The carpal bones, also known as the metacarpals (metacarpal bones), are the proximal joints of the five fingers, alternatively called Yonggu (Ong Bone), and commonly referred to as "Tiger Bone." There are six bones of varying sizes that converge to form the palm; they are not a single solid mass of muscle. Their bases connect superiorly to the ends of the two auxiliary bones of the forearm. The bone on the lateral side is named Gaogu (High Bone), also known as Ruigu (Sharp Bone) or Huai Gu (Ankle Bone), and colloquially as Longgu (Dragon Bone). It is termed "Wan" (Wrist) because it can flex and extend up and down. If falling from a carriage or horse with the palm striking the ground, only the wrist may be injured; however, if the fingers strike the ground and flip back to adhere against the forearm, the joint space of the wrist must separate. For wrist injuries characterized by swelling and pain, the treatment method involves massaging the wrist with both hands, taking internally Zhenggu Zijin Dan (Orthopedic Purple Gold Pill), and applying externally Wanling Gao (Ten Thousand Spirits Ointment); if the dorsum of the hand flips backward to adhere against the forearm, use both hands to press the dorsum and gently rotate it back into position, then massage the tendons to ensure they are aligned smoothly, taking internally Renshen Zijin Dan (Ginseng Purple Gold Pill) and applying Hunyuan Gao (Primordial Unity Ointment) externally.

The bones of the five fingers are named Chuigui (Hammer Bones), referring to the proximal phalanges (proximal joints) of each finger. If fractured by blunt force trauma, the condition is similar for all five fingers, resulting in connected swelling and pain because their tendons are interconnected. Although the palm and dorsum appear externally as one undifferentiated unit, internally the bones consist of the proximal joints of each finger connected together. If both the dorsum and palmar surfaces become hard, swollen, hot, and painful, the bone joints must be realigned to prevent future complications. If not treated promptly, the stagnant blood will eventually transform into pus. Those with robust Qi may take formulas for treating sores and toxins, and recover with proper care; those with deficient Qi may develop fistulas later. Wash with Sanyu He Shang Tang (Blood-Dispersing and Injury-Healing Decoction) and apply Wanling Gao.

The Zhujie Gu (Bamboo-Joint Bone) refers to the middle phalanges (secondary joints) of each finger. In cases of falls or blows causing bone fragmentation and tendon curvature where the finger cannot extend, manipulating the flexed joint by hand will allow the finger to straighten out. Wash with Sanyu He Shang Tang and apply Wanling Gao. If toxic blood accumulates in the nail fold, the nail will inevitably fall off; if a new nail grows, its shape will likely differ from the original. If the distal phalanx (third segment) is injured, the treatment is the same as for the middle segment; the nail is also called Zhua Jia (Claw Nail).
