Whenever excessive consumption of raw and cold food during the summer months damages the Six Fu-organs: injury to the Stomach leads to sudden, profuse diarrhea; injury to the Spleen results in slippery diarrhea with undigested food; injury to the Large Intestine causes white stools accompanied by abdominal pain. In all these cases, Jin Ye Dan (Golden Fluid Pill) and Pi Li Tang (Thunderbolt Decoction) are appropriate, and recovery occurs within three days. If not cured, it develops into Pi Xie (Spleen Diarrhea), requiring urgent moxibustion on Shenque (Belly Button, CV8) for one hundred moxa cones. (Shenque may be a miswriting of Mingguan.) Although the Nan Jing (Classic of Difficult Issues) discusses the Five Types of Diarrhea, it does not transmit treatment methods. For all types of diarrhea, treatment should follow this principle.
Clinical Case
A woman suffered from diarrhea accompanied by delirious speech. Her six pulses were tight and rapid, indicating accumulated heat in the Stomach. Inquiry revealed that her husband had consumed too much pepper and ginger, causing diarrhea. Five days later, she began speaking deliriously. She was prescribed Huang Qin Zhi Mu Tang (Scutellaria and Anemarrhena Decoction) and recovered. (Those who indulgently eat roasted and fried foods and prefer pepper and ginger often have accumulated heat in the Stomach, leading to this syndrome. This is clear in clinical practice, though such cases are rare. Furthermore, those whose diarrhea depletes body fluids, resulting in dry tongue coating, fever, coma, and restless delirium, suffer from severe deficiency of the Spleen and Stomach. The method should be warming and tonifying; if cold-natured herbs are used, collapse will occur immediately.)
When people consume excessive amounts of raw and cold food, damp-heat damages the Spleen and Stomach, leading to dysentery. At the onset, Ru Sheng Bing Zi (Like-Sage Pills) should be taken to expel accumulation and cure the disease. If there is no stool but only red pus is discharged, it indicates severe heat in the Stomach damaging the blood; Dang Gui Shao Yao Tang (Angelica and Peony Decoction) or E Jiao Tang (Donkey-Hide Gelatin Decoction) is suitable. If white pus is discharged, it indicates that cold food has damaged the Large Intestine; Tao Hua Tang (Peach Blossom Decoction) or Quan Zhen Dan (Complete True Pill) should be taken for recovery. If abdominal pain, fever, drowsiness, and rapid, flooding pulses occur with pure discharge of red pus, it is due to heat qi stagnating in the intestines and stomach, named Gangu Dysentery. It can also result from the erroneous use of hot herbs; in such cases, Huang Lian Wan (Coptis Pill) is used, and for severe cases, Da Tong San (Great Unblocking Powder). (Dysentery certainly requires resolving accumulation and clearing heat. Formulas such as Xiang Lian and Cheng Qi, when used appropriately, can indeed achieve immediate cures. However, if the condition involves deficiency-cold of the Spleen and Stomach, sinking of meridians, and loss of transportation by the Triple Burner, warming and tonifying must be prioritized. Generally, errors from hot herbs are easier to correct, whereas errors from cold herbs are difficult to rescue. With a humble and sincere approach, clinical mistakes will naturally be minimized.)
