[Appendix on Heatstroke]
Coptis Rhizome (1 jin, remove rootlets and chop) Wine (2.5 sheng, soak Coptis slightly then boil with fire until wine is dry). Grind the above into fine powder. Make pills the size of Sophora seeds using wheat paste. Take 30 pills each time, swallowed with warm water. For excessive alcohol consumption causing internal toxicity and bloody stool, or diarrhea, swallow with warm rice drink. Take two doses before meals. During midsummer, if one is struck by heat energy on the road and faints losing consciousness, immediately help them to a cool and shady place. Do not give cold water. Use cloth towels, clothing, etc., and apply hot water compresses to the area below the navel, as well as the Dantian and Qihai acupoints. Continuously pour hot water over the cloth applied there, allowing the heat to penetrate the navel and abdomen. When the body becomes gently warm, they will gradually regain consciousness. If it is a merchant or hired laborer who, in haste, has no hot water, scoop up hot earth from the road and place it on the navel, still making a hole in the middle, and have someone urinate into it. Then crush garlic, mix with water, and administer orally. Alternatively, use an equal amount of road hot earth and garlic, crush thoroughly, mix with water, and take. Also place a small amount of garlic in the nose; once the scent penetrates, they will revive. Whenever one feels heatstroke, urgently chew ginger and swallow a large bowl of cold water. When heat energy attacks the person, strictly avoid using only cold water for drenching. Laifu Dan (Reviving Pill) and Xiaoshu Wan (Summer-Heat Resolving Pills) can all be used. The syndrome of heatstroke presents with a dirty complexion, deep and hidden pulse in all six positions, spontaneous cold sweat, coma and loss of consciousness. First, apply hot towel compresses to the navel and abdomen as described above. Next, grind Laifu Dan into powder, mix with cold boiled water, and administer. Then administer Baihu Tang (White Tiger Decoction) and Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction). This is a fixed method that must not be changed. Often, patients' families lack judgment; relatives and friends ask about the illness, each proposing their own theory and transmitting their own prescription, all claiming frequent efficacy. With many visitors, discussions are chaotic, and one does not know who is right. Hesitation leads to severe deterioration. It is better to refer to external symptoms and take medication with conviction, without believing rumors to cause regret later.
Shui Piao Wan (Water Ladle Pill) (from *Ren Cun Fang*), treats exposure to summer-heat toxin, relieving irritability and thirst.
Mume Fruit Flesh (4 liang) Licorice Root Green Salt (each 2 liang) Dried Quince Sandalwood White Poria (each 1 liang) Musk (2.5 qian, processed with refined honey, adjust dosage according to other herbs). Except for Musk which is ground separately, grind the rest into fine powder. Form pills with refined honey, making 30 pills per liang of powder. Take one pill each time, dissolve in the mouth. Or chew with fresh drawn warm water. No specific time limit. Another formula includes Ginseng.
Shui Hu Lu Wan (Water Gourd Pill) (from *Bai Yi Xuan Fang*)
Chuan Bai Yao Jian (3 liang) Ginseng (3 qian) Licorice Root Ophiopogon Tuber Mume Fruit Flesh White Plum Flesh Dried Kudzu Root (each half liang). Grind the above into fine powder. Form pills the size of cocklebur seeds using wheat paste. Take one pill each time, dissolve in the mouth. In summer months, taking one pill can last a day.
Treats summer-heat injury causing tidal fever, irritability, thirst, and difficult urination.
Use Tongling San (Passive Herb Powder) for the above symptoms. Take 3 qian each time. Use 1 zhen of water, 20 stems of Juncus Effusus, 20 grains of Ophiopogon Tuber (core removed), 10 slices of Light Bamboo Leaf, and 5 stems of Plantago Spike. Decoct together. Take at any time. For severe thirst, add a small amount of Late Grain Root, 5 stems of Imperata Rhizome, and 2 slices of Loquat Leaf (remove white hairs, coat with honey, and lightly roast before decocting).
Gui Ling Wan (Cinnamon and Polyporus Pill) (from *Ren Cun Fang*), treats summer-heat irritability and thirst, eliminates phlegm-fluid retention, and expands the chest and diaphragm.
Cinnamon Heart Atractylodes Macrocephala Rhizome (each 2 liang) Red Polyporus (2 liang) Mume Fruit Flesh (2.5 liang) Dried Fresh Ginger (1 liang) Licorice Root (half liang). Grind the above into powder. Form pills the size of bullet seeds using honey. Take one pill each time, chew finely, and swallow with warm water. Can also take up to two pills.
Wuling San (Five-Ingredient Powder with Poria) (from *Xuan Ming Lun*), treats summer-heat injury causing irritability and thirst, excessive drinking. Also treats Cold Damage and Warm-Thermal diseases where exterior and interior are not resolved, leading to irritability, thirst, and desire to drink; vomiting immediately after drinking water; or difficult urination; and persistent irritability and thirst after sweating and resolution of the exterior. Also treats Huoluan (colic with vomiting and diarrhea), and plague/miasma malaria causing irritability and thirst, red and scanty urine.
Alisma Rhizome (2.5 liang) Cinnamon Heart (1 liang) Polyporus (remove skin) Red Polyporus (remove skin) Atractylodes Macrocephala Rhizome (remove rhizome head, each 1.5 liang). Grind the above into powder. Take 2 qian each time, mixed with boiling water. No specific time limit. After taking, drink plenty of hot water; recovery occurs with sweating. For Warm-Thermal disease, add 1 liang of roasted Licorice Root. For stasis-heat in the interior causing yellowing of the body and jaundice, decoct Yinchen Tang (Artemisia Capillaris Decoction) strongly and take before meals. For jaundice onset with thirst and summer-heat induced drinking, mix with plain water. For children, add Atractylodes powder or a little sugar. To disperse deficiency-heat and resolve chills/fever, take 2 roots each of Artemisia Scoparia (Shan Yin Chen) and Plantago Leaf. To stop diarrhea, add Perilla Leaf and aged rice in small amounts, and half a spoonful of ginger juice. For chronic dysentery, add Myristica Seed (5 qian); take with one salted plum per dose. For red dysentery, take one Mume fruit. For white dysentery, take two Chestnut Shells (remove stem and red membrane, stir-fry with vinegar); prepare similarly, add a pinch of millet. For abdominal pain, take Nan Mu Xiang (half qian). For thirst, take one Mume fruit, two Loquat Leaves (remove white hairs), and a pinch of glutinous rice. For latent summer-heat epistaxis, take a handful of Imperata Flower. For hematuria, use raw ingredients; add three Gardenia fruits and a pinch of Plantago Seed per dose. For irritability and mental confusion, take one zi of Cinnabar, 20 stems of Juncus Effusus, 20 grains of Ophiopogon Tuber (core removed), 10 slices of Light Bamboo Leaf, and 2 roots of Plantago Leaf. For summer-heat injury causing hematemesis, take a handful of Imperata Flower. For heat-induced diarrhea, take a pinch of Plantago Seed and 20 grains of Ophiopogon Tuber (core removed). For summer-dampness diarrhea, take 5 Perilla Leaves, 3 slices of Quince, and a pinch of Plantago Seed. For summer-heat injury causing headache, decoct Scallin Stem strongly. For watery stool and difficult urination, add a small amount of true Plantago Seed powder. For loss of consciousness, first take Suhexiang Wan (Styrax Pill); wait for recovery, then add Coptis Rhizome and Notopterygium Rhizome (each 1 qian) to this formula. For summer-dampness swelling and fullness, combine with Xiang Su San (Perilla Powder), add 3 slices of Fresh Ginger, 2 slices of Quince, and 2 roots of Plantago Leaf. For latent summer-heat headache and turbid, scanty urination, add 2 roots each of Artemisia Scoparia and Plantago Leaf. For Huoluan vomiting and diarrhea, add 2 slices of Quince, a little Huoxiang (Agastache), and 3 slices of Fresh Ginger. For summer-heat injury causing abdominal pain, add 3 cun of aged Polygonum Aviculare, a piece of aged wall earth the size of a finger, and 2 slices of Quince. For internal organ heat causing bloody stool, add Coptis Rhizome, Magnolia Bark, Aurantium Fruit, and Mume Fruit (each 1 qian). For hematuria, add 1 qian of Dianthus Spike, a pinch of Plantago Seed, and 20 stems of Juncus Effusus, adding them all. For summer-dampness invasion causing painful bi-cold in hands and feet, combine with Xiang Su San. Take 4 qian each time, with 3 slices of ginger, 3 slices of quince, and 3 cun of aged Polygonum Aviculare, decoct and take. Reserve the dregs, add a handful each of Scallion Leaf, Orange Leaf, Pepper Leaf, and aged Polygonum Aviculare. Add 1 dou of water, boil to 7 fen. Fumigate first, then wash; immediate effect. For Hong Kong foot (podagra) pain and difficult walking, add to each dose: Quince, stir-fried Notopterygium Rhizome, Atractylodes Lancea Rhizome, Aurantium Fruit (pith removed), Tangerine Peel, Pinellia Tuber, and Licorice Root (each 1 liang), totaling 12 flavors. Each dose add 3 slices of Fresh Ginger and 2 Scallin Stems. For spleen-stomach disharmony, vomiting, nausea, summer-heat invasion causing heart and abdominal fullness, remove Notopterygium Rhizome, add Agastache and Mume Fruit (each 1 qian). For high fever and extreme thirst, heat in the belly or five hearts, add 20 stems of Juncus Effusus, Ophiopogon Tuber (core removed), Imperata Rhizome (each 1 qian), 10 slices of Light Bamboo Leaf, and a washed handful of Late Grain Root. For four-season common cold with vomiting and diarrhea, also combine with Xiang Su San. Take 4 qian each time, add 5 slices of Fresh Ginger and 2 slices of Quince. For various heat toxins and red/turbid urine, take 4 qian each time, add Plantago Seed and Coptis Rhizome (remove rootlets, each 1 qian) to clear and promote urination. All the above are taken warm. For severe irritability and tidal fever in summer months, add 2 stems each of Artemisia Capillaris and Plantago Leaf, 7 slices of bitter Bamboo Leaf, and 3 Gardenia fruits (broken open). Another formula for treating dry thirst removes Cinnamon Heart and adds Ginseng, called Chunze Tang (Spring Spring Powder). For dampness-diarrhea with body pain, add a little Atractylodes Lancea Rhizome. In autumn with excessive rain, when people suffer from plague, taking this in winter is effective, especially wonderful. One formula adds Cinnabar. For stasis-heat and mental irritability, Doctor Dai Zhai Chongfu said that due to his mother's illness, his filial piety moved the North Star, who granted this medicine in a dream. However, Wuling San uses Cinnamon, Xiao Chaihu Tang uses Ginseng, Zhengqi uses Magnolia Bark, Beijii Wan uses Dried Fresh Ginger, etc., intending to balance hardness and softness, representing attack and defense strategies. Therefore, formularies state that Wuling San without Cinnamon, or stored for a year, should not be used. Recently, pharmacies sell Wuling San without Cinnamon; those unaware are misled by it. If Cinnamon is removed and Ginseng added, it is called Chunze Tang. Peng Family Secret Formula states that Wuling San can be made into pills using Fresh Ginger Juice, the size of cocklebur seeds, and taken with Wuling San decoction; it can also be used on journeys.
Weiling Tang (also known as Weiling San, from *Ren Cun Fang*), treats summer-heat injury causing irritability, thirst, and excessive drinking, with watery stools.
Wuling San Pingwei San Combine the above two medicines. Take 2 qian each time, swallow with decoction of Perilla Leaf and Mume Fruit. If ineffective, add Costus Root, Amomum Fruit, Atractylodes Macrocephala Rhizome, and Clove, decoct and take. Laifu Dan can also be used.
Lengxiang Yinzi (Cold Fragrance Drink) (from *Ji Sheng Fang*), treats elderly and deficient persons with latent summer-heat, irritability, excessive drinking, nausea, fatigue, who cannot tolerate cooling medicines. This formula is indicated. Suitable for regular use in summer.
Amomum Fruit Kernel (3 liang) Aconite Root (processed, remove skin and umbilicus) Tangerine Peel (each 1 liang) Licorice Root (roasted, half liang). Chop coarsely. Take 1 liang each time.
Shuipo Huosan (Water-Splashing Fire-Scattering Powder) (also known as Diyu San, from *Bai Yi Xuan Fang*), treats summer-heat injury causing irritability and thirst, bitter taste, dry tongue, headache, nausea, loss of appetite, coma and loss of consciousness approaching death. Also treats bloody dysentery and women's heat-induced metrorrhagia.
Citrus Green Peel (remove white pith) Red Peony Root Coptis Rhizome (remove rootlets) Sanguisorba Root (equal parts). Grind into fine powder. Take 3 qian each time, mixed with cold water. If heat accumulation causes reckless blood movement, add Licorice Root in equal parts. Another formula uses Jiangshui (fermented grain water) to mix. For bloody dysentery, decoct with 1 zhen of water and take.
Huanglong Wan (Yellow Dragon Pill) (from *Bai Yi Xuan Fang*), treats exposure to summer-heat toxin, relieves irritability and thirst, headache, and vomiting.
Pinellia Tuber (8 liang, soak in 1 dou of strong vinegar for 3 days, simmer slowly in silver vessel until vinegar is exhausted, take out, wash with fresh drawn water, dry completely).
