On the second day of Shanghan (Cold Damage), the pathogen affects the Yangming channel. The Yangming channel is the meridian of the Stomach, primarily governing the muscles. Its collaterals connect to the nose and enter the eyes. Therefore, this occurs after two days of illness.
If there is no sweating and aversion to cold persists, Shengma Tang (Costus Root Decoction) is appropriate. If there is sweating accompanied by aversion to cold, it indicates that the exterior syndrome has not yet resolved, and Guizhi Tang (Cinnamon Twig Decoction) is appropriate. If there is no sweating, the pulse is floating and large (excessive outward projection), and the patient experiences wheezing, administer Mahuang Tang (Ephedra Decoction).
Another question: The twelve meridians usually each have one pathway, yet the Yangming is divided into three types. Why? This is because there are combinations of Taiyang and Yangming, Shaoyang and Yangming, and Zhengyang (Direct) Yangming.
The external symptoms must include headache, lower back pain, muscle heat, eye pain, and nasal dryness. A floating and large pulse indicates Taiyang disease. A long pulse indicates Yangming disease. Headache and lower back pain indicate Taiyang disease. Muscle heat, eye pain, and nasal dryness indicate Yangming disease. If there is still aversion to cold, Shengma Tang can be used to induce sweating for treatment. If there is no aversion to cold but rather aversion to heat, and constipation...
Another question: What is the difference between combined disease (Hebing) and concurrent disease (Bingbing) involving two Yang channels? A floating and large pulse, accompanied by headache, lower back pain, muscle heat, eye pain, and nasal dryness, is called Hebing (Combined Disease).

Treat unresolved Shanghan in the initial two to three days, characterized by headache and high fever.
Treat Shanghan on day one or two, characterized by high fever, headache, widespread body pain, tendon and vessel tension, nasal congestion, dry throat, loss of voice, phlegm-fullness in the chest, excessive clear nasal discharge. Sudden exposure to rain and wind, or catching cold while walking at night, causes aversion to cold and fever, compounded by headache. Or due to cold seasonal Qi, catching cold leads to cough. Or due to injury by cold Qi, causing dizziness of head and eyes, pain in neck and legs, tension in all joints, mental discomfort, restless headache, heavy body. Or suddenly catching cold causes bone-chilling coldness in shoulders and back. For any case of Shanghan presenting with these symptoms, this medicine is suitable. Generally, Shanghan is defined as having aversion to cold and high fever within the first one or two days of onset. If the disease begins without aversion to cold, but only with splitting headache, burning body heat, flushed face and head, red and exposed eyes, constipation with heat, and yellow-red urine, this is not called Shanghan, but Rebing (Heat Disease), and this medicine should not be taken; other formulas apply.
