Freshwater leeches (also known as *Ma Huang Qi*. Take several, place them in a bowl of water, and release them after use. In winter, they hibernate near stagnant water or field ditches; dig up the soil to retrieve them.)
Cut a piece of thin paper into a circle the size of the sore; observe how many pus heads are present, and cut small holes in the paper corresponding to each pus head. Place the paper over the sore. Position the freshwater leeches on the holes in the paper, using one leech per pus head, to first suck out the pus, which will immediately stop the pain. If there is no obvious pus head, press with fingers; the hottest spot indicates the lesion. Alternatively, mix tea powder with water and apply it to the sore; mark where it dries first. If indistinguishable, apply to the center. If there is only one pus head but a slightly larger area of redness, add more leeches closer to the center. Once the pus and blood are fully sucked out, pain subsides, and swelling recedes, apply a medicated plaster.
During the Shaoxing era of Emperor Gaozong's reign, Huang Wu, the Prefect of Tingzhou, developed a carbuncle between his shoulder and back. Medical treatments were ineffective, but treatment with freshwater leeches yielded immediate results. Because he slept under a slightly warm quilt, he woke up to find his clothes and bedding soaked. Upon inspection by candlelight, it was found that the wound had spontaneously bled. (This occurred because the leeches had sucked out all the foul blood; without the application of a plaster and due to body warmth, healthy blood flowed out along the path.) He urgently ordered lotus roots from the pond to be fetched, and mud from the lotus root joints was applied to the wound, stopping the bleeding immediately. According to the *Bencao* (Materia Medica), freshwater leeches attack and treat carbuncles and toxic swellings. Placing about ten leeches on the affected area until the skin wrinkles and the flesh turns white ensures recovery. In winter, when freshwater leeches are not available in water, they can be dug from the ground and kept in warm water to stimulate activity. First, wash away the saltiness from human skin, store the leeches in a bamboo tube, suspend them; within moments, they will bite and suck blood, detaching themselves when full. Use hungry leeches subsequently. (Zhang Zhongqin, the Transport Commissioner, recounted: In the Ren-Shen year of Shaoxing, while inspecting Tingzhou, I witnessed this personally.)
