Injuries caused by metal blades may result in evisceration (protrusion of intestines), or the intestines may protrude and subsequently rupture. By observing the severity of the symptoms, one can determine the prognosis for life or death. If only one end of the intestine is visible, it should not be sutured; instead, it should be rapidly blown back into the body with the mouth.
Subsequently, administer supportive decoctions. In cases where abdominal wall injuries are complete with intestines exposed externally, treatment is difficult only if the intestines are completely severed. However, if the injury involves tearing without severance, all such cases are treatable. For injuries involving both intestinal and abdominal wall rupture, use Huarui Shi (Pyrite) as a suture thread. The incision size should match the wound aperture. If the intestines and omentum (abdominal fat) protrude, they should be reduced into the cavity followed by suturing. If the aperture is small and only the omentum protrudes, first administer Qingxin Yao (Heart-Clearing Medicine), manually retract the omentum, clear the area, and then proceed with treatment.
Cishi San (Magnetite Powder) [From *Shenghui Fang*]
Indicated for evisceration due to metallic wounds. The powder should be applied internally.
Cishi (Magnetite, ground), Huashi (Talcum, ground), Tiejing (Iron Essence) — two liang each. These are equal parts, ground together into a fine powder. Take one qianbi (a small measuring spoon) per dose, mixed with warm wine. It can also be taken on an empty stomach with rice gruel. Three times a day.
Formula for Treating Evisceration from Metallic Wounds [From *Shenghui Fang*]
Take dried human feces, grind into powder, and apply to the intestines before reducing them. If the intestines are dry, moisten them with thick wheat starch paste before reduction. Spray cold water on the patient's back while instructing them to inhale deeply, which facilitates reduction.
For treating metallic wounds where intestines protrude but remain intact, preventing them from drying out and adhering to grass or soil [From *Shenghui Fang*]
Prepare barley porridge and use its juice to wash the intestines before pushing them back. Maintain a diet of rice gruel. After twenty days, gradually introduce slightly thicker congee. Full recovery is expected after one hundred days. Avoid contact with grass and soil.
For metallic wounds where intestines protrude and cannot be reduced [From *Shengji Zonglu*]
Take three sheng of wheat and nine sheng of water. Boil until five sheng remain. Filter through silk cloth and let cool. Hold the liquid in the mouth and spray it onto the wound to gradually reduce the intestines. Simultaneously, spray cold water on the patient's back. Do not use excessive amounts... Sutures for metallic wounds with evisceration [See Sangpi Formula].
Another Formula [From *Bencao*]
Treating metallic wounds with evisceration.
Use Sangbaipi (Cortex Mori / Mulberry Bark) as a thread for suturing. Apply hot chicken blood over the suture site for immediate healing. This method was effective when Tang Anzhang underwent a cesarean section.
