Dampness is the rainy clouds and lingering fog of the sky, and the steaming vapor from mountains and marshes on the ground. If a person is invaded by dampness, it will inevitably overflow into the blood vessels and flow into the joints. Those who dwell for long periods in low-lying and damp places, reside frequently near water channels, walk in rain and dew, or suffer fatigue with sweating where clothes become cold and wet inside, can all develop diseases. The classics state: "Dampness tends to stagnate and adhere." *Zuo Zhuan* says: "Excessive dampness causes abdominal disorders." Therefore, those affected by dampness experience whole-body pain and heaviness, soreness and aching in all joints, swelling and fullness in the four limbs, laxity and elongation of tendons and vessels, fever with aversion to cold, constipation of urine, and diarrhea with undigested food (Sun Xie). If Qi mechanism is not harmonious and dampness is slightly severe, it may also lead to hemiplegia, deviation of the mouth and eye, flooding of saliva, and stupor with obstruction, resembling wind patterns. The treatment method should be to promote urination. Treating dampness without promoting urination is incorrect. One should seek methods in formulas such as Shen Shi Tang (Damp-Draining Decoction). One must strictly avoid using fire to force sweating and reckless purgation. Mistakenly inducing sweating in damp patients will inevitably cause spasms (Jing), leading to opisthotonos resembling stroke. Mistaken purgation leads to sweating on the forehead, slight panting, and inability to urinate, which is a death sign; if diarrhea persists, it is also fatal. Cold, heat, wind, and dampness can combine to cause disease, hence there are Wind-Damp and Damp-Warm patterns, each with different manifestations and treatments, which must be clearly distinguished.
Wind-Damp refers to the struggle between wind pathogen and damp pathogen. Symptoms include heavy and uncomfortable limbs, inability to turn over, pulling pain in tendons and vessels, inability to flex and extend, slight sweating on the forehead, reluctance to remove blankets, or slight body swelling. To induce sweating, if the body becomes slightly moist, both wind and damp will be expelled. If excessive sweating is induced, wind may leave but damp will merge with it. Symptoms include painful body, no sweat, aversion to cold and fever, floating, moderate, and fine pulse. Mahuang Baizhu Tang (Ephedra and Atractylodes Macrocephala Decoction) is suitable to release the exterior, dispel dampness, and remove cold.
Zhong Shi (Central Dampness) presents with a deep, fine, and slow pulse because dampness overflows into the skin and muscles, making the pulse deep and fine. Dampness, in the sky, is rain; on earth, it is soil; in humans, it corresponds to the Spleen organ. Thus, dampness tends to affect the Spleen, and Spleen deficiency makes one susceptible to dampness. Hence, dampness flows into joints; invading the body causes distension, pain and irritability in limbs and joints, and over time edema, panting, fullness, and coma. If dampness settles in the Kidney, there is lower back pain and body heaviness as if sitting in water; if it settles in the Spleen, limbs swell and cannot move; if combined with wind, there is vertigo, vomiting, and hiccups; if combined with cold, there is cramping and pulling pain, or diarrhea with yellowing, fever, nasal congestion, staring gaze, confused speech, spontaneous sweating, beriberi, abdominal distension, and hardness below the umbilicus.
Wind-Damp is when wind and damp pathogens jointly injure humans. Wind is the empty evil from eight directions; Wind-Damp is the vapor of water dampness. If underground dampness is heavy and frost/snow is scarce, mountain stream vapors rise. Combined with warm and muggy weather, pores open, making one susceptible to Wind-Damp. This causes lethargy, mental confusion, and if chronic, flaccidity of limbs. If entering organs, it causes inability to control mouth/tongue, foot numbness, becoming beriberi. *Zhengao* states: Combing hair frequently unblocks blood vessels and disperses wind-damp; changing combs often is advisable.
Damp-Warm occurs when a person is previously injured by dampness, then suffers summerheat, or bathes before sweat dries. Heat and dampness struggle, causing Damp-Warm. Patients suffer from reverse coldness in calves, excessive chest sweating, head/eye pain, delirium, aversion to cold with fever, joint pain, and weak hands/feet unable to support weight. Pulse is superficial and soggy/weak, deep and small/rapid. The disease location is Taiyin. Lingjun Baizhu Tang (Poria and Atractylodes Macrocephala Decoction) is suitable. Sweating must not be induced. If sweated, the patient loses voice, becomes deaf and disoriented, body turns blue/purple, named "Chong E" (Heavy Miasma). Such cases result in medical error.
For men, women, and children with numbness/paralysis of limbs and insensibility of muscles, this is Bi syndrome caused by mixing of wind, cold, and damp. First use Huangqin Shaoyao Tang (Scutellaria and Peony Decoction) to induce vomiting. After vomiting, use Tongjie Wan (Unblocking Pill) to purge. After purging, use acrid-sweet agents to induce sweating. After sweating and purging, use Danggui Qingliang Yinzi (Angelica Cooling Drink), Wu Jing Wan, Chushi Dan, and blood-nourishing/moving herbs to cure.
For men, women with persistent watery diarrhea due to consuming cardamom, smoked plum, ginger, aconite, etc., causing internal Yang depletion and Yin sinking. *Neijing* states: "All dampness-related swelling and fullness belong to Spleen Earth." Use Dusheng San (Single Miracle Powder) to vomit. In cold seasons, use a warm room with fire to induce sweating. Then use Daoshui and Yugong formulas to purge 10+ times based on deficiency/excess. When damp leaves and swelling reduces, cure is achieved. This uses sweating, purging, and vomiting together. After these three methods, Zang-Fu are empty; nourish stomach/intestines with thin porridge for 2-3 days. Then take Wuling San (Five-Ling Powder) and Yiyuan San decocted together, or taken with Juncus effusus tea. If major symptoms persist, take Shengong San to moisten dryness, separate Yin/Yang, and benefit water passages. After recovery, rest carefully for 3 years, avoiding oil, salt, alcohol, fruit, and sexual activity to prevent recurrence.
