The *Daquan* (Compendium of Great Importance) states: After childbirth, when lochia (postpartum vaginal discharge) has just begun to flow, if it suddenly stops, accompanied by a heavy pain in the waist radiating downward and stabbing pain in both thighs as if driven into the bone. This is caused by blood stasis in the meridians; if not unblocked immediately, it will inevitably develop into carbuncles and sores. Tao Ren Tang (Peach Kernel Decoction) and Wuxiang Lianqiao Tang (Five-Aromatic Fritillaria Decoction) are suitable.
Shen Yaofeng said: The aforementioned formulas are not stable enough; it is more appropriate to use Tao Ren (Peach Seed), Hong Hua (Safflower), Di Long (Earthworm), Rou Gui (Cinnamon Bark), Moyao (Myrrh), and Dang Gui (Angelica Sinensis).
Rushen Tang (Miraculous Efficacy Decoction) treats lumbago caused by blood stasis. Yanhusuo (Corydalis Rhizome) and Dang Gui (Angelica Sinensis) in equal parts, decocted in water and taken orally.
Shen Yaofeng said: Lumbago without the aforementioned symptoms mostly belongs to deficiency of the Liver and Kidney. Drugs such as Dang Gui (Angelica Sinensis), Duzhong (Eucommia Bark), and Buguzhi (Astragalus Complanatus Fruit) are suitable.
Xue Lizhai said: If pressing with the hand aggravates the pain, it is blood stasis; if pressing relieves the pain, it is blood deficiency.
Shen Yaofeng said: For postpartum edema, one must first distinguish between Water Disease (fluid retention) and Qi Disease (qi stagnation). In Water Disease, the skin is thin, white, and shiny, resembling a state of being wrapped in water; in Qi Disease, the skin is thick and the color remains unchanged. The Classics state: The Kidney is the gate of the Stomach. If the gate is not functioning properly, fluid accumulation occurs leading to disease. Generally, after childbirth, Kidney Qi is inevitably damaged, and the Yang energy at the bottom of the Stomach is weak, unable to vaporize and distribute body fluids or regulate the water passages. This is the cause of fluid accumulation. Shengji Wan/Tang (Kidney Qi Pill/Decoction) is suitable. The presentation of thin, white skin corroborates this. The Ying (Nutritive) and Wei (Defensive) Qi of the human body are peaceful when flowing smoothly and become distended when stagnant. Stubborn phlegm and blood stasis can both block the qi pathways causing swelling. For presentations with thick skin and unchanged color, a wiry pulse indicates Phlegm; a knotted and choppy pulse indicates a Blood pattern; these should be treated separately according to their respective discussions and medications. There is another type where Qi stagnation results from Blood deficiency; the swelling is not severe, and the color is pale yellow. It is suitable to use Dang Gui Shen (Angelica Sinensis Root Head) as the sovereign drug, assisted by Baizhu (Atractylodes Macrocephala Rhizome), Chenpi (Citrus Reticulata Peel), and Fuling (Poria) among others.
Shen Yaofeng said: A woman at seven or eight months of pregnancy suffered from incessant coughing, sometimes vomiting several bowls of thick sputum. Erchen Tang (Two-Cured Decoction) and Xuanfu Decoction were ineffective. Using methods to clear the Lung and nourish Yin worsened the condition, leading her to stop taking medicine. Upon full-term delivery, the coughing persisted, preventing sleep day and night. Three days postpartum, Erchen Tang was modified with Danxing (Arisaema Cum Bile) and Zhuli (Bamboo Juice); she vomited several bowls of thick sputum, but the cough continued. Then, Erchen Tang was modified with Xuanfu (Inula Flower) and Dang Gui (Angelica Sinensis); symptoms decreased slightly, allowing for some food intake. Because the cough did not subside and the sputum gradually became thinner, Si Qian Shengdi (four qian of Rehmannia Glutinososa Root) was added. Immediately, appetite decreased sharply, and the cough intensified, accompanied by sweating over the whole body and a slightly wiry pulse. Guishen San Qian (three qian of Angelica Sinensis Root Head), Fuling Er Qian (two qian of Poria), Chao Gancao Yi Qian (one qian of stir-fried Licorice Root), and Zhi Shiying San Qian (three qian of Hematite) were used. Because of sweating, Huangqi (Astragalus Root) was considered; because of the persistent cough, after much deliberation, Chao Huangqi San Qian (three qian of stir-fried Astragalus Root) was ultimately retained. After one dose, sweating stopped, and the cough significantly decreased; after ten doses, she recovered.
