Ten Incurable Conditions:
Falling or being struck with injury penetrating into the Lung (Zang organ), even if death does not occur immediately, it is difficult to survive beyond fourteen days; penetrating internal injury under the left hypochondrium; rupture of the intestines; severe internal injury in the lower abdomen; conditions with numerous and chaotic symptoms; damage to the external genitalia; crushing fracture of the left thigh in the elderly; exhaustion of blood; penetrating internal injury in the shoulder interior and behind the ear; pulse that is neither firm nor heavy upon deep palpation.
Medication is unnecessary for the above conditions.
The crown (Dian) refers to the top of the head. In men, the cranial suture forms a trident shape, while in women, it forms a cross shape. It is also known as Tianlinggai (Heavenly Spirit Cover). Located at the highest point of the body, it encloses the brain marrow like a lid, governing the entire body. If injury results from jumping or collision, presenting as sudden death, stiff body, audible respiration through nose and mouth, closed eyes, earth-colored complexion, warmth and pulsation in the heart region, this condition is treatable. One must absolutely avoid forcibly prying joints or lifting the patient into a seated position, as this may cause Qi (vital energy) disturbed by shock to rush upward, or escape through the wound or the seven orifices (eyes, ears, mouth, nose), inevitably endangering life. The patient should be placed in a flexed lateral recumbent position. First, apply Hunyuan Gao (Mixed Primordial Ointment) mixed with aged vinegar to the crown to relieve pain, reduce swelling, invigorate blood, and draw out toxins. Then, light rolled straw paper and smoke the patient's nose and mouth with the fumes. Alternatively, quench hot coal in vinegar to generate heat vapor for fumigation of the nose and mouth; if coal is unavailable, quenched red-hot iron works equally well. This guides the blood vessels of the five Zang organs to achieve smooth circulation. Once moaning sounds are heard from the mouth, administer Bali San (Eight-Centigram Powder) warmed with child's urine (Tongbian) to restore Qi flow and Yang energy. Externally, use manual techniques to push and press the chest, heart, hypochondriac regions, axillae, and abdomen. Gently lift and manipulate the tendons at the inner wrist, frequently rubbing and massaging, specifically the high bone posterior to the palm (the Cun, Guan, Chi pulse-taking sites).
For collision injuries causing stiffness of sinews and vessels, frequent rubbing and massaging restores Heart blood and unblocks the vital pulse, potentially reviving the patient. Regularly take Zhenggu Zijin Dan (Bone-Setting Purple-Gold Pill) externally wash off the previous Hunyuan Gao using Sanyu He Shang Tang (Blood-Dispelling and Injury-Healing Decoction), then reapply Hunyuan Gao. After taking the pills, if stools are black and dry, this indicates residual blood stasis in the intestines and stomach. For cases of deafness, administer Jiajian Suzi Taoren Tang (Modified Perilla Seed and Peach Kernel Decoction) to expel blood stasis, strengthen the Spleen and Stomach, and nourish the spirit. Simultaneously, insert Daoqi Tongyu Ding (Qi-Moving and Stasis-Unblocking Plaster) into the ear. Diet should consist of plain porridge and soups; avoid anger, greasy foods, and wheat products. The resting environment should be clean and quiet, free from noise and disturbance. If the injury is so severe that death has already occurred, wrap the head with white cloth, gently tap the sole of the foot with a wooden stick, pull the hair to align the head straight, stabilize the cervical vertebrae, and relax the sinews. Apply Hunyuan Gao externally and administer Zijin Dan internally. For injuries to the crown sutures from falling from carts or horses, the pathogenic factor tends to settle on the left more than the right due to the dominance of the right hand; the treatment method is the same as for other collision injuries. If the cranial bone collapses, disturbing the brain marrow, with bleeding from the seven orifices, rigid extension, convulsions, coma, and complete loss of consciousness, it is incurable.
