又方
Hong Kong Foot (Jiaogongjiao) disease is actually caused by water-dampness pathogenic factors. Dampness harms the skin, flesh, sinews, and vessels of the human body, and the pathology is mostly located in the lower part. However, there are two main causes: one is contracted from the exterior, and the other arises from the interior. Why do some people living in low-lying and damp areas suffer more frequently from this disease? Is it not due to the sensation of underground dampness? The answer is: Diseases in the South are mostly contracted from the exterior; diseases in the North mostly arise from the interior. Why is this so? People in the North often consume dairy products (Tongru), drink alcohol without restraint, and overindulge to the point of injury without satiety. Moreover, dairy products have a taste similar to milk and a nature similar to water. Wine is the same. When humans ingest food and fluids into the stomach, the Stomach Qi steams and vaporizes them. Their Qi and flavors disseminate through the meridians, transforming into Blood and Qi, nourishing the four extremities outwardly, and pouring into the five Zang and six Fu organs inwardly, circulating endlessly to correspond with time cycles. This is called the discipline of Heaven and Earth. This occurs because Yuan Qi (Original Qi) is sufficient, and the Qi of the Spleen and Stomach is undamaged. If Yuan Qi is insufficient, the root of Stomach Qi becomes weak. If food intake doubles, the Qi of the Spleen and Stomach is damaged. Since the ingested food cannot be steamed and transformed, its Qi and flavors cannot be disseminated and circulated. The nature of water is moistening and downward-flowing, leading to disease. Those invading from the exterior manifest only as swelling and pain in the lower calves; those arising from the interior may even cause swelling in the hand joints. The Classics state: "Swelling of the feet and calves is called Water Disease." Pathologies caused by the Taiyin Meridian present as dorsum pedis swelling. This refers only to cases invading from the exterior. The treatment methods provided by predecessors are complete. For those arising from the interior, treatment methods were not established. Yang Dashou said: Hong Kong Foot is a disease of stagnation. Treatment should use agents that promote circulation and unblocking, preventing Qi stagnation. If stagnation has formed and is severe, bi-stone therapy should be used to pierce and expel evil blood to reduce swelling. The Classics state: "Accumulation leads to swelling and heat; piercing removes it." After bi-stone therapy, medicinal treatment follows.
"Medical Invention Discourse" states: The classics mention that groundwater in the South is weak, and fog and dew accumulate there. Regions like Jiangdong and Lingnan are generally like this. During the transition between spring and summer, mountain vapors... (text missing/errata here)... At this time, rest and recuperation should be adjusted, dampness gradually thins out. Cutting toenails and fingernails at Yin and Chou hours slightly invades the flesh to remove dampness. In summer, when pores open, one should not sleep facing the wind... (text missing/errata here)... Spleen Qi cannot distribute evenly, thus flowing downward to invade the Liver and Kidney positions, pouring into the calves. Coupled with unrestrained sexual activity, Yang deficiency and Yin excess occur, forming Hong Kong Foot. Sun Simiao said: Ancients rarely suffered from this disease. Since the southward migration during the Yongjia era, many aristocrats and scholars suffered from it, for this very reason. Palpating the pulse: floating and slow indicates Wind; soggy and weak indicates Dampness; surging and rapid indicates Heat; tight and choppy indicates Cold. Cases caused by Wind heal with sweating; cases caused by Dampness heal with fumigation, along with promoting urination; cases caused by Heat heal with purgation; cases caused by Cold heal with dispersion. Whenever Hong Kong Foot occurs, acupuncture and moxibustion should be used promptly. Using only decoctions for washing is a major taboo. Observing Hong Kong Foot, it arises from Kidney deficiency. However, women also suffer from Hong Kong Foot, necessarily due to emptiness of the Sea of Blood, compounded by retained masses (stasis), anger, worry, grief, and sorrow, thus forming this disease. Given that women suffer frequently, it is known that women contract it due to Blood deficiency, similar to men's Kidney deficiency. The treatment method for women is the same as for men; the medicines are essentially no different, but combined with herbs regulating the Seven Emotions, efficacy is assured. Ancient treatments for Hong Kong Foot lacked tonification, favoring draining and unblocking. However, one must follow the methods of tonifying and draining according to the four seasons. Spring calls for mild draining; Summer calls for mild tonification. Otherwise, Blood and Qi will decline daily, inevitably causing recurrence during hot and humid periods, which is logical. The general treatment method... (text missing/errata here)... or abdominal pain with diarrhea, or constipation and difficulty in urination and defecation, or chest palpitations and distress, aversion to light, mental confusion, delirious speech, high fever and headache, severe cold and pain in the body, muscle twitching, apparent swelling without actual swelling, numbness and paralysis of legs and feet, intermittent flaccidity and inability to move, spasms of all joints, insensitivity of the lower abdomen. These are all manifestations of Hong Kong Foot. Women are the same. Postpartum women who seek coolness in spring and summer often contract this toxin; caution is essential. Symptoms include stifling heat, pulling pain, palpitations, irritability, vomiting, and upward rushing of Qi. Also, cold pain in the lower umbilical region, discomfort... (text missing/errata here)... accompanied by dribbling urination differing from normal, indicates Hong Kong Foot.
Throughout the four seasons, one must not stand or sit for long periods in cold and damp places, nor should one expose oneself to wind after drinking to intoxication and sweating, removing clothes, boots, or socks to cool down, which leads to Hong Kong Foot. Sitting for too long... (text missing/errata here)... To prevent illness, upon initial sensation of disease, moxa should be applied to the affected area, 30-20 moxa cones. This cures it immediately without recurrence. The Yellow Emperor said: Cooling off in the wind and engaging in intercourse after drunkenness can cause this disease. Question: Why do some patients present with cold symptoms and others with heat? Answer: The feet have three Yin and three Yang meridians. Cold invading the Three Yang meridians results in cold pathology; Heat invading the Three Yin meridians results in heat pathology. Thus, there are differences in exterior/interior and cold/heat. Heat is treated with cold medicine; Cold is treated with hot medicine, adjusting according to judgment. Spleen affected by Yang toxin becomes hot and stiff; Kidney affected by Yin dampness becomes cold and bi-syndrome. Knowing this, even if suffering from Hong Kong Foot, if mineral drugs (Rushi) trigger reactions, one must take suppressing minerals medication. For complications arising from Hong Kong Foot, combine various medicines. If urination is difficult, use Polyporus and Poria plus diuretics. If defecation is difficult, use Wurou Mahuang Wan etc. For generalized edema becoming Water Disease, use water-draining formulas from water-treatment sections. Others follow this pattern, with no rigid taboos. Upon slight abnormality after illness, rapidly administer potent remedies. Delay allows Qi to rush into the abdomen, causing chest and hypochondriac fullness and rebellion, Qi rushing to shoulders and breathing difficulty. Acute cases die instantly; mild cases die within days. Urgent treatment is required. Look for urgency below the heart, continuous panting, spontaneous sweating, alternating chills and fever, pulse that is hurried, short, and rapid, and incessant vomiting—these indicate death.
"Qianjin Lun" states: People suffering from Hong Kong Foot often do not recognize it initially. Later, triggered by other illnesses, if treated successfully, some experience vomiting followed by leg weakness... (text missing/errata here)... Although Hong Kong Foot... (text missing/errata here)... diagnostics vary, the pulses of the three positions must not contradict the four seasons to be auspicious. Contradicting the four seasons means do not treat. Dark and thin individuals... (text missing/errata here)... Hong Kong Foot presentations are numerous, but differentiate Yin and Yang via pulse diagnosis to avoid error. Floating pulse indicates Wind; Tight indicates Cold; Slow and fine indicates Dampness; Surging and rapid indicates Heat; Dampness excess... (text missing/errata here)...
