Treating heat in the Bladder (Bladder organ). Obstruction of the water passages. Scanty and difficult urination with a burning sensation. Urine resembling bean dregs, or forming stones, or becoming like paste/juice, or hematochezia due to heat stagnation.
Wu Lin San (Five Strangulations Powder) uses Gardeniae Fructus (Shanzhiren), while Danggui Radix Angelicae Sinensis, Shaoyao Paeoniae Radix Alba/Rubra, and Fuling Poria are also precious. (Chifuling Red Poria 3 qian, Shaoyao Paeoniae Radix and Shanzhiren Gardeniae Fructus each 2 qian, Danggui Radix Angelicae Sinensis, Xixin Herba Asari, and Gancao Glycyrrhizae Radix et Rhizoma each 1.4 qian. Decoct with Dengxin Grass Junci Medulla in water and take orally.) The transformation of Qi originally relies on Yin for nourishment; regulating and moving the water passages is miraculous and divine.
Ke Yunbo said: The Classics state: "The Bladder is the official of the provincial capital, where fluids are stored." It further elaborates: "Transformation of Qi enables its discharge." Why is this? Because the Bladder has an upper opening but no lower opening; it can receive but not expel. Only Qi acts as the mother of Water. It must be the transformation of Taiyang (Greater Yang) Qi that allows urine from the Bladder to be discharged. Thus, the water passages indeed rely on formless Qi transformation, not solely on the material Bladder itself. The water and fire of the five Zang organs all originate from Qi. When Qi is balanced, it becomes Shaohuo (Lesser Fire); Lesser Fire generates Qi, and Qi becomes Water. The essence of Water spreads throughout the body and flows downward to the Bladder. If the source is clear, the flow is pure. If Qi is excessive, it becomes Zhuanghuo (Blazing Fire); Blazing Fire consumes Qi, causing the source of transformation to lose its reliance. This leads to strangury, difficulty in urination, scanty and painful urination, paste-like urine, bean-dreg-like urine, stones, pus, and blood, making the water passages unfavorable. This is entirely due to the impurity of the source of transformation, not the failure of the Dujiang (water-conducting officials). If treated with formulas such as Bazheng San (Eight-Ingredient Powder for Rectification), Zhouche San (Boat Journey Powder), Yugong San (Great Effort Powder), or Xunchuan San (River-Dredging Powder), the Yin deficiency of the five Zang organs and the Qi transformation of Taiyang will be severed. Therefore, urgently use Zhizi Gardeniae Fructus and Fuling Poria to treat the Heart and Lung, to unblock the Qi of the Upper Jiao (Upper Burner), so that the fire of the five emotions clears. Use Danggui Radix Angelicae Sinensis and Shaoyao Paeoniae Radix to nourish the Liver and Kidney, to calm the Qi of the Lower Jiao (Lower Burner), so that the Yin of the five Zang organs recovers. Use Gancao Glycyrrhizae Radix et Rhizoma to regulate the Qi of the Middle Jiao (Middle Burner), so that Yin and Yang are clarified. Then the Qi of Taiyang transforms naturally, and the water of the Bladder becomes pure. This is a plan to treat the root cause, a method that is perfect.
Also known as Zishen Wan (Kidney-Nourishing Pill). Treating damp-heat in the Lower Jiao (Lower Burner). Inability to pass even drops of urine, leading to abdominal fullness and desire to vomit.
Strangury without thirst indicates clearing the Lower Jiao. (The disease is in the Lower Jiao, hence no thirst. It is appropriate to clear the heat of the Lower Jiao and unblock the water passages.) Zhi Mu Anemarrhenae Rhizoma and Huangbai Phellodendri Cortex move together, assisted by Rouguai Cinnamomi Cortex. (Huangbai Phellodendri Cortex and Zhi Mu Anemarrhenae Rhizoma are both stir-fried with wine, each 2 liang. Rougui Cinnamomi Cortex 2 qian. Make pills using refined honey, the size of paulownia seeds. Take 50 pills at empty stomach with plain warm water. Named Tongguan Wan (Pass-the-Gate Pill).) The pill named Tongguan can promote diuresis; also known as Zishen (Nourishing Kidney) to supplement Yin deficiency. (The original formula was designed for lung萎 (lung flaccidity), hoarseness, throat obstruction, hemoptysis, and irritability. Dong Yuan borrowed it to treat strangury, dyspnea, and distension.)
Chen Xiuyuan said: The urethral orifice is also called the Qi Gate, because urine is expelled through Qi transformation. Qi is Yang. Yang transforms when it obtains Yin. If heat knots in the Lower Jiao, and there is no symptom of thirst above, using this pill to clear the heat of the Lower Jiao will make urine flow like a surge. If this condition presents with thirst, Jisheng Shenqi Wan (Kidney Qi Pill from the Golden Chamber) should be used, primarily Jin Kui Qumai Wan (Qumai Pill from the Golden Chamber). However, there is another ingenious method: imagine a dripping vessel. If the upper orifice is closed, the lower orifice does not flow. Removing the closure of the upper orifice allows water to flow freely. Using Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) or emesis therapy is very excellent. Furthermore, adding Mahuang Ephedrae Herba, which is potent, to diuretic herbs can unblock Yang Qi in the place of extreme Yin. Paired with Xingren Armeniacae Semen, which descends, allowing Lung Qi to reach down to the provincial capital (Bladder). This guides from the high plateau. Its response is immediate. For deficient persons, taking Ren Shen Ginseng Radix and Mahuang Ephedrae Herba, each 1 liang, decocted in water and taken orally, is also excellent. In summer months, taking Suye Perillae Folium, Fangfang Saposhnikoviae Radix, and Xingren Armeniacae Semen, each 3 qian, decocted in water and taken warm, then covering to induce slight sweating, is also excellent.
