticulatae Pericarpium Rubrum, 1 qian), Hei Shanzhi (Black Gardenia Fruit, 3 qian), Jiao Shumi (Charred Sorghum, wrapped in silk, 2 qian), Zhu Fushen (Poria with Hostwood coated with Cinnabar, 3 qian), Danzhi Chao Zaoren (Ziziphus Seed stir-fried with Bile, 2 qian, crushed), Chao Zhimu (Stir-fried Fritillaria Bulb, 1.5 qian), Xian Zhuru (Fresh Bamboo Shavings, 1 qian), Zhenzhumu (Mother-of-Pearl, 3 qian, crushed).
Third visit: Insomnia and other symptoms have largely subsided; the pulse feels soft. It is indeed internal disturbance by Phlegm-Heat. Continue the effective prescription.
Prescription: Zhuli Banxia (Pinellia Rhizoma cum Bile et Phragmitis Rhizome, 3 qian), Chen Danning (Aged Arisaema Cum Bile, 0.6 fen), Fuling (Poria, 4 qian), Danzhi Chao Zaoren (Ziziphus Seed stir-fried with Bile, 3 qian), Yejiaoteng (Polygonum Multiflorum Vine, 3 qian), Zhimu (Fritillaria Bulb, 2 qian), Zhishi (Immature Bitter Orange, 1 qian), Jiao Shumi (Charred Sorghum, 3 qian).
Patient Zhu (Left pulse): Insomnia and mental restlessness have largely subsided; the pulse feels slightly soft. However, the Left Cun position remains wiry and large, indicating Heart Fire is not yet calm. Further treatment should calm the Spirit and drain heat. If Heart Fire descends, Kidney Water will naturally secure itself.
Prescription: Zhuzhi Chao Suanzaoren (Ziziphus Seed stir-fried with Pig Bile, 2 qian), Fen Danpi (Moutan Cortex, 2 qian), Hei Shanzhi (Black Gardenia Fruit, 3 qian), Zhuli Banxia (Pinellia Rhizoma cum Bile et Phragmitis Rhizome, 2 qian), Kuai Chensha (Chunk Cinnabar, wrapped in silk, 3 qian), Yejiaoteng (Polygonum Multiflorum Vine, 3 qian), Fuling Shen (Poria with Hostwood, each 2 qian), Xi Shengdi (Young Rehmannia Root, 4 qian, stir-fried until loose), Chuan Yalian (Coptis Chinensis Rhizoma, 0.3 fen), Dengxin (Juncus Effusus Medulla, 5 chi).
Left pulse: Qi rises upward, causing frequent weeping and profuse phlegm flow. This is Liver Qi carrying phlegm invading the Lung. Only a broad-minded attitude can achieve efficacy.
Prescription: Daizheshi (Hematitum, 4 qian), Gouteng (Uncaria Rhynchophylla, 2 qian), Muli (Oyster Shell, 4 qian), Xuanfuhua (Inula Flower, 2 qian), Dong Baishao (East White Peony Root, 1.5 qian), Sheng Xiangfu (Cyperi Rhizoma, 2 qian), Jiuye (Citrus Reticulata Leaf, 1 qian), Longgu (Fossilized Mammoth Bone, 3 qian), Baijili (Tribulus Terrestris, 3 qian), Chao Zhuru (Stir-fried Bamboo Shavings, 1 qian).
This case is very peculiar: the syndrome manifests as weeping, and severe weeping leads to gradual recovery. Because Wood-Fire invades the Lung, and the Lung governs grief, excessive weeping vents the Wood Qi, thus leading to recovery.
Patient [Surname omitted]: After a damp-heat illness, damp-phlegm remains unresolved. Liver Fire carries phlegm upward, causing weeping and syncope. After regaining consciousness, the pulse remains wiry and rapid. Phlegm-fire is not yet pacified. Clear and drain it.
Prescription: Zhi Banxia (Prepared Pinellia Rhizome), Fuling Shen (Poria with Hostwood), Zhenzhumu (Mother-of-Pearl), Guang Yujin (Curcuma Longae Radix), Nanxing (Arisaema), Chao Zhishi (Stir-fried Immature Bitter Orange), Chao Zhuru (Stir-fried Bamboo Shavings), Kuai Zhusha (Chunk Cinnabar), Qingguo Zhi (Green Fruit Juice).
Left pulse: During sleep, there is involuntary laughter; upon slight disagreement, anger arises. This indicates excess Fire in the Jueyin and Shaoyin meridians.
Prescription: Chen Maidong (Ophiopogon Japonicus Rhizoma), Zhu Fushen (Poria with Hostwood coated with Cinnabar), Chao Loupi (Stir-fried Trichosanthis Pericarpium), Qingge San (Blue Clam Powder), Guang Xingren (Apricot Seed), Fen Danpi (Moutan Cortex), Guang Yujin (Curcuma Longae Radix), Fenghua Xiao (Efflorescent Mirabilite), Pipaye (Loquat Leaf).
