Peach Kernel (Tao Ren, 50 seeds, skin and tip removed, sweet and neutral) Cinnamon Twig (Gui Zhi, 2 liang, bark removed, pungent and hot) Rhubarb (Da Huang, 4 liang) Mirabilite (Mang Xiao, 2 liang) Licorice Root (Gan Cao, 2 liang, honey-fried).
In cold damage disease after eight or nine days, if purgation is mistakenly applied, symptoms such as chest fullness, irritability, palpitations, difficulty in urination, delirium, heaviness of the whole body, and inability to turn the body occur. This is the indication for Chaihu Jia Longgu Muli Tang (Bupleurum Decoction with Dragon Bone and Oyster Shell).
(After eight or nine days of cold damage, the pathogenic factor has transformed into heat and transmitted to the Yang meridians. Mistaken purgation weakens the interior, while the heat remains. Chest fullness and irritability are caused by Yang-heat. Heaviness of the whole body and inability to turn indicate that Yin qi moves internally and fails to nourish the exterior. Administer Chaihu Tang (Bupleurum Decoction) to relieve chest fullness and irritability; add Long Gu (Dragon Bone) and Mu Li (Oyster Shell) to anchor floating Yang.)
Formula for Chaihu Jia Longgu Muli Tang:
Pinellia Tuber (Ban Xia, 2 he, washed) Jujube Fruit (Da Zao, 6 pieces) Bupleurum Root (Chai Hu, 4 liang) Fresh Ginger (Sheng Jiang, 1.5 liang) Codonopsis Root (Dang Shen, 1.5 liang) Dragon Bone (Long Gu, 1.5 liang)
Oyster Shell (Mu Li, 1.5 liang) The above eleven ingredients. Boil in 8 sheng of water... Strain and take 1 sheng warm.
In cold damage disease, abdominal fullness and delirium occur, with a floating and tight pulse at the Cun position. This is Liver Wood overacting on Spleen Earth, named "Zong" (Vertical Overaction). Prick Qimen (LR14).
(Abdominal fullness and delirium are diseases of the Spleen and Stomach. A floating and tight pulse indicates the Liver pulse. When Spleen disease presents with a Liver pulse, it is Wood overacting on Earth. The Classics state...)
In cold damage disease, fever and aversion to cold occur, accompanied by extreme thirst and desire to drink water, leading to abdominal fullness. If spontaneous sweating and free urination occur, the disease will resolve. This is Liver Wood overacting on Lung Metal.
(Fever and aversion to cold indicate Lung disease. Extreme thirst and desire to drink indicate Liver Qi excess. The Yu Han Jing states: presenting with extreme thirst and desire to drink vinegar juice...)
In Taiyang disease, restlessness appears on the second day contrary to expectation. Counterfeiting the back induces profuse sweating. Excessive heat enters the Stomach, exhausting Stomach fluids, causing restlessness and irritability, inevitably leading to delirium. After more than ten days, if tremors and diarrhea occur, this indicates the recovery of Zheng Qi, and the disease will resolve.
(On the second day of Taiyang disease, the pathogen is in the exterior and should not cause restlessness. Restlessness indicates heat moving inward. Counterfeiting the back induces sweating, leading to excessive heat entering the Stomach and fluid exhaustion. Tremors and diarrhea after ten days indicate Yin fluid recovery and the expulsion of pathogens.)
In Taiyang Wind Strike, using fire coercion to induce sweating causes the wind pathogen to be forced by fire heat, making blood and Qi flow excessively and lose their normal rhythm. The combination of two Yangs scorches the body, causing jaundice. Excess Yang leads to epistaxis; Yin deficiency leads to difficult urination. With both Yin and Yang exhausted, the body becomes dry. Only the head sweats, stopping at the neck. Abdominal fullness, slight wheezing, dry mouth, throat ulceration, constipation, delirium, severe cases of hiccups, restless limbs, and picking at clothes or beds may occur. If urination is free, the patient can be treated.
(Wind is a Yang pathogen. Combined with fire heat, the pathogen intensifies, forcing blood and Qi to flow excessively. This combination is called "Flaming Up." The Classics state: Internal fire emission causes dry mouth and throat ulcers due to upward scorching. If heat rises but does not descend, stool is not hard. If heat descends into the Stomach, hiccups may occur...)
In cold damage disease with a floating pulse, if a physician uses fire coercion to induce sweating, Yang collapse will inevitably cause panic and mania, with restlessness in sitting and lying. Treat with Guizhi Qu Shaoyao Jia Shuqi Muli Longgu Jiuni Tang (Cinnamon Twig Decoction without Peony plus Chinese Asarum, Dragon Bone, and Oyster Shell to Rescue from Collapse).
(A floating pulse indicates the pathogen is in the exterior. Fire coercion causes profuse sweating, leading to Yang collapse. Sweat is the fluid of the Heart; Yang collapse leads to Heart Qi deficiency... Use astringents to secure...)
Formula for Guizhi Qu Shaoyao Jia Shuqi Muli Longgu Jiuni Tang:
Cinnamon Twig (Gui Zhi, 3 liang, bark removed) Licorice Root (Gan Cao, 2 liang, honey-fried) Fresh Ginger (Sheng Jiang, 3 liang, sliced) Oyster Shell (Mu Li, 5 liang, calcined, sour and salty) Dragon Bone (Long Gu, 4 liang, sweet and neutral)
Boil Chinese Asarum (Shu Qi) first, reduce by 2 sheng...
The presentation resembles cold damage, but the pulse is weak rather than string-tight. Weakness implies thirst; fire attack implies delirium. If weakness presents with fever and a floating pulse, resolve the exterior; sweating will lead to recovery.
(Resembling cold damage refers to headache and fever. A pulse that is not string-tight lacks the exterior pulse of cold damage. The Classics state: Weakness with fever indicates interior heat...)
In Taiyang disease, if moxibustion is used without inducing sweating, the patient will become irritable. If the disease does not resolve when the cycle ends, bleeding will occur. This is called Fire Pathogen.
(This is fire pathogen forcing blood downward. In Taiyang disease, moxibustion without sweating prevents heat from exiting. Yin deficiency combined with fire causes irritability...)
If the pulse is floating with intense heat, moxibustion is contraindicated. This is an Excess pattern treated as Deficiency. Due to fire, blood will move, causing dry throat and bloody saliva.
(This is fire pathogen forcing blood upward. Floating pulse with intense heat indicates exterior Excess. Treating it as Deficiency with fire moves blood...)
A slightly rapid pulse should never be treated with moxibustion. Fire becomes a pathogen, causing irritability and rebellion. It pursues Deficiency and attacks Excess, scattering blood in the vessels. Although fire energy is subtle, its internal attack is powerful.
(A slightly rapid pulse indicates Heat. Moxibustion dispels Cold but cannot clear Heat, so it is contraindicated. If moxibustion is used, heat intensifies due to fire...)
If the pulse is floating, sweating should be used to resolve the exterior. Using moxibustion prevents the pathogen from exiting, allowing it to flourish due to fire. The lower body will become heavy and numb. This is called Fire Reversal.
(A floating pulse indicates the exterior, suitable for sweating. If moxibustion is used without sweating, the pathogen cannot exit, and heat increases due to fire assistance...)
Spontaneous resolution requires prior irritability, followed by sweating and resolution. How do we know? Because the pulse is floating, indicating the pathogen returns to the exterior via sweating.
(Irritability indicates Heat. Pathogen returning to the exterior causes irritability. Sweating resolves the disease because the pulse is floating, indicating the pathogen is at the exterior.)
If needle-cauterization induces sweating, and the needle site is exposed to cold, forming red nodules, Ben Tun (Rolling Pig) will inevitably occur, with Qi rushing from the lower abdomen to the heart. Moxa one cone on each nodule. Administer Guizhi Jia Gui Tang (Cinnamon Twig Decoction plus Cinnamon).
(Needle-cauterization damages Yin Blood and startles Heart Qi. Cold accumulation at the needle site forms nodules. Heart Qi becomes deficient due to fright, allowing Kidney Qi to rush up with cold. Moxa the nodules to disperse cold. Administer Guizhi Jia Gui Tang to drain Ben Tun Qi.)
If Fire Reversal is followed by purgation, and irritability occurs due to needle-cauterization, treat with Guizhi Gancao Longgu Muli Tang (Cinnamon Twig and Licorice Decoction with Dragon Bone and Oyster Shell).
(Fire is reversal; purgation further weakens the interior. Needle-cauterization adds to the interior deficiency, causing irritability due to fire heat. Administer Guizhi...)
Formula for Guizhi Gancao Longgu Muli Tang:
Cinnamon Twig (Gui Zhi, 1 liang) Licorice Root (Gan Cao, 2 liang) Oyster Shell (Mu Li, 2 liang, calcined) Dragon Bone (Long Gu, 2 liang)
(Pungent and sweet diffuse. The pungency and sweetness of Cinnamon Twig and Licorice disperse fire pathogen in the meridians. Astringency removes collapse. The astringency of Dragon Bone and Oyster Shell anchors floating...)
In Taiyang Cold Damage, adding warm needles will inevitably cause fright.
(Cold injures Ying Qi. If Ying Qi is weak, warm needles cause blood stasis. Fright results from warm needles damaging Ying Blood and disturbing Heart Qi. The Jin Gui Yao Lue states...)
In Taiyang disease, there should be aversion to cold and fever. Now, there is spontaneous sweating, no aversion to cold, but fever, with a fine and rapid pulse at the Guan position. This is due to erroneous vomiting by the physician. After one or two days of vomiting... caused by...
(Aversion to cold and fever indicate Taiyang exterior disease. Spontaneous sweating without aversion to cold indicates Yangming syndrome. The original Taiyang exterior disease was wrongly treated with vomiting, damaging... becoming heat... vomiting... morning food intake... thus stopping...)
In Taiyang disease, vomiting causes the patient to no longer feel aversion to cold and to dislike clothing. This is internal irritability caused by vomiting.
(Taiyang exterior disease wrongly treated with vomiting damages Stomach Qi. Pathogenic heat enters the Stomach due to deficiency, causing internal irritability, hence no aversion to cold and dislike of clothing...)
If the patient's pulse is rapid, indicating Heat, digestion should be accelerated, but vomiting occurs instead. This is because sweating made Yang Qi weak and Diaphragm Qi deficient, resulting in a rapid pulse. Rapid indicates...
(Yang receives Qi in the chest. Sweating externally weakens Yang Qi, causing Yang Qi weakness and Diaphragm Qi deficiency. Rapid pulse indicates Heat. Original Heat aids digestion, but guest Heat does not...)
After Taiyang disease passes the meridian cycle for more than ten days, there is nausea with a desire to vomit, chest pain, loose stools, slight abdominal fullness, and slight irritability. Previously... extreme... vomiting...
(Nausea, irritability, and chest pain indicate pathogenic heat in the chest. Loose stools and abdominal fullness indicate pathogenic heat has moved downward... pathogen, if... Stomach heat...)
In Taiyang disease after six or seven days, exterior signs remain, but the pulse is faint and deep, without Jie Xiong (Chest Binding). The patient goes mad because heat is in the Lower Jiao, and the lower abdomen should be hard and full.
(Taiyang is the meridian; Bladder is the fu organ. This is Taiyang transmitting to the fu organ. On the sixth or seventh day, when pathogen transmits inward, a faint and deep pulse indicates pathogen... binding... no blood...)
Formula for Didang Tang (Definitely Break Through Decoction):
Leech (Shui Zhi, 30 pieces, calcined, salty, bitter, cold) Drone Fly (Meng Chong, 30 pieces, calcined, wings and feet removed, bitter, slightly cold) Peach Kernel (Tao Ren, 20 seeds, skin and tip removed, bitter, sweet, neutral)
(For moving blood, salty overcomes blood. The salty and bitter nature of Drone Fly and Leech removes blood stasis. Sweet moderates constriction, bitter drains heat. The bitterness of Peach Kernel and Rhubarb promotes downward movement...)
In Taiyang disease, jaundice, deep and knotted pulse, hard lower abdomen, and difficulty in urination indicate no blood stasis. Free urination and manic behavior confirm blood stasis.
(Jaundice, deep knotted pulse, hard lower abdomen, and difficulty in urination indicate Stomach heat causing jaundice. Can use Yinchen Tang (Artemisia Capillaris Decoction). Jaundice, deep knotted pulse, hard lower abdomen...)
In cold damage disease with fever and lower abdominal fullness, urination should be difficult. If it is free, it indicates blood stasis. Purge it. Do not leave any residue; use Didang Wan (Definitely Break Through Pill).
(Fever and lower abdominal fullness indicate blood stasis in the Lower Jiao. If heat accumulates, fluids do not circulate, causing difficulty in urination. If heat does not accumulate, fluids...) Also...
Formula for Didang Wan:

(If drinking much water with free urination, water does not accumulate internally. But excessive water in the abdomen causes palpitations in the epigastrium. The Jin Gui Yao Lue states: Eating little and drinking much, water...)
