Q: What are the causes of abdominal pain, diarrhea, and dysentery occurring after childbirth?
A: After childbirth, the Spleen and Stomach are deficient and weak, making them susceptible to invasion by Cold pathogenic factors. If within one month postpartum, cold drinks are consumed or exposure to wind occurs, Wind-Cold pathogens invade due to deficiency, leading to dysentery with bloody stools (Chibai). Caution is advised.
Postpartum Qi and Blood are both deficient; dietary intake easily damages the Spleen and Stomach, causing disharmony between the Spleen and Stomach and failure to transform food and fluids. Consequently, abdominal distension, borborygmus, and diarrhea occur. If further exposed to Cold, the condition worsens.
Jiawei Wuji San (Modified Five Accumulations Powder): Treats postpartum injury caused by improper diet, severe abdominal pain, failure to digest food and fluids, profuse watery diarrhea with borborygmus.
Composition: Remove Ephedrae Herba (Mahuang) from Wuji San (Five Accumulations Decoction), add Alpiniae Officinarum Rhizoma (Gaoliangjiang), Aconiti Lateralis Radix Praeparata (Fuzi), Zingiberis Rhizoma Recens (Shengjiang), Mume Fructus (Wumei), and Jujubae Fructus (Hongzao). Decoct and take orally. Effective.
Diyu San (Sanguisorba Powder): Treats persistent postpartum diarrhea accompanied by irritability, thirst, fatigue, and anorexia.
Composition: Sanguisorbae Radix (Diyu), Cinnamomi Ramulus (Gui), Cardamomi Fructus (Caodoukou), Coptidis Rhizoma (Huanglian) [each 2 fen, remove stamens], Arecae Semen (Binglang) [crushed], Angelicae Sinensis Radix (Danggui) [sliced], Myristicae Semen (Roudoukou) [dehulled], Colla Corii Asini (Ejiao) [stir-fried]. Grind into fine powder. Take 2 qian each time, mixed with warm wine.

Commentary: Postpartum dysentery is not a single syndrome; treatment must follow the specific etiology. Since drinking cold and exposure to wind are mentioned, various pathogens may arise. Cold, Heat, Wind, and Dampness belong to external causes; Joy, Anger, Worry, and Thought stem from internal nature. Moreover, overexertion or lack thereof, hunger, and satiety can all cause disease. For profuse watery diarrhea, Tiaozhong Tang may be used; for red and white dysentery, other measures are required.
