Coldness of the female genitals can be categorized into cold syndromes and heat syndromes. Cold syndromes arise from deficiency of Yang, representing a true cold syndrome; heat syndromes arise from damp-heat, presenting as a false cold syndrome. False cold must necessarily be accompanied by heat signs, such as scanty, frequent, and dark yellow urination, dry and constipated stools, irritability, and excessive thirst. True cold is characterized by clear and profuse urination, along with aversion to cold due to Yang deficiency. For true cold, it is appropriate to tonify Yang using formulas such as Liyin Jian (Reinforce Yin Decoction), Shibu Wan (Ten Tonics Pill), or Jiajian Xusi Jiangsheng Dan (Modified Fertility and Birth Support Pill). For false cold, one should clear fire using Longdan Xiegan Tang (Gentian Liver-Draining Decoction) or Jiawei Xiaoyao San (Modified Free Wanderer Powder). In cases of deficiency-cold of the Liver and Kidney, Nuangan Jian (Warm Liver Decoction), Zhenyin Jian (Settle Yin Decoction), and Daying Jian (Great Camp Decoction) are indicated. For deficiency-cold of the Spleen and Stomach, Lizhong Tang (Regulate Middle Decoction), Liyin Jian (Reinforce Yin Decoction), and Shoupi Jian (Spleen-Sustaining Decoction) are suitable.
Vaginal bleeding immediately upon sexual intercourse in women is mostly caused by weakness of Yin Qi, insecurity of the Kidney Yuan (Primordial Qi), or the presence of fire in the Yin aspect. If the cause is Spleen Qi deficiency with sinking Qi failing to control blood, Buzhong Yiqi Tang (Tonify the Middle and Augment Qi Decoction) or Buyin Yiqi Jian (Nourish Yin and Augment Qi Decoction) should be used. If there is Spleen-Kidney deficiency with insecure Yin Qi, Shoupi Jian (Spleen-Sustaining Decoction) or Guipi Tang (Restore the Spleen Decoction) are appropriate. If Liver-Kidney Yin deficiency leads to failure of Yin blood to remain within, Guyin Jian (Secure Yin Decoction) is indicated. If Yin fire moves blood causing hemorrhage, Baoyin Jian (Protect Yin Decoction) should be employed.
