Hongjiao (a disease characterized mainly by swelling and pain in the legs and feet) is caused by wind-cold-damp-heat pathogens. Its symptoms manifest as alternating chills and fever, resembling Shanghan (Cold Damage). If the legs and feet are painful, swollen, and hot, it is due to Fire (excess heat); if they are not swollen or hot but painful, it is due to Cold (cold-dampness).
[Annotation] Hongjiao arises from internal damp-heat combined with external contraction of wind-cold, leading to alternating chills and fever, presenting similarly to Shanghan. If both legs and feet are painful, swollen, and burning hot like fire, this indicates an excess of Fire; if there is no swelling or heat but only pain, this indicates an excess of Cold. This condition is named "Dry Hongjiao."
In Hongjiao, the pulse is rapid/tight, and the lower abdomen feels numb and stiff, with loss of sensation to pain and itch. Within three to five days, if the pathogen invades the Heart region, symptoms such as vomiting, panting, chest fullness, blackening of the eyes and forehead, mental confusion, and delirium may occur. At this stage, the condition is critical and life-threatening.
[Annotation] In Hongjiao, a rapid/tight pulse accompanied by numbness and stiffness in the lower abdomen, with insensitivity to pain and itch, suggests that within three to five days, the pathogen will inevitably invade the Heart. If it invades the Heart, resulting in vomiting, panting, and chest fullness, this is the syndrome of Hongjiao rushing to the Heart. Blackening of the eye sockets and forehead, along with mental confusion and delirium, signifies Water overcoming Fire, hence stating that life cannot be preserved.
