Liver disease corresponds to the Wind Wheel (the cornea and sclera). If the eye color is yellow with red, it indicates invasion by wind-heat; if the eye color is yellow with black, it indicates cold in the Liver and Lungs. Heart disease corresponds to the Blood Wheel (the eyelids and conjunctiva). If the eye color is red with a sense of urgency, or if heat-toxin affects the heart causing redness and pain, this reflects the Heart's association with the summer Qi (fire). Spleen disease corresponds to the Flesh Wheel (the eyelids). If the eyelids are yellow and swollen, it indicates Spleen deficiency; if they are red and itchy, it indicates accumulation of heat in the Spleen Qi. Kidney disease corresponds to the Water Wheel (the pupil and iris). If the eye appears yellow and bright, it indicates the Kidney has been invaded by wind-heat pathogenic factors; if the vision is dim and turbid, it indicates failure and deficiency of Kidney Qi. Lung disease corresponds to the Qi Wheel (the white of the eye/sclera). If the eye is yellow with red, it is attacked by wind-heat toxin; if the redness is extreme and painful, it indicates Fire-toxin injuring Metal (Lung). If physicians can clearly distinguish these patterns, treatment will be infallible, and medication will not fail.
Liver heat causes *Rou Pan Jing* (pterygium growing onto the cornea); it is appropriate to regulate the Liver and cool the Gallbladder. Gallbladder heat causes formation of corneal opacities and pain; it is appropriate to cool the Gallbladder and dissipate Liver heat. Heart heat manifests as persistent red vessels in the eyes; it is appropriate to cool the Heart. Lung heat causes sores on the eyelids; it is appropriate to cool the Lungs. Spleen heat causes eye pain; it is appropriate to cool the Spleen and Stomach. Kidney heat causes false cataracts with pain and swelling; it is appropriate to harmonize Kidney Qi. Bladder heat causes inverted eyelashes (*Dao Jie Quan Mao*). Intestine heat causes floating opacities obscuring the pupil; it is appropriate to cool the Heart and Lungs.
Liver cold causes tearing and photophobia; it is appropriate to tonify the Liver and Kidney and harmonize Qi and Blood. Gallbladder cold causes dim vision and dryness/roughness in both eyelids; it is appropriate to tonify the Kidney and benefit the Liver. Heart cold with stagnant Qi causes discomfort; it is appropriate to warm the Spleen. Eye pain caused by cold should be treated by warming the Spleen to generate blood. Kidney cold generates "black flowers" (floaters); it is appropriate to tonify the Kidney and generate essence. Bladder cold causes frequent dim vision; Large Intestine cold causes blurred vision; it is appropriate to take warming medicines. Lung cold due to wind-cold pathogens requires tonifying the Heart and Lungs. Spleen cold causes inability to open the eyelids. In all cases requiring medication, one must investigate the root cause to avoid errors.
