When a "Fa Bei" (Back Carbuncle) first begins, the symptoms may resemble a small boil, or it may present with severe pain, or mild pain. Initially, the head of the lesion is as large as a hemp seed; these are minor signs that should be observed promptly.
If there is any slight change in condition, one must be highly vigilant and treat it immediately. Dietary restrictions should be observed, and various medications should be taken continuously to clear heat and toxins from the body. If medical treatment is unavailable in remote areas, direct moxibustion of 100 "zhuang" (moxa cones) should be applied to the affected area. If significant swelling has already occurred, apply 200-300 "zhuang" around the periphery and on top; the more, the better. Additionally, apply cold medicinal plasters and employ various rescue treatments to ensure recovery.
When using topical plasters, a hole must be cut in the center to allow the discharge of toxic vapors. "Fa Bei" arises from the disharmony of the six fu organs, obstruction and stagnation of the meridians and collaterals, blood congestion and blockage, and the accumulation of heat toxins, which condense to form an abscess. The swelling appears in the middle of the back between the shoulder blades, initially white like a millet grain, spreading and connecting around the periphery, becoming red, hot, and painful. It may be ulcerated or non-ulcerated; this refers to the "Tuo" (pressing/washing) method.
Formulas:
Dahuang San (Rhubarb Powder): Treats early-stage Fa Bei resembling a hemp seed, gradually hardening and becoming painful.
Ingredients: Chuan Dahuang (Radix et Rhizoma Rhei), Muxiang (Radix Aucklandiae), Xuanshen (Radix Scrophulariae), Huangqin (Radix Scutellariae), Chi Shaoyao (Radix Paeoniae Rubra), Bailian (Rhizoma Belamcandae), Zige (Cortex Cinnamomi Cassiae var. Cognatae? Note: Text says Zige, likely specific bark), Chixiaodou (Semen Phaseoli Angularis) (each 1 liang).
Preparation and Administration: Grind the above ingredients into a fine powder. Mix with egg white (Jizi Bai) to form a paste resembling batter. Apply onto silk fabric and plaster over the lesion. Reapply when dry until healed.
Xionghuang San (Realgar Powder): Treats Fa Bei swelling and toxicity, redness and pain.
Ingredients: Xionghuang (Realgar) (1 liang, finely ground), Huanglian (Rhizoma Coptidis) (1 liang, remove root heads), Huangbai (Cortex Phellodendri) (0.5 liang, sliced), Chixiaodou (Semen Phaseoli Angularis) (3 fen), Chuan Pu Xiao (Natrii Sulfas) (1 liang), Huangqin (Radix Scutellariae) (0.5 liang), Baiji (Bulbus Bletillae) (3 fen).
Preparation and Administration: Grind into a fine powder. Mix with pig gallbladder bile (Zhudan) to form a paste. Apply to the swollen area 3-4 times daily.
Liu Mu'er Bing (Willow Wood Ear Cake): Treats Fa Bei caused by mineral stone drugs, where the sore is red and black, suitable for plasting.
Ingredients: Mu'er growing on old willow trees (2 liang), Huanglian (Rhizoma Coptidis) (1 liang, remove threads), Longgui Gen (Radix Solani Lycopersici) (one handful, washed clean, soil removed, sliced), Ruxiang (Resina Draconis) (1 liang), Ren Fen Hui (Ash of Human Feces) (0.5 liang), Xingren (Semen Armeniacae Amarum) (1 liang, soaked in hot water, skin and tips removed).
Preparation and Administration: Mix ingredients and pound 300-500 times. Shape into cakes, thickness at least 5 qian. Apply according to the size of the sore. Secure with a single silk band to prevent slipping. When the patient feels itching and cold penetrating the heart, do not scratch. Replace every distance equivalent to walking 20 li. Soon, unbearable itching will occur, and the surrounding skin will wrinkle, allowing pus to drain. Immediately remove the cake, wash gently with Licorice Decoction, and apply a healing ointment. Change once daily, always washing with Licorice Decoction until healed. Itching indicates easier recovery; lack of redness and itching indicates difficult recovery.
Badu San (Toxin-Pulling Powder): Formula for applying to Fa Bei sores.
Ingredients: Yangqishi (Mineral Pyrite), Hanshui Shi (Mineral Calcitum), Fanshi (Alumen), Baishi Zhi (Mineral Halloysitum), Shigao (Mineral Gypsum Fibrosum), Maifan Shi (Mineral Calcariae) (each 2 liang).
Preparation and Administration: Grind into powder and sieve until fine as flour. Mix with fresh well water and apply to the sore.
Jiedu Shengji San (Detoxification and Muscle-Regeneration Powder):
Ingredients: Duanshi (Calx Ferrum) (1 fen, preferably from old ship planks, burned red in a clean vessel), Huangdan (Mineral Plumbi Subcarbonas) (1 fen, fried until purple), Longgu (Os Costae Mastodi) (1 fen), Shexiang (Moschus) (1 fen), Shenzi (Fructus Mori) (1 fen), Mituoseng (Mineral Lithargyri) (1 fen).

Modaoshi Xiaozhong Fang (Whetstone Swelling-Reduction Formula):
Ingredients: Modaoshi Chishi (Red Stone from Whetstone) (one piece, 3 cun wide, 3 cun long, 1 cun thick), Bailian (Rhizoma Belamcandae) (2 liang), Chuan Dahuang (Radix et Rhizoma Rhei) (2 liang), Cu (Vinegar) (1 sheng), Lu Jiao (Cornu Cervi Pantotrichum) (8 liang, burned to ash).
Preparation and Administration: Heat the whetstone until red, quench in vinegar, reheat, and quench again. When vinegar reduces by half, combine all herbs, pound into fine powder, and mix with vinegar to form a paste. Apply on soft silk. Replace when dry.
Gualou Gen San (Trichosanthes Root Powder): Treats Fa Bei and various toxic swellings, suitable for topical application.
Ingredients: Gualou Gen (Radix Trichosanthis) (3 liang), Yu Baipi (Cortex Ulmi) (3 liang, sliced), Hu Yan Ke Tu (Soil from Wild Swallow's Nest) (5 liang), Shu Tu (Soil from Rat Hole) (5 liang).
Preparation and Administration: Grind into fine powder. Mix with juice extracted from Banana Root (Bajiao Gen) to form a thin paste. Apply to swelling; reapply when dry. If ulcerated with a head, apply around the periphery. Alternative method: Use menstrual cloth of a woman, wash in water, take the juice, mix into mud-like consistency, and apply.
Duanshi San (Burnt Iron Powder): Treats all swellings, including Fa Bei and breast abscesses.
Ingredients: Fenghua Duanshi (Weathered Burnt Iron) (1 he), Xiaomai Mian (Wheat Flour) (3 he), Zaogao Hui (Ash of Gleditsia Sinensis) (1 he), Bailian (Rhizoma Belamcandae) (1 he).
Preparation and Administration: Grind into fine powder. Mix with strong fermented bean curd juice (Yan Jiangshui) to form a batter-like paste. Apply and change 3-4 times daily.
Another Formula: Treats early Fa Bei toxic swelling.
Ingredients: Meishizi (Galla Chinensis) (one piece), Qiangshang Xiugu (Decayed Bone on Wall) (1 fen), Yanzhi (Vermilion/Cochineal) (1 fen).
Preparation and Administration: Mix and grind uniformly. Apply appropriately. If no pus head has formed, use only Floating Duckweed (Fuping Cao) to apply, which will resolve spontaneously.
Disi Xiongdan Wan (Bear Gallbladder Pill): Treats Fa Bei sores, redness, swelling, heat, and pain.
Ingredients: Xiongdan (Bile Ursi), Shexiang (Moschus) (equal parts).
Preparation and Administration: Grind together and form pills the size of millet grains. Before application, wash the sore with warm water. Place one pill inside the sore, sprinkle Jiedu Shengji San on top, cover with vinegar-batter spread on old cloth, and press firmly. Immediate relief if pressure causes unbearable pain.
Qingliang Gao (Cooling Ointment): Treats Fa Bei redness, swelling, heat, pain, and detoxifies swelling.
Ingredients: Nuomi (Oryza Sativa Glutinosa) (2 sheng), Longnao (Camphora) (1 fen).

Preparation and Administration: Mix with water to form a paste. Apply to swelling. Replace when dry. Continue until healed.
Treats early Fa Bei, after several days of swelling and heat, with fierce toxic qi causing day-and-night headache.
Ingredients: Jizi Qing (Egg White) (3 eggs), Xin Gou Fen (Fresh Dog Feces) (one handful).
Preparation and Administration: Mix to desired consistency, shape into cakes, thickness 1-2 qian. Cut a small hole in the center, apply to swelling, secure with cloth. Apply three times daily until healed.
Treats sensation resembling Fa Bei; applicable to any heat-swelling to resolve internally.
Ingredients: Zaojia (Fructus Gleditsiae Sinensis) (one stick, remove black skin and seeds), Lizi (Fructus Castaneae) (10 pieces, peeled and dried).
Preparation and Administration: Grind into fine powder. Mix with fresh well water to form batter. Spread on cloth and apply to swelling. Replace when dry.

Take cow dung from fields during rainy season, preferably white-tinted. Remove soil, dry in sun. Half burn with charcoal until smoke ceases, half burn until black and crumbly. Mix and grind extremely fine. Mix with raw Sesame Oil (Sheng Mayou) to form a face cream consistency. Apply 3-4 times daily.
Formula for Fa Bei and non-ulcerated carbuncles:
Use Langdang Ye (Folium Daturae Metelis). As soon as a sore is suspected, take leaves in any quantity, heat them, and apply to the sore. Replace when cool. Stop when pain ceases. If leaves are unavailable in a given month, use roots sliced and heated for application.
Formula for Fa Bei with toxic swelling:
Pound Disong (likely a local herb name, possibly Pinus massoniana related) and squeeze out one small cup of juice. Mix with 1 qian of Ruxiang (Resina Draconis) powder, dilute with water, and apply. Reapply when dry. If no head has formed, apply around the periphery. Another version omits Ruxiang.
