Prepared Pinellia Rhizome (Zhi Banxia), Polyporus with Hostwood (Zhu Fushen), Prepared Cyperus Rhizome (Zhi Xiangfu), Tribulus Fruit (Bai Jili), Citron Peel (Xiangyuan Pi), Chrysanthemum Morifolium from Chuzhou (Chu Juhua), Tangerine Peel (Guangpi), Eucommia Bark (Duzhong), Mulberry Leaf (Sangye), Moutan Cortex (Danpi), Dried Lotus Leaf Edge (Gan Heyebian), Bamboo Shavings Stir-fried in Salt Water (Yanshui Chao Zhuru).
Patient Xi (Right pulse): Abdominal distension resulted from epigastric pain, relieved after defecation. This is disharmony between the Liver and Spleen, with Qi and Dampness obstructing and failing to circulate. Qi is the commander of Blood; thus, menstruation fails to arrive on time. Treatment should prioritize regulating Qi.
Prescription: Prepared Cyperus Rhizome (2 qian), Amomum Fruit (5 fen), Salvia Root (Danshen), Calamus Wood (Sumu), Aurantium Fruit (Zhike), Poria (Fuling), Fresh Finger Citrus (Xian Foshou), High-quality Tangerine Peel (Shang Guangpi), Aucklandia Root (Muxiang), Dalbergia Odoriferous Lignum (Jiangxiang).
Second visit: Abdominal distension has eased somewhat, but there remains a stifling pain in the middle epigastrium. Continue treating from the perspective of the Liver, Spleen, and Stomach. The absence of menstruation need not be over-concerned at this stage.
Prescription: Prepared Cyperus Rhizome (2 qian), Tangerine Peel (1 qian), Bellflower Fruit (Jinlingzi), Peucedanum Root (Qianhu), Fresh Finger Citrus (1 qian), Amomum Fruit (5 fen), Corydalis Rhizome (Yanhusuo, stir-fried in wine, 1.5 qian), Apricot Seed (Guang Xingren, 3 qian, crushed), Red Sage Root (Zi Danshen), Perilla Stem (Sugeng, 2 qian).
Patient Qian (Right pulse): Menstruation is delayed, accompanied by abdominal pain and a stagnant, heavy sensation below the umbilicus, exacerbated by pressure. This indicates stagnation of Qi in the Chong Vessel. Temporarily employ methods to diffuse and unblock.
Prescription: Charred Rehmannia Root (Shudi Tan), Red and White Paeonia Roots (Chibai Shao, stir-fried in wine, 1 qian each), Prepared Cyperus Rhizome (2 qian, crushed), Taizhou Lindera Root (Tai Wuyao), Charred Hawthorn Fruit (Nanzha Tan), Whole Angelica Sinensis Root (Quang Danggui), Ligusticum Rhizome (Chuanxiong), Dalbergia Odoriferous Lignum slices (7 fen), Shangyao Cinnamon Twig (4 fen, taken as rice pill).
Second visit: Lower abdominal pain persists, and menstruation has not started. Again, diffuse and unblock Qi and Blood.
Prescription: Prepared Cyperus Rhizome (2 qian), Lindera Root (1.5 qian), Cinnamon Wood (Chuan Guimu), Leonurus Fruit (Chongweizi), Small Fennel Fruit (Xiaohuixiang), Corydalis Rhizome (Yanhusuo, stir-fried in wine, 1.5 qian), Amomum Fruit (5 fen), Landside Mint Leaf (Zelan Ye), Dalbergia Odoriferous Lignum slices (7 fen), Charred Hawthorn Fruit (3 qian).
Third visit: Menstruation has arrived but is still not smooth, with persistent lower abdominal pain. This is obstruction of Ying (Nutritive) Qi. Continue to diffuse and unblock.
Prescription: Whole Angelica Sinensis Root (Quang Danggui, stir-fried in wine, 2 qian), Lindera Root (1.5 qian), Stir-fried Small Fennel Fruit (5 fen), Roasted Ginger (Paojiang), Ligusticum Rhizome (1 qian), Cinnamon Twig (Chuan Guizhi, 3 fen), Cyperus Rhizome (2 qian), Red Sage Root (Zi Danshen, 2 qian), Leonurus Fruit (3 qian), Motherwort Herb (Yimucao, 6 qian).
Patient Jie (Right pulse): Postpartum Blood deficiency with Qi stagnation, causing frequent abdominal distension. Upon menstruation, blood loss is excessive. Since Qi commands Blood, it is appropriate to regulate Qi.
Prescription: Angelica Sinensis Root (2 qian), Stir-fried Ziziphus Seed (Chao Zaoren, 2 qian), Black Soybean Coat (Heidouyi), Strong Eucommia Bark (Nong Duzhong, 3 qian), Charred Rubia Root (Qiancao Tan), Tuckahoe (Tong Shayuan, 3 qen), Chrysanthemum Morifolium (Chi Juhua, 1.5 qian), Cuttlefish Bone (Wuzeigu, 3 qian), Huoxiang Zhengqi Pill (take 3 qian first), Polyporus with Hostwood (Fushen, 3 qian).
Second visit: Menstrual flow this time was not excessive, yet abdominal distension persists, sometimes with Qi rushing up to the epigastrium. The Qi of the Chong Vessel remains unsettled; slowly discuss tonification and regulation.
Prescription: Prepared Cyperus Rhizome (3 qian), Earth-fried White Paeonia Root (Tu Chao Baishao, 1.5 qian), Stir-fried Aurantium Fruit (Chao Zhike, 1 qian), Tangerine Peel (Guangpi, 1 qian), Polyporus with Hostwood (Fushen, 3 qian), Stir-fried Ziziphus Seed (Chao Zaoren, 3 qian), Bellflower Fruit (Jinlingzi, 3 qian), Amomum Shell (Sharen Ke, 5 fen), Four-Processed Cyperus Pill (Sizhi Xiangfu Wan, take 2 qian in the morning), Papaya Skin (Muqupi, 1 qian, stir-fried).
Third visit: Yang Qi is about to recover, but Liver Yang is unbalanced, causing abdominal distension, epigastric pain, dizziness, and vertigo. Normally, menstrual volume is excessive. All these stem from excess Qi and Fire in the Liver Meridian. Again, extinguish Liver Wood (calm Liver Yang).
Prescription: Chinaberry Seed (Chuanlianzi, 1.5 qian), Earth-fried White Paeonia Root (1.5 qian), Black Soybean Coat (3 qian), Black Gardenia Fruit (Heishanzhi, 1.5 qian), Prepared Cyperus Rhizome (2 qian), Chrysanthemum Flower (1.5 qian), Mandarin Peel (Ju Pi, 1 qian), Stir-fried Aurantium Fruit (Chao Zhike, 1 qian), Dried Lotus Leaf Edge (3 qian), Privet Fruit (Nvzhenzi, 3 qian).
Patient Zhu (Right pulse): Abdominal distension before menstruation, leukorrhea, sore waist, palpitations, dizziness, insomnia, and heart pain. This is Qi stagnation and Blood deficiency, where Blood fails to nourish the Liver. The Extraordinary Vessels belong to Liver Wood; when Liver Wood is hyperactive, Yang Qi floats upward, and the Dai Vessel fails to secure downward. Propose to supplement insufficient Blood and disperse excessive Qi.
Prescription: Kuei Dangshen (5 liang), Black Soybean Coat (2 liang), Honey-fried Rehmannia Root (Zhashengdi, 3 liang), Large Asparagus Root (Datiantong, 2 liang), Xinhui Tangerine Peel (1 liang), Whole Angelica Sinensis Root (3 liang), Honey-fried Black Licorice Root (Zhei Heicao, 7 qian), Dendrobium Stem (Chuanshihu, 3 liang), Chrysanthemum Morifolium (1 liang), Achyranthes Root (Chuanduanrou, 3 liang), Stir-fried Yam (Chaoshanyao, 3 liang), Tuckahoe (3 liang), Strong Eucommia Bark (3 liang), Ligusticum Rhizome slices (1 liang), Cloud Polyporus with Hostwood (Yun Fushen, 3 liang), Large Prepared Rehmannia Root (Sandao Chao, 5 liang), Dodder Seed (Tusizi, salt-water fried, 3 liang), Wild Atractylodes Macrocephala Rhizome (Ye Yuzhu, 2 liang, stir-fried with decoction of Aucklandia Root), Stir-fried Cornus Fruit (Chao Yurou, 1.5 liang), Euryale Seed (Jitouzi, 1.5 liang), Hangzhou White Paeonia Root (Hang Baishao, 1.5 liang), Dry Cistanche Stem (Gan Congrong, 1.5 liang), Prepared Cyperus Rhizome (3 liang, decocted separately and added), Alisma Rhizome (Zexie Pian, 1 liang), Stir-fried Ziziphus Seed (1 liang, ground), Goji Berry (Ganqizi, 3 liang), Amomum Powder (7 qian, finely ground, mixed into paste during collection), Deer Antler Glue (Lujiao Jiao, 1 liang), Tortoise Shell Glue (Guiban Jiao, 3 liang), True Donkey-hide Glue (Zhen Ejiao, 3 liang).
Decoct the above herbs to obtain a thick juice, dissolve the three glues, and boil down to an old paste. Take one spoonful every morning.
Patient Zhou (Right pulse): Menstruation flows very smoothly, stasis is transformed and cleared, and lower abdominal pain/heaviness has stopped. Although accumulated stasis is cleared, new blood flows out with it, inevitably causing collateral damage (like burning good and bad weeds together), leading to rising Wind-Yang, tinnitus, and dizziness. Once the weeds are removed, one should plant good grain (nourish healthy Qi).
Prescription: White Angelica Root (Baiguishen, 2 qian), Cuttlefish Bone (3 qian), Achyranthes Root (3 qian), Privet Fruit (3 qian), Eclipta Herb (Hanliancao, 3 qian), Black Soybean Coat (3 qian), Fried Donkey-hide Glue Bead (Ejiao Zhu, 2 qian), Tuckahoe (salt-water fried, 3 qian), Polyporus with Hostwood (3 qian), Perilla Stem (2 qian), Charred Typha Pollen (Puhuang Tan, 5 fen), Raw Atractylodes Macrocephala Rhizome (Sheng Yuzhu, 2 qian).
Patient Wang (Right pulse): Menstruation is delayed, with sore waist and leukorrhea, aversion to cold, pale and lusterless blood color. This is deficiency of Qi and Blood. It is suitable to nourish Blood and warm the menses.
Prescription: Whole Angelica Sinensis Root (2 qian), Achyranthes Root (3 qian), Oyster Shell (Muli, 4 qian), Red Sage Root (Zi Danshen, 2 qian), White Paeonia Root (1.5 qian), Strong Eucommia Bark (3 qian), Stir-fried Yam (3 qian), Ligusticum Rhizome (1 qian), Shiquan Dabu Pill (10 Tonics Pill, 3 qian, taken in two doses with water).
Case Note: Menstruation arrives early and flows continuously/dribbles. The Chong and Ren vessels are insecure; urgent measures should be avoided.
Prescription: Charred Rehmannia Root (4 qian), Charred Angelica Sinensis Root (2 qian), Polyporus with Hostwood (3 qian), Polygala Root (Yuanzhi Rou, 5 fen), Charred Cuttlefish Bone (3 qian), Luodangshen (Roasted with millet, 3 qian), Stir-fried Winter Atractylodes (2 qian), Stir-fried Ziziphus Seed (2 qian), Longan Aril (4 pieces), Old Ginger (2 slices), Buzhong Yiqi Pill (Tonify Middle and Augment Qi Pill, 3 qian, taken in the morning).
Case Note: Menstruation arrives early, with severe chills and rising fire, belching, and dizziness. Tongue coating is yellow, pulse is wiry, and the Chi position is choppy. This indicates insufficiency of Liver Yin and failure of Gallbladder and Stomach Qi to descend properly. Propose to diffuse and descend Yang, and clarify/assist in draining Liver Wood.
Prescription: Prepared Pinellia Rhizome, Cloud Polyporus with Hostwood, Cassia Seed (Jueming), Raw Gardenia Fruit (Sheng Shanzhi), Honey-fried Turtle Shell (Zhe Biejia), Uncaria Ramulus with Hook (Gougou), Moutan Cortex, Tangerine Peel, Bamboo Shavings Stir-fried in Salt Water.
Patient Ding (Right pulse): Menstruation is delayed, mostly due to deficiency-cold. However, deficiency manifests as weak limbs or slight fever; cold manifests as abdominal pain and a deep, fine pulse. Now, menstruation is late, similar to normal people, except for soreness in the four limbs. The pulse is soggy and slippery. This indicates Phlegm-Dampness occupying the Sea of Blood, preventing the free circulation of Ying and Wei Qi. It is appropriate to regulate Qi, resolve Phlegm, and expel Dampness, treating the cause of the blood disorder rather than the blood itself.
Prescription: Fine Whole Angelica Sinensis Root, Gentian Macrophylla Root (Qinjiao), Prepared Pinellia Rhizome, Notopterygium Rhizome (Duohuo), Achyranthes Root, Tribulus Fruit, Alisma Rhizome, Prepared Cyperus Rhizome, Poria, Ligusticum Rhizome.
Patient Zhu (Right pulse): Tian Gui (menstruation) should have arrived but did not, coinciding with prolonged heat. Ying Blood is dry and astringent, leading to internal heat rising, emaciation, constituting a severe case of Ganxue Lao (Dry Blood Consumption Disease).
Prescription: Stir-fried Whole Angelica Sinensis Root (2 qian), Silver Birchbark Root (Yin Chaihu, 5 fen), Stir-fried Red Paeonia Root (1.5 qian), Honey-fried Turtle Shell (4 qian), Mulberry Leaf (1 qian), Red Sage Root (1.5 qian), Corydalis Rhizome (1.5 qian), Stir-fried Anemarrhena Root (Chao Baiwei, 1.5 qian), Powdered Moutan Cortex (1.5 qian).
Patient Zhu (Right pulse): Menstruation dribbles continuously, with sour pain in the lower abdomen, a mass attacking on the right side, and dark purple-red blood color. This is stagnation of Qi in the Chong Vessel. It is appropriate to regulate the Chong and Ren vessels and diffuse Qi stagnation.
Prescription: Fried Donkey-hide Glue Bead (3 qian), Ligusticum Rhizome (1.5 qian), Honey-fried Mugwort Leaf (7 fen), Prepared Cyperus Rhizome (2 qian), Achyranthes Root (3 qian), Charred Rehmannia Root (3 qian), Wine-stir-fried White Paeonia Root (1.5 qian), Dried Tangerine Leaf (1.5 qian), Wine-stir-fried Angelica Sinensis Root (2 qian), Male Clove Bud (Gong Dingxiang, 3 fen).
Second visit: Dribbling continues, with epigastric fullness and lower abdominal sour heaviness. The Qi of the Chong vessel is discordant, and the Chong vessel is insecure. Propose to harmonize Ying and calm Liver Wood.
Prescription: Cuttlefish Bone (3 qian), Spatholobus Stem Paste (Jixueteng Gao, 3 qian, dissolved), Fried Donkey-hide Glue Bead (2 qian), Earth-fried White Paeonia Root (1.5 qian), Tangerine Peel (1 qian), Charred Rubia Root (1.5 qian), Dried Tangerine Leaf (1.5 qian), Pinellia Fermented Curd (Banxia Qu, 1.5 qian, stir-fried in salt water), Prepared Cyperus Rhizome (2 qian), Zuojin Pill (take 5 fen in two doses).
