There are five types of lower back pain. The first is Shaoyin lower back pain. The Shaoyin channel belongs to the Kidney. In the tenth lunar month, the yang qi of all things declines, leading to lower back pain. The second is wind-bi lower back pain. Wind-cold pathogen attaches to the waist, causing lower back pain. The third is Kidney deficiency lower back pain. Overexertion injures the Kidney, causing lower back pain. The fourth is trauma-induced lower back pain. Falling or striking the waist causes lower back pain. The fifth is cold-induced lower back pain. Injury from earthy damp-cold qi causes lower back pain. If the pain persists, it radiates to the lumbar spine, causing pain throughout.
Generally, regarding the pulse of lower back pain: if it is deep (Chen), wiry (Xian), and tight (Jin), it indicates Cold; if wiry and floating (Fu), it indicates Wind; if deep and soggy (Ru), it indicates Dampness. For pain caused by falling, striking, or spraining, where Qi and Blood stagnate, the pulse is often deep, wiry, and excess (Shi).
All meridians connect to the Kidney and network through the lumbar spine. Once Kidney Qi is deficient, various factors such as exposure to wind, contraction of dampness, accumulation of cold-heat, blood stasis, Qi stagnation, water retention, trauma, emotional frustration, and overwork can cause lower back pain to emerge continuously. "Chong Feng" refers to catching wind while sweating, which is the embryo of wind pathogen and wind toxin. "Shou Shi" refers to stepping in rain or lying on dampness, which is the萌芽 of heaviness, swelling, and stagnation. Cold sensation in the waist like water indicates injury from cold; thirst and constipation indicate accumulated heat. Stagnant blood causes pain like needle pricks when turning. Qi stagnation causes depression and inability to stretch. Accumulated water causes heaviness and impedes the diffusion of the Small Intestine. Trauma causes blood stasis to coagulate. Sprains and emotional loss are diseases of the Kidney. Given these conditions, how can the waist not hurt? The *Neijing* states: "The waist is the mansion of the Kidney." If rotation and shaking are difficult, one will feel exhausted even when lying down. Thus, if the pain is in the Shaoyin, one must investigate the source of the disease for proper treatment. However, the Zongjin gathers at the genitalia; the Liver is of the same system as the Kidney. The Five Zang organs obtain Qi from grain; the Spleen is the granary of the Kidney. Anger injures the Liver, causing flaccidity of all sinews; worry injures the Spleen, causing stagnation of Stomach Qi. These two can also become bandits causing lower back pain.
Taiyang lower back pain radiates to the neck, spine, sacrum, and back, feeling heavy. Yangming lower back pain prevents looking back; looking back may cause hallucinations and sadness. Shaoyang lower back pain feels like skin being pricked, with slow inability to bend/extend or look back. Taiyin lower back pain involves irritability and heat, feeling like a horizontal wood block in the lower waist, potentially leading to emaciation. Shaoyin lower back pain radiates pain to the inner aspect of the spine. Jueyin lower back pain feels like a drawn bowstring. This lists the Six Meridians as preparation for treating external causes. Generally, Taiyin and Shaoyin lower back pain often involve internal Cold; Shaoyang and Jueyin often involve Wind-Heat; Taiyin and Yangming often involve Dryness-Dampness. By inferring from these, one should distinguish the disease according to the pulse. As mentioned in the *Neijing*, lower back pain from Xie Mai, San Mai, Tong Yin, Hui Yin, Yang Wei, Heng Luo, Zhi Yang, Fei Yang, Rou Li, and Kao Jiao meridians does not exceed the flow of the Six Meridians but has specific dominances. I do not wish to cite them extensively; please refer to the *Neijing* chapter on Needling Lower Back Pain for clarity. Based on this, treating external causes involves sweating or purging methods. Emotional loss injuring the Kidney, anger injuring the Liver, and worry injuring the Spleen all cause lower back pain because the Liver and Kidney share a source, and the Spleen and Stomach are exterior-interior pairs. Spleen stagnation and Stomach closure most easily cause lower back pain. Symptoms include emaciation, dark complexion, inability to walk far or stand long, caused by emotional loss. Abdominal urgency, hypochondriac distension, blurred vision, unfulfilled desires, and flaccidity of Zongjin leading to white turbidity are caused by anger. Muscle wasting, numbness, indigestion, abdominal fullness, and constipation radiating to the waist and hypochondrium are caused by worry. Based on this, treating internal causes involves regulation. Renzhe lower back pain feels cold like water, body heavy but not thirsty, normal urination and appetite, with cold heaviness below the waist like hanging 5,000 coins. It results from long-term exposure to cold dampness after sweating during labor. Lower back pain with stooping, swelling, and heaviness leading to hypochondriac pain, or due to trauma with blood stasis, or sexual exhaustion depleting essence and Qi, causes lower back pain. Based on this, treating non-internal/external causes involves tonification and purgation. Lower back pain belongs to the Kidney, often due to post-exertion Kidney injury, empty organ Qi allowing wind-cold-dampness to invade, or anger/worry damaging it. However, for pain due to trauma and Qi-Blood stagnation, no specific medicine was previously mentioned. Weilingxian Wan is the main prescription. Nowadays, people often take Qian Niu (Pharbitis Seed) for lower back pain without distinguishing deficiency/excess. Unknowingly, Qian Niu damages Kidney Qi. If it is truly Qi stagnation lower back pain, three doses suffice. If ineffective, use Ju He (Tangerine Seed) fried with salt and soaked in wine, taken warm with Xiao Qi Xiang Wan for miraculous effect. If it is Kidney deficiency lower back pain, how can Qian Niu be suitable? Be cautious.
A man over sixty suffered from lower back, sacral, spinal, hip, and back pain for years. Quiet by day, restless by night, severe pain came and went, leading to suicidal thoughts. Pain worsened at dusk, requiring beating until dawn when it subsided. Those around him attributed the pain on both sides to ghosts, curses, or "White Tiger biting." They prayed morning and evening, summoning shamans and monks, which temporarily reduced pain. He dreamed of ghost battles and sacrificed to all temples. After years, he became emaciated, ate little, grew angry daily, and awaited death. A scholar said, "If it is a ghost curse, why did prayers fail? I heard Zhang Dairen in Chen Commandery is skilled in medicine. Ask him about the White Tiger and illness. If his skills fail, we accept fate." His family agreed. Dairen palpated both pulses: deep, stagnant, firm, and strong like a drawn bowstring. He said, "Though emaciated and unable to eat, pain in the waist/sacrum/spine/hip implies hard, dry stool." Others reported passing one or two hard, pellet-like stools every 5-9 days, requiring manual extraction, followed by whole-body itching and cracked skin. Knowing the deficiency/excess, Dairen secretly used Da Chengqi Tang decocted with Ginger and Jujube, adding 3 qian of Qian Niu powder. He did not write "purgative" because the patient feared cold/purging and preferred warmth/tonification. The decoction was given warm, increasing dosage over three days. At dusk, the bowels moved 4-5 times, expelling half a basin of dry fecal blocks mixed with bloody stasis, foul-smelling. Pain decreased by 90% instantly. He slept deeply, breathing normally. Waking next evening, he asked for porridge. After two more days of sleep, pain vanished. Dietary care followed, with Dao Yin Wan, Gan Lu San, and bowel-lubricating medicines. Recovery took over 40 days. The *Lingshu* states: "Even old excretions can be cleared; even old knots can be resolved." Waist/spine/hip pain belongs to the Foot-Taiyang Bladder Meridian. Hip pain belongs to the Foot-Shaoyang Gallbladder Meridian. The *Nanjing* says: "All pain is excess." The *Neijing* says: "All pain, itching, sores belong to Fire." Commentary: "Calm mind = mild pain; agitated mind = severe pain." Shamans reducing pain calms the mind, but doesn't remove the root. Ancient saying: "Pain decreases with benefit (bowel movement)." Without benefit, how does pain leave? The patient recovered and lived to 80. Thus, dryness syndromes belong to the Three Yang.
Wei Dexin from the North passed through Xijin. Drinking cold water in winter caused cold disease: straight back unable to bend/extend, heavy legs, difficult walking, carried on a bed. Doctors diagnosed Kidney deficiency, using Cong Rong, Ba Ji Tian, Fu Zi, Lu Rong. Constipation worsened, tidal fever rose. After nearly a year, he sought Dairen. Dairen said, "Ten days for effect." Wei asked if delay was an issue. Dairen said, "This is abundant blood in Taiyang. Disease makes waist feel broken, stiff like a knot, tearing like a split. Taiyang causes limited flexion/extension. Waist is Kidney's mansion and major joint. Now stiff, use salty herbs to soften. Few doses will relax it. *Nanjing* says: 'Forced intercourse injures Kidney, dries Marrow, ruins Diaphragm/Stomach.' Your case matches. Taiyang compressed by cold, blood stagnates downward. There must be blood accumulation in the waist, not Kidney deficiency." Sequential medication induced dozens of bowel movements, expelling 1-2 sheng of blood. Then steamed in a four-sided stove. Sweating 3-5 times cured him. On day 5, Dairen asked if abdomen gurgled. Wei said no. Day 6, gurgling started. Day 7, he could bow. Dairen warned: "Do not steam heat cases; it harms eyes. Your Kidney/Waist decline, poor vision/hearing, Yin attacking Yang, short Qi, weak spirit, wiry pulse—all indicate Yang deficiency hiding in Yin."
In Guiyou year, June-July, continuous rain/cold for over a month caused diarrhea in many (Dampness causing Five Diarrheas). One day, body heaviness, joint pain, three diarrheas, but closed, dark urine. *Neijing*: "Below, draw and exhaust." First promote urination. "Diarrhea with poor urination: first separate/drain." "Treat dampness without promoting urination is incorrect." Use bland-draining agents. However, external cold-damp invading internally is violent. Using bland-draining agents reduces it further, increasing Yin and exhausting Yang. Yang weakens, spirit fades. Here, ascending-Yang herbs are appropriate. Qiang Huo, Du Huo, Sheng Ma (1 qian each); Fang Feng, Zhi Gan Cao (0.5 liang each). Crushed. Decoct 3 bo to 1 bo. Strain. Drink hot. Cured. Scripture: "Cold-damp victory, soothe with Wind." "Sinking, lift it." Ascending Yang wins, hence cure. This is bending to straighten. Sage methods are analogical; one example reveals hundreds.
