The five zang organs and six fu organs correspond to the Five Movements (Wu Yun) and Six Qi (Liu Qi). When a disease is mild, it manifests as its own original transformation (the inherent physiological nature of the organ or fu); when severe, it involves the transformation of its "ghostly enemy" (an abnormal excess in the controlling relationship). Extremes lead to harm; only through control can balance be maintained.
When mild, it is the original transformation of the respective meridian and fu organ, representing the transformation of natural creation. When severe, it is like Metal being too weak to control Wood; Wood then generates Fire. Both Wood and Fire belong to Yang, so Wind and Fire pathogens are prevalent. If transformations combine, Wind-Heat fights together, damaging Metal (the Lung).
The Qi of the zang and fu organs corresponds to the Three Yin and Three Yang. In excess conditions, the Qi of the original transformation circulates; after purgation, if the body's Qi declines, the Qi that overcomes it circulates. This type of purgation aims to eliminate the "ghostly enemy" Qi.
For example, when Yang heat accumulates, Liangge San (Cooling the Chest Powder) combined with Jiedu Tang (Detoxifying Decoction) is taken to dissipate extreme heat. To purge fire-heat, Jiedu Tang or Tiaowei Chengqi Tang (Regulate the Stomach and Direct Downward Decoction) are used. For emetic therapy, Erxian San (Two Immortals Powder) is employed. For regulating cold damage, Baihu Tang (White Tiger Decoction) and Liangge San are taken together to resolve the condition.
Within three days of cold damage onset, Shuangjie agents (Dual-resolving formulas) are administered continuously. If there is no sweating, it indicates pathogen transmission; waiting until the third or fourth day, Xiaochaihu Tang (Minor Bupleurum Decoction) is used to resolve interior-exterior heat, while Baihu Tang and Liangge San are taken intermittently. If latent heat persists, between the seventh and eighth days, if purgation is desired but exterior signs remain, Dachengqi Tang (Major Order the Qi Decoction) and Da Chaihu Tang (Major Bupleurum Decoction) are decocted together to attack the interior and release the exterior, which is most appropriate.
If internal heat is severe causing spontaneous diarrhea, only Liangge San is used, followed by Jiedu Tang and Baihu Tang. If nausea, dry retching, or vomiting occur, a large dose of Baihu Tang is prepared, mixed with five qian of Jiedu San powder; taking this stops symptoms immediately. For blockage of both urination and defecation accompanied by unbearable abdominal pain, 3 liang of Puxiao (Natrii Sulfas) is decocted in wine without ash and taken hot; this cures the condition, following the method of "treating according to the disease" (Cong Zhi).
For cold trapped within heat, after purgation, adding Muxiang (Aucklandiae Radix) to Dachengqi Tang for purgation is particularly effective.
If cold damage fails to receive timely purgation, and initially mistaken administration of Mahuang (Ephedrae Herba) and other hot agents was excessive, extreme heat will result. There may be deep Yang collapse with cold limbs and faint pulse, a syndrome where extreme Yang mimics Yin. Quack doctors often mistake this for a Yin syndrome. One must urgently rescue Yin by administering Baihu Tang and Liangge San, three doses daily. As the pulse gradually returns and the body warms, then Dachengqi Tang is used for purgation. Dachengqi Tang is a miraculous emergency formula. If there is regurgitation and obstruction in the throat and diaphragm, it should be taken hot to open heat knots; once unblocked, recovery follows.
After sweating or purging in cold damage, if spontaneous sweating and deficiency-heat persist, adding Renshen (Ginseng Radix et Rhizoma) and Cangzhu (Atractylodis Rhizoma) to Baihu Tang works miraculously in one dose, stopping sweat and cooling the body. This method is excellent and needs no further elaboration.
For red and white dysentery, first administer Huanglian Ejiao Wan (Coptis and Donkey-Hide Gelatin Pill), then Jiedu Tang.
Cold damage-induced sores, ulcers, and tetanus are treated similarly to cold damage, but Shuangjie agents, Baihu Tang, and Chengqi Tangs are used according to circumstances.
Shuangjie San, Liangge San, Baihu Tang, and Xiexin Tang are miraculous formulas for treating cold damage.
Pregnant women near term may take Yiyuan San (Benefit Origin Powder) to cool the fetus, continuing postpartum. If lochia is not fully expelled, Liangge San is decocted with Siwu Tang (Four Substances Decoction) to regulate menstrual blood. In severe cases, Dachengqi Tang is combined with Siwu Tang, representing purgation with tonification. Liangge San combined with Siwu Tang is called Yuzhu San (Jade Candle Powder), a miraculous formula for postpartum women. Liangge San, Siwu Tang, and Dachengqi Tang combined are called Sanhe Tang (Three Harmonies Decoction). Dachengqi Tang combined with Siwu Tang treats all blood accumulation and mass disorders in women; adding Honghua (Carthami Flos) is particularly effective.
Newborns aged five to seven days with heat signs, if unavoidable, should only be given Yiyuan San frequently via gavage. For infant night crying due to hunger, Liangge San is used for regulation, taken before sleep.
For all sores, ulcers, and urticaria, Liangge San added with Danggui (Angelicae Sinensis Radix) is used for treatment.
