虽云不动。自有东行之意。以其上动而下静。不得不尔也。天地之道如故。 守所言。从乎

阴证身静。重语无声气难布息。目睛不了了。鼻中呼不出吸不入。往来口与鼻中气冷。

Discussed in terms of upper and lower, it is like the rotation of a wheel. The Heavenly Way (rotation of celestial bodies) rotates to the left, with the sun situated in the west; the Earthly Way (bearing of the earth) rotates to the right, while the east seems not to match perfectly. Generally speaking, blood follows Qi, just as a husband sings and his wife follows. Although blood follows Qi, its substance is static and unmoving. Thus, the relationship between Qi and Blood is like the shape of a millstone: the upper part turns towards the west, while the lower part remains stable and unmoving. Although the lower part does not move, it naturally has an intention to move towards the east. Because the upper moves and the lower is still, it must be so. The way of Heaven and Earth remains as before. Wang Shou's words follow Heaven. From Gen trigram to Xun trigram, Jin Ling's words follow Earth. From Qian trigram to Kun trigram. Thus, the functions of Qian and Kun are complete. Speaking of the Heavenly Way is from outside to inside. Speaking of the Earthly Way is from inside to outside. From outside entering inside is Shanghan (Cold Damage). From inside emerging outside is Zabing (Miscellaneous Diseases).
Suppose Shaoyang syndrome occurs: headache, alternating chills and fever, floating and wiry pulse. If any one of these three symptoms exists, it is a Exterior Syndrome. Loss of taste in the mouth, disharmony in the abdomen, constipation or diarrhea with irregularity. If any one of these exists, it is an Interior Syndrome. If there are no Exterior/Interior or Upper/Lower symptoms, the rest are all Deficient Heat. The principle lies here.
Yin Syndrome: The body is quiet, speech is low or silent, breath is difficult to distribute, vision is blurred, nasal breathing is obstructed (exhalation out, inhalation in), air from mouth and nose feels cold, inability to take fluids, incontinence of urine and stool, aversion to cold on the face as if scraped by a knife.
Yin Syndrome (active type): The body moves, speech is light but audible, vision is clear, nasal breathing is smooth and regular, air from mouth and nose is normal.
If the body surface is cool, the disease is in the Yin Meridians. Named Yin Syndrome.
If the body surface is hot, the disease is in the Yang Meridians. Named Yang Syndrome.
Wind Injury: Nasal exhalation is coarse, mouth closed and cannot open, because Lung Qi connects to Heaven. Food Injury: Tasteless mouth, saliva not retained, nasal respiration is even. Spleen Qi connects to...
External injury causes whole-body heat, first involving the Taiyang Meridian. From outside to inside, initially involves formless pathogens.
Internal injury causes disharmony of hands and feet, heat in both hypochondriac regions, indicating initial involvement of the Shaoyang Meridian. From inside to outside, initially involves tangible pathogens.
Both external and internal injuries: Renying and Qikou pulses are both full, or solid and large upon lifting and pressing. Exterior heat with aversion to cold, abdominal disharmony with excessive saliva. This is dual injury of exterior and interior.
In general diagnosis, one must palpate the palms and backs of hands. Heat in the palm indicates Internal Injury; heat in the back of the hand indicates...

Shanghan discusses Foot Meridians; Hand Meridians have excess [pathogen], Foot Meridians have deficiency [of righteous Qi].
Winter injury by Cold leads to Warm Disease in Spring, Heat Disease in Summer, and Great Heat Disease in Late Summer. These diseases arise from sexual exhaustion and hard labor. Kidney Water deficiency fails to nourish, resulting in Heat Disease. In Late Summer, when Fire is prosperous and equates with Autumn, Hand Meridians dominate. When Wood is prosperous and equates with Spring, Foot Meridians dominate. The injury affects the deficiency of Foot Meridians. When Summer Fire becomes prosperous, pathogenic Qi assists the Hand Meridians, making the deficiency worse. Therefore, the medicines used drain excess rather than tonify Foot Meridians. Why? Draining excess benefits the deficient. This is because Shanghan originally belongs to Foot Meridians, hence only discussing Foot Meridians and not Hand Meridians. The main idea is thus. Cases transmitting to Hand Meridians do exist and should be discussed separately. Diseases of the Extraordinary Vessels are also included herein.
