Women's various diseases are the same as men's; however, during menstruation and pregnancy, they cannot be treated according to standard rules. Therefore, a specific category for "Menstrual and Obstetric Conditions" is established.
Using Siwu Tang (Four Substances Decoction) as the sovereign drug, its profound meaning is far-reaching. [Danggui Radix Angelicae Sinensis, Shudihuang Rhizoma Rehmanniae Preparata, Chuanxiong Rhizoma Ligustici Chuanxiong, Baishao Radix Paeoniae Alba.] For Taiyang Exterior Deficiency Syndrome (Syndrome of the Bladder Channel characterized by exterior deficiency) caused by Cold Damage, use Guizhi Twig and Digupi Cortex Lycii. [Manifestations include spontaneous sweating due to exterior deficiency, fever, aversion to cold, headache, and floating pulse. Adding Guizhi Twig and Digupi Cortex Lycii, each 7 qian, to 4 liang of Siwu Tang results in two herbs that release the muscle layer and consolidate the exterior, named Biaoxu Liuhe Tang (Exterior Deficiency Six-Harmony Decoction).] For Taiyang Exterior Excess Syndrome (Syndrome of the Bladder Channel characterized by exterior excess), use Xixin Herba Asari and Mahuang Herb Ephedrae. [Manifestations include headache, body heat, absence of sweat, and tight pulse. Adding Xixin Herba Asari and Mahuang Herb Ephedrae, each 5 qian, to 4 liang of Siwu Tang results in two herbs that warm the channels and induce sweating, named Biaoshi Liuhe Tang (Excess Six-Harmony Decoction).] For Shaoyang Syndrome (Syndrome of the Gallbladder Channel), add Chaihu Bupleuri Radix and Huangqin Scutellariae Radix. [Manifestations include alternating chills and fever, hypochondriac pain, irritability with a desire to vomit, bitter taste in the mouth, and wiry pulse, indicating Shaoyang Syndrome. Adding Chaihu Bupleuri Radix to release the exterior and Huangqin Scutellariae Radix to clear interior heat results in Chaihu Liuhe Tang (Bupleurum Six-Harmony Decoction).] For Yangming Syndrome (Syndrome of the Stomach Channel), add Shigypsum and Zhimu Anemarrhenae Rhizoma. [Manifestations include high fever, severe thirst, and large, long pulse, indicating Yangming Syndrome. Adding Baihu Tang (White Tiger Decoction) to clear Lung heat and purge Stomach fire results in Shigao Liuhe Tang (Gypsum Six-Harmony Decoction).] For difficulty in urination, add Fuling Poria and Zexia Plantaginis Seed to promote diuresis, resulting in Fuling Liuhe Tang (Poria Six-Harmony Decoction). For insomnia, Huangqin Scutellariae Radix and Zhizi Gardeniae Fructus are beneficial. [Following sweating or purging therapies, if sleeplessness occurs, adding Huangqin Scutellariae Radix and Zhizi Gardeniae Fructus nourishes Yin and eliminates irritability, named Zhizi Liuhe Tang (Gardenia Six-Harmony Decoction).] For Wind-Dampness, use Fangfeng Saposhnikoviae Radix and Cangzhu Atractylodis Rhizoma. [Accompanied by wind and dampness, causing painful joints, internal heat, and floating pulse, adding Fangfeng Saposhnikoviae Radix to dispel wind and Cangzhu Atractylodis Rhizoma to dry dampness results in Fengshi Liuhe Tang (Wind-Dampness Six-Harmony Decoction).] For macules appearing due to accumulated toxicity, Shengma Cimicifugae Rhizoma and Lianqiao Forsythiae Fructus take command. [If the condition persists after purging, and toxicity manifests as macules resembling brocade patterns, adding Shengma Cimicifugae Rhizoma and Lianqiao Forsythiae Fructus disperses fire and resolves toxicity, named Shengma Liuhe Tang (Cimicifuga Six-Harmony Decoction).] For fetal restlessness and vaginal bleeding, it is named Jiao'ai Tang (Donkey-hide Gelatin and Moxa Leaf Decoction), specifically Jiao'ai Siwu Tang (Donkey-hide Gelatin and Four Substances Decoction). [After sweating or purging therapies for Cold Damage, if fetal restlessness and bleeding occur, adding Ejiao Donkey-hide Gelatin and Aiyе Folium Artemisiae Argyi to nourish blood and calm the fetus results in Jiao'ai Siwu Tang.] For chest fullness and epigastric distension, Houpo Magnoliae Bark and Zhishi Immature Bitter Orange are quite appropriate. [Manifesting as chest fullness and distension, adding stir-fried Houpo Magnoliae Bark and Zhishi Immature Bitter Orange dissipates fullness and eliminates stagnation, named Pushi Liuhe Tang (Magnolia and Bitter Orange Six-Harmony Decoction).] For deep pulse and cold extremities, Guizhi Twig and Fuzi Aconiti Lateralis Radix Preparata may also be used. [Manifesting as cold body, stiffness, abdominal pain, and deep pulse, in desperate cases, adding Fuzi Aconiti Lateralis Radix Preparata and Rougui Cinnamomi Cortex to dispel cold and restore yang results in Fuzi Liuhe Tang (Aconite Six-Harmony Decoction).] For constipation and blood stasis, Taoren Prunus Seed and Dahuang Rheum Radix et Rhizoma are used. [Manifesting as hard stools, red urine, rapid and full pulse, or bladder blood stasis, adding Taoren Prunus Seed and Dahuang Rheum Radix et Rhizoma moistens dryness and unblocks the bowels, named Dahuang Liuhe Tang (Rhubarb Six-Harmony Decoction).] Calming the fetus and nourishing blood should be the primary focus; other conditions require careful consideration based on specific symptoms. Later generations applied this method to treat menstrual disorders, distinguishing between warming and cooling approaches for excessive or scanty flow. For Wen Liuhe Tang (Warm Six-Harmony Decoction) treating excessive flow, add Huangqin Scutellariae Radix and Baizhu Atractylodis Macrocephalae Rhizoma. [Adding Huangqin Scutellariae Radix to suppress yang and Baizhu Atractylodis Macrocephalae Rhizoma to tonify Spleen qi, as the Spleen controls blood, treats excessive menstruation.] If the blood is dark and the cycle is delayed, consider Lianfu Liuhe Tang (Coptis and Cyperus Six-Harmony Decoction) with Huanglian Coptidis Rhizoma and Xiangfu Cyperi Rhizoma. [Adding Huanglian Coptidis Rhizoma to clear heat and Xiangfu Cyperi Rhizoma to move qi results in Lianfu Liuhe Tang.] For Re Liuhe Tang (Heat Six-Harmony Decoction) treating reckless bleeding due to blood heat, add Zhizi Gardeniae Fructus and Huanglian Coptidis Rhizoma. For Han Liuhe Tang (Cold Six-Harmony Decoction) treating deficiency-cold of the Blood Sea, add Paojiang Prepared Zingiberis Rhizoma and Fuzi Aconiti Lateralis Radix Preparata. For Qi Liuhe Tang (Qi Six-Harmony Decoction) treating qi stagnation obstructing menstruation, add Chenpi Tangerine Peel and Houpo Magnoliae Bark. For Feng Liuhe Tang (Wind Six-Harmony Decoction) treating wind spasms due to blood deficiency, add Qinjiao Gentianae Macrophyllae Radix and Qianghuo Notopterygii Rhizoma et Radix. These are all general formulas for menstrual and obstetric conditions; I hope contemporary physicians will examine and evaluate them carefully.
