Indicated for the gradual dilation of Shen Shui (aqueous humor and related intraocular fluids), visual obscuration as if walking in fog or dew, gradually seeing black spots floating in the air, diplopia (seeing objects as double images), and internal cataracts presenting with Shen Shui appearing pale green or white. Also indicated for tinnitus and deafness. Ke Yunbo stated: This formula is miraculous in treating deafness, epilepsy, mania, and convulsions.
Ci Sha Wan (Magnetite and Cinnabar Pill) best harmonizes Yin and Yang. Shen Qu (Medicated Leaven) can promote the flourishing of Gu Qi (Grain Qi/Spleen-Stomach digestive function). [Formula composition: Ci Shi (Magnetitum, Magnetite) 2 liang, Zhu Sha (Cinnabaris, Cinnabar) 1 liang, Sheng Shen Qu (Levisticum Officinale Medicinal Leaven) 3 liang raw; additionally, 1 liang of Shen Qu is mixed with water to form a cake, boiled until it floats, then added to the previous medicines, and formed into pills with refined honey.] This formula simultaneously treats internal cataracts, floating black spots, and deafness. If used to treat epilepsy, it has extraordinary efficacy.
Wang Youyuan stated: The *Classic of Internal Medicine* records: The essence of the five zang organs and six fu organs all ascend to nourish the eyes. Thus, the ability of the eyes to see relies on Qi; the reason the eyes can see relies on Essence. The Kidney exclusively stores Essence, therefore Shen Shui originates from the Kidney. The Heart is like the light of the Li trigram shining forth, therefore Shen Guang (Spiritual Light/Vision) originates from the Heart. Shen Guang emanates from Yang and projects outward, reflecting and relying on Yin Essence to guard it, thus it does not scatter and remains bright. Shen Shui emanates from Yin and condenses, relying on Yang Qi to distribute it, thus it sees clearly without limit. Only when the Heart and Kidney are deficient does Shen Shui become dry and Shen Guang become scanty, causing symptoms such as blurred vision and internal cataracts. The *Qian Jin Fang* uses Ci Shi (Magnetitum, Magnetite) to directly enter the Kidney meridian, gathering scattered Shen (Spirit); its nature attracts iron and absorbs the Qi of Lung Metal, returning it to store in Kidney Water. Zhu Sha (Cinnabaris, Cinnabar) has a physical nature of Yang but an energetic nature of Yin; it can subdue floating fire and calm the Shen Ming (Spiritual Consciousness). Water acts as a mirror, Fire acts as a candle; when Water and Fire mutually support each other, the brilliance will not scatter outward. However, the eyes receive the essence of the zang-fu organs, which is derived from grains. Shen Qu (Medicated Leaven) can digest the five grains, thereby facilitating the formation of essence. Generally, when Shen Shui dilates, if left slowly it cannot be retracted; it relies on substances with sedative and descending properties to rapidly gather and attract it back, hence this is an emergency rescue formula. Its treatment of tinnitus and deafness also utilizes these sedative and descending functions to restrain the upward rushing of Deficient Yang. Ke Yunbo stated: This pill is a sacred remedy for epilepsy. Mania and dementia are diseases of the Heart, Spleen, and Kidney. The Heart stores Shen (Spirit), the Spleen stores Yi (Intellect/Thought) and Zhi (Wisdom), and the Kidney stores Jing (Essence) and Zhi (Will). The Heart is the master of Shen Ming (Spiritual Consciousness). The *Classic* says: If the Master is unclear, the twelve officials (organs) are endangered, causing the pathways to become blocked and the body to suffer greatly; this refers to that situation. But why is the Master unclear? The Heart corresponds to the Li trigram and belongs to Fire, with True Water hidden within. If the True Water of Tian Yi (Heavenly Number One) is insufficient, the empty fire of Di Er (Earthly Number Two) will act recklessly. This means the Heavenly Qi is obstructed, and the Earthly Qi overpowers the brightness, like the sun and moon losing their clarity, allowing pathogenic factors to harm the orifices, resulting in visual hallucinations and producing such strange diseases. Without heavy mineral and stone sedatives to suppress it, mania will certainly not cease. Zhu Sha (Cinnabaris, Cinnabar) inherits the red color of the South, connects internally with the Heart, and can descend rootless fire to calm the Shen Ming. Ci Shi (Magnetitum, Magnetite) inherits the black color of the North, connects internally with the Kidney, absorbs Lung Metal Qi to generate essence, and descends the fire of Inflaming Earth to stabilize the Will. These two mineral substances are heavy and primarily descending, cold in nature and nourishing Yin; they share similar goals and actions, so their efficacy can be expected immediately. Shen Qu (Medicated Leaven) eliminates the old and generates the new, communicates upward with Heart Shen and downward with Kidney Zhi, thereby generating Yi and Zhi. Moreover, food enters to nourish Yin and generates Yang; removing the source of food resolves the condition—this is the method for treating mania in the *Inner Canon*. When food is digested, Yi and Zhi become clear and Spirit is regulated; this is the purpose of using Shen Qu. Forming pills with refined honey further employs the sweet flavor to moderate urgency.
