When a pregnant woman suffers from vexation and oppression, during the fourth month of pregnancy, she receives the Qi of Shaoyin Junhuo (Ministerial Fire of the Hand-Shaoyin Heart Meridian) to nourish the fetus's essence. During the sixth month, she receives the Qi of Shaoyang Xianghuo (Ministerial Fire of the Foot-Shaoyang Gallbladder Meridian) to nourish the fetus's Qi. If the mother experiences heart palpitations and gallbladder fear,
vexation and oppression often occur. This is named "Zifan" (Fetal Vexation). *Chanbao* states: When Zifan occurs during pregnancy, if the Lung organ is deficient and heat invades the Heart, it causes mental vexation. If the vexation is caused by heat alone, there is merely a sensation of heat. If the vexation is caused by phlegm-fluid retention, there is vomiting of saliva and froth; upon smelling food odor, one becomes irritable and uneasy. Generally speaking, pregnant women have retained phlegm and accumulated fluids, which combine with deficiency-heat. The Qi stagnates and fails to flow smoothly, resulting in either irritability or vomiting of saliva and froth. In severe cases, fetal movement becomes restless. All these are collectively referred to as "Zifan".
Formulas
Shengma Tang (Cinnamon Twig Decoction, also known as Maidong Tang, from *Shenghui Fang*). Treats congestion-heat during pregnancy, mental restlessness, dry mouth, and vomiting/nausea. This condition is largely similar to minorly different from Zifan. Zhimu Wan can also be used universally.
Chuan Shengma (Rhizoma Cimicifugae), Huangqin (Radix Scutellariae), Renshen (Radix Ginseng), Maidong (Radix Ophiopogonis), Zhizi Ren (Semen Gardeniae), Chaihu (Radix Bupleuri), Fushen (Poria with Hostwood), Gualou Gen (Radix Trichosanthis), Xijiao Xie (Hornis Rhinocerotis, sliced), (each 1 liang); Zhi Mu (Rhizoma Anemarrhenae); Gancao (Radix Glycyrrhizae).
Suanzaoren Tang (Sour Jujube Seed Decoction). Treats vexation, depression, and deficiency-related oppression during pregnancy, pain in the four limbs, and insomnia.
Suanzaoren (Fried, 2 liang), Shaoyao (Radix Paeoniae Alba), Fangfeng (Radix Saposhnikoviae, remove forked roots), Chaihu (Radix Bupleuri, remove seedlings), Chi Fuling (Poria, remove black skin), Xijiao (Hornis Rhinocerotis, shaved), Wuweizi (Fructus Schisandrae), Gancao (Radix Glycyrrhizae, honey-fried), Renshen (Radix Ginseng), Binglang (Arecae Semen, crushed, each 1 liang). Pound and sieve the above ingredients. Take 5 qian per dose. Add 1.5 zhan of water, decoct until 1 zhan remains, strain.
Take at any time, warm.
Chi Fuling San (Red Poria Powder, from *Shenghui Fang*). Treats mental vexation during pregnancy, headache and neck pain, loss of appetite, and heat in the hands and feet.

Pound and sieve the above ingredients to make a powder. Take 4 qian per dose. Add 1 zhan of water and 1 fen of Tian Zhuru (Sweet Bamboo Shavings).
Retui Tang (Heat-Removing Decoction). Treats deficiency-vexation and depressive heat during pregnancy. Deficiency-vexation and depressive heat during pregnancy occur because Yang Qi is excessively predominant, and Heat Qi acts alone, causing depressive oppression in the epigastrium, headache, flushed face, yellow and scanty urine, and in severe cases, painful dysuria. *Bingyuan* also refers to this as Zifan.
Renshen (Radix Ginseng), Gancao (Radix Glycyrrhizae, honey-fried), Huangqin (Radix Scutellariae, remove black heart, each 2 liang), Danggui (Radix Angelicae Sinensis, sliced and baked), Shaoyao (Radix Paeoniae Alba), Zhizi Ren (Semen Gardeniae), Fangfeng (Radix Saposhnikoviae, remove forked roots), Chaihu (Radix Bupleuri,

Zhi Mu (Rhizoma Anemarrhenae, sliced and baked), Fangfeng (Radix Saposhnikoviae, remove forked roots), Huangqin (Radix Scutellariae, remove black heart), Gancao (Radix Glycyrrhizae, honey-fried), Maidong (Radix Ophiopogonis, remove heart and bake), Chi Fuling (Poria, remove black skin and crush), Shengma (Rhizoma Cimicifugae, each 1 liang). Pound and sieve the above ingredients. Take 3 qian per dose. Add 1 zhan of water and 3 slices of Shengjiang (Rhizoma Zingiberis Recens). Decoct together until 70% remains.
Add a small amount of Zhuli (Bamboo Sap).
Stir evenly.
Fuling Tang (Poria Decoction). Treats deficiency-vexation and depressive heat during pregnancy, mental oppression and confusion, dizziness with heaviness, vomiting/nausea, and fatigue in the four limbs.
Chi Fuling (Poria, remove black skin; alternatively use Bai Fuling/Poria Alba), Fangfeng (Radix Saposhnikoviae, remove forked roots), Renshen (Radix Ginseng), Baizhu (Rhizoma Atractylodis Macrocephalae, crush and stir-fry), Zhike (Fructus Aurantii, remove pulp and bran-fry), Gancao (Radix Glycyrrhizae, honey-fried, each 1 liang). Pound and sieve the above ingredients. Take 3 qian per dose. Add 1 zhan of water and 3 slices of Shengjiang (Rhizoma Zingiberis Recens). Decoct until 70% remains, strain. Take warm. At any time.

Fangfeng (Radix Saposhnikoviae, remove forked roots), Maidong (Radix Ophiopogonis, remove heart and bake), Huangqin (Radix Scutellariae, remove black heart), Shengma (Rhizoma Cimicifugae), Shigao (Gypsum Fibrosum, crush), Zhizi Ren (Semen Gardeniae, each
1 liang). Pound and sieve the above ingredients. Take 3 qian per dose. Add 1 zhan of water and half he of Zhuli (Bamboo Sap). Decoct until 70% remains, strain. Take warm after meals. Twice daily.
Zhimu Yin (Anemarrhena Drink, from *Dquan Liangfang*). Treats congestion-heat in the Heart and Spleen during pregnancy, dry mouth and thirst, and bitter vexation.
Chi Fuling (Poria), Huangqin (Radix Scutellariae), Huangqi (Radix Astragali, each 3 liang), Zhi Mu (Rhizoma Anemarrhenae), Maidong (Radix Ophiopogonis, remove heart), Gancao (Radix Glycyrrhizae, each 2 liang). Make into coarse particles above.
Decoct Sangbaipi (Cortex Moriperadicis) in water to take, or add half he of Zhuli (Bamboo Sap) to take. At any time. For heat in the fetal organ, this is most suitable. Another formula removes Huangqi (Radix Astragali) and Zhi Mu (Rhizoma Anemarrhenae), adding Digupi (Cortex Lycii), Xijiao Xie (Hornis Rhinocerotis, sliced), and Nenge (Cortex Puerariae, tender, each 3 fen). Crush. Decoct as before.
Xijiao San (Rhinoceros Horn Powder, from *Shenghui Fang*). Treats mental vexation, heat, and oppression during pregnancy.
Xijiao Xie (Hornis Rhinocerotis, sliced), Digupi (Cortex Lycii), Huangqin (Radix Scutellariae), Maidong (Radix Ophiopogonis, remove heart), Chi Fuling (Poria, each 1 liang above); Gancao (Radix Glycyrrhizae, half liang, honey-fried until slightly red, then crushed). Make into a powder above. Take 4 qian per dose. Add 1 zhan of water, decoct until 60% remains, strain. Add half he of Zhuli (Bamboo Sap). Boil for one more boil. At any time. Take warm.
Zhuli Tang (Bamboo Sap Decoction, also known as Zhuye Tang, from *Qianjin Fang*). Treats chronic suffering from vexation and oppression during pregnancy. This is Zifan. Because in the fourth month, the body receives the Qi of Shaoyin Junhuo (Ministerial Fire of the Hand-Shaoyin Heart Meridian) to nourish essence. In the sixth month, it receives the Qi of Shaoyang Xianghuo (Ministerial Fire of the Foot-Shaoyang Gallbladder Meridian) to nourish Qi. If the mother has heart palpitations and gallbladder fear, she often suffers from vexation. It is appropriate to use this formula.
Zhuli (Bamboo Sap, 1 sheng, alternatively Zhuye/Leaf of Bambusa), Maidong (Radix Ophiopogonis), Fangfeng (Radix Saposhnikoviae), Huangqin (Radix Scutellariae, each 3 liang), Fuling (Poria, 4 liang). Add 4 sheng of water above.
Combine with Zhuli (Bamboo Sap) and boil to obtain 2 sheng. Divide into three doses. If not cured, prepare again. Another formula uses Zhi Mu (Rhizoma Anemarrhenae), without Huangqin (Radix Scutellariae). Another formula adds Renshen (Radix Ginseng) half liang.
Maidong Tang (Ophiopogon Decoction, also known as Zhuye Tang, from *Jisheng Fang*). Treats chronic suffering from vexation and oppression during the fourth and sixth months of pregnancy, with heart palpitations and gallbladder fear.
Named Zifan.
Bai Fuling (Poria Alba, peel removed, 1 liang), Renshen (Radix Ginseng), Maidong (Radix Ophiopogonis, remove heart), Fangfeng (Radix Saposhnikoviae, each 1 liang). Crush above. Take 4 qian per dose.
Add 1.5 zhan of water.
Add 5 slices of Jiang (Zingiberis Rhizoma). Add 10 pieces of Dan Zhuye (Leaf of Bambusa Densa). Decoct until 80% remains, strain. Take warm. At any time.
Treats chronic suffering from vexation and oppression during pregnancy. This is named Zifan.
Fuling (Poria, 2 liang), Zhuli (Bamboo Sap, 1 sheng). Add 4 sheng of water above. Combine with Zhuli (Bamboo Sap) and decoct to obtain 2 sheng. Divide into three doses. If not cured, re-preparation is also acceptable. Take Zhuli (Bamboo Sap) frequently.
Treats pregnant women in their seventh or eighth month who suffer from summerheat and heat injury, leading to Zifan. The fetal Qi presses upward, causing obstruction in the throat, abdominal distension and fullness, a feeling of bearing down resembling dysentery. Each time using the toilet, sitting for the duration of a meal, suddenly the Qi descends, allowing only then for a bowel movement. Laypeople usually use dysentery medicines, not knowing the disease lies in fetal heat and Zifan.
Use Xiao Chaihu Tang (Minor Bupleuri Decoction) to administer Huanglian Ejiao Wan (Coptis and Donkey-Hide Gelatin Pill). Or use fried Ejiao (Colla Corii Asini) and purified Huanglian (Rhizoma Coptidis), each 1 fen. Zhike (Fructus Aurantii) dosage is half that of Dahuang (Radix et Rhizoma Rhei). Divide into two doses. Decoct Wumei (Fructus Mume), Jiang (Zingiberis Rhizoma), and Mi (Mel) to take. Wait for the stool to regulate, then continue with Chuanxiong (Rhizoma Chuanxiong), Fuling (Poria), Suosha (Fructus Amomi Rotundus), and Gancao (Radix Glycyrrhizae). If using Wuling San (Five-Poria Powder), Yinggan Wan, Xianglian, or Zhu
Treats chronic suffering from vexation and oppression during pregnancy. This is Zifan. (From *Dquan Liangfang*)
Use Zhuli (Bamboo Sap) in whatever quantity. Take frequently, and it will heal itself.
Treats Zifan during pregnancy, dry mouth, inability to lie down. (From *Dquan Liangfang*)
Remove the whiskers from Huanglian (Rhizoma Coptidis) and grind into fine powder. Take 1 qian per dose, mixed with congee water. Wine-steamed Huanglian (Rhizoma Coptidis) made into pills is also acceptable.
Yimu Wan (Mother Pill, from *Dquan Liangfang*). Treats fetal unrest caused by medication during pregnancy, presenting as deficiency-vexation, unable to lie down.
Use 1 liang of Zhi Mu (Rhizoma Anemarrhenae), wash and bake into fine powder. Use date flesh to make pills, size of a bullet. Take one pill per dose, chew thoroughly, and swallow with Ginseng decoction.
I often see physicians who do not recognize this syndrome, treating it as deficiency-vexation, thereby damaging the fetal Qi. Those with insight have seen this medicine. Obstetrician Zheng Zongwen obtained it from Chen Cangqi's *Bencao Shiyi*. It is effective. Another method uses Qinjiao (Radix Gentianae Macrophyllae) and Nuomifen (Semen Oryzae Glutinis) decoction to dissolve it, named Wan'an Wan.
Treats mental vexation and oppression during pregnancy, vomiting upon seeing food, nausea at irregular times, inability to eat, sudden fainting at irregular times, surging and full pulse, excess heat in the large and small bowels. (From Yang Zijian's *Wanquan Huming Fang*)
Maidong (Radix Ophiopogonis, remove heart), Fangfeng (Radix Saposhnikoviae), Fuling (Poria), Shanzhi (Fructus Gardeniae), Qianhu (Radix Peucedani, remove hairs), Shengma (Rhizoma Cimicifugae), Huangqin (Radix Scutellariae), Shigao (Gypsum Fibrosum, equal parts). Pound and sieve finely above into powder. Take 3 qian per dose. Add 1.5 zhan of water and 3 stalks of Congbai (Bulbus Allii Cepae). Boil for six or seven boils. Take after meals and before sleep.
