The condition of "Fa Bei" (Back Carbuncle) arises when cold pathogenic factors invade the meridians, obstructing the flow of Qi and Blood. When blood becomes stagnant and fails to circulate smoothly, a Fa Bei forms. After the pus is discharged, if the whites of the eyes turn cyan-black and the pupils shrink, this constitutes the first adverse sign (Ni); if vomiting occurs after taking medicine, this is the second adverse sign; if pain is severe accompanied by extreme thirst, this is the third adverse sign; if there is immobility in the shoulders and neck, this is the fourth adverse sign; if the voice is hoarse and the complexion is sallow and devoid of luster, this is the fifth adverse sign. All five conditions are considered incurable critical states. Other symptoms such as fever, thirst, diarrhea, and vomiting are not urgent or difficult to treat and can be managed gradually. Generally, heat pathogens enter the Fu organs in cases of Fa Bei. After the abscess ruptures, Qi and Blood become deficient. Accumulated heat in the Fu organs causes thirst and desire for drinking. If cold water enters the intestines and stomach, it transforms into diarrhea. Deficient Stomach Qi with rebellious upward movement leads to vomiting. If vomiting or hiccups are suppressed by cold-natured medicines, the obstruction of Qi mechanism results in persistent hiccups. If the pus within the wound is not fully drained and the wound closes prematurely, although it may appear healed initially, it will recur later. Continuous discharge of foul-smelling pus will transform into a fistula.
