Do not sit on the birthing mat too early, lest the fetus be unable to turn within the uterus, causing the amniotic sac to rupture prematurely, resulting in transverse or breech presentation, which is extremely dangerous. If hands or feet are seen protruding, immediately prick the palms and soles with fine needles to a depth of one or two fen, pricking three or four times, apply salt to the puncture sites, and gently push the limbs back. The infant will subsequently move due to pain and fright, thereby converting to a normal presentation. In cases where water breaks but delivery does not occur after a day, it is due to lack of regulation and negligence before term; this must be carefully differentiated: if the body feels hot yet cold, the face is black, the tongue is cyanotic and cold, both mother and child will die; if lips and mouth are cyanotic with foaming, both mother and child will die; if the face is red and the tongue cyanotic, the mother survives but the child dies; if the face is cyanotic and the tongue red with foaming from the mouth, the mother dies but the child survives. For retained placenta, prevent umbilical cord rupture; if the placenta ascends to cover the heart, death occurs instantly. Tie the umbilical cord tightly at both ends, cut it, and use an object to weigh it down so blood does not enter the uterus; the uterus will naturally shrink and descend. If the mother remains calm, even if delayed for several days, it will not harm her.
Tianma Wan (Houttuynia Cordata Pill): In early June, harvest Houttuynia Cordata (Yimucao) including roots, dry in shade, powder, and form pills with refined honey the size of a longan. Take at parturition warmed with neonatal urine or wine/water.
For severe lower back pain during labor, mix one egg white with the medicine, then dilute with ginger juice and take cold. This can immediately correct transverse or breech presentations, ensuring safety for mother and child.
Another Prescription:
For premature rupture of membranes and dried amniotic sac preventing delivery, mix one bowl of sesame oil and one bowl of honey. Slowly boil in a copper pot for one or two boils, skim off foam, mix in one liang of Talcum (Huashi), and take at once. Massage the mother's abdomen above and below the navel with the oil-honey mixture externally.
Huangjin San (Gold Leaf Powder): For difficult labor lasting one or two days, effective as magic. Use five large pieces or seven small pieces of real gold leaf. Place in a small porcelain cup with a little water, remove paper, add gold leaf, grind evenly with finger, add water to half full. Have one person support the laboring woman in a sitting position, another press the Liangjian acupoints (GB21) on both shoulders with the thumb, and take the previous medicine warm. The fetus will be delivered.
Duoming Wan (Life-Rescuing Pill): For miscarriage with excessive bleeding, fetal death in utero, aversion to cold, cyanosis of fingers, nails, lips, and tongue, or fetal qi rushing to the heart causing suffocation, cold sweat, ingestion of toxins or erroneous herbs causing continuous bleeding. It calms the fetus if alive, or expels dead fetus if deceased.
Peach Kernel (bran-fried, skin removed), Red Peony Root, Official Cinnamon, White Poria, Moutan Cortex, each 2 fen. Honey-pilled to the size of a bullet. Chew one pill thoroughly, swallow with light vinegar water. Take two pills consecutively; even if the fetus has decomposed in the uterus, it can be expelled immediately.
Tunaowan (Rabbit Brain Pill): Mother Clove, Frankincense, each 1 qian; Musk, 1 zi. Grind to ultra-fine powder.
In the twelfth lunar month, take rabbit brain (skin removed, ground to paste), mix with powder, form pills the size of corn kernels. Coat with 1 qian of Cinnabar. Dry in shade, seal in oiled paper. Take one pill per dose.
Single Prescription:
For labor lasting three to four days without delivery, immediate expulsion follows. Mix one zhong of honey, sesame oil, and good wine. Boil together. Laboring woman faces east and takes it.
Another Prescription:
Fish glue, 7 cun, burnt over sesame oil lamp, powdered, taken with wine.
Tuoyi San (Placenta-Expelling Powder): For retained placenta.
Talcum, 4 qian; Cyathula Root, Akebia Stem, each 3 qian; Angelica Sinensis, Malva Seed, Aurantium Fruit, each 2 qian.
Another Prescription:
For mothers with deficient Yuan Qi, use Ligusticum Wallichii and Angelica Sinensis tail, with doubled Cinnamon Twig for warming, leading to natural delivery.
Another Prescription:
Use seven Castor Seeds, remove shells, grind into paste, apply to soles. Wash off immediately after placenta delivery. If intestines prolapse due to non-washing, apply paste to vertex to retract them. Alternative: Laboring woman swallows hair until vomiting occurs, leading to expulsion.
Assisting Expulsion of Dead Fetus:
Official Cinnamon, 3 qian; Musk, 5 fen. Both powdered, mixed with warm wine. Expelled quickly like being pushed by hand.
Another Prescription:
For transverse/oblique presentation with fetal death in utero. Longmax Earth (Fulonggan) powder, 2 qian, mixed with warm wine. The fetus exits with earthy matter. Another Prescription: Pingwei San, decocted with wine and water (one sheng each) to 0.8 sheng, add Mirabilite, 5 qian, boil again. Feed when warm; the fetus dissolves into water.
Postpartum Care:
Men should not rejoice, women should not anger. Do not sleep immediately; have the patient sit with eyes closed for a moment, then lie on bed leaning back, supine with knees bent. Do not extend legs, do not sleep deeply. Frequently fumigate with vinegar smoke to prevent fainting. Gradually introduce white porridge. Do not force getting out of bed for five to seven days. Within one month, sexual misconduct causes difficult-to-treat complications. Strictly avoid. Subsequent exogenous/endogenous disorders are similar to men's but require adding blood-regulating herbs. Observe and adapt.
Six Types of Postpartum Fever:
Postpartum pulse should be empty. He Hanzhong said: Wavy and slippery is auspicious; solid, large, wiry, and rapid indicates death. Large pulse invading Cun position with rapid irregularity indicates death; deep, thin, adherent to bone indicates survival.
Postpartum fever arises from: incomplete lochia discharge, excessive blood loss, physical strain, dietary stagnation, wind-cold invasion, cold with food/qi stagnation, or milk engorgement after three days. All present with body and abdominal pain. Treatment focuses on regulating blood. Do not abruptly use Ginseng, lightly use Peony, induce sweating, or use bitter-cold herbs.
For incomplete lochia: fever, aversion to cold, hypochondriac distension, masses in upper/lower abdomen causing pain. Use Siwu Tang plus Psoralea Seed, Moutan Cortex, Peach Kernel, Safflower, Corydalis Rhizome, Cyperus Rhizome, Tangerine Peel, Dried Ginger, Official Cinnamon. Decoct with wine and water (one zhong each), a pinch of Black Soybean, reduce to 0.8 zhong. Grind Costus Root, add neonatal urine and ginger juice. Best to expel foul matter. Heishen San is also excellent, followed by Bawu Tang plus Dried Ginger, Tangerine Peel, Cyperus Rhizome (fried in neonatal urine) for regulation.
For excessive blood loss: fever, empty-large powerless pulse, no internal pain. Indicates Yin deficiency with Yang floating. Siwu Tang minus Peony, add Atractylodes Macrocephala, Poria to leach out heat. If high fever persists, add Charred Ginger (effective) or Xionggui Tiaoxue Yin. Physical strain treated similarly.
For dietary injury: fever, headache, belching with sour taste, aversion to food odor, tastelessness, chest fullness, tight-full Chi Kou pulse. Treat Middle Jiao.
Exogenous Wind-Cold: Pulse floating and slight, or spontaneous sweating, covered by wind. Use Xiongzhi Xiangsu San plus Notopterygium Root, Saposhnikovia Root. Pulse wiry and suspended, or lochia not stopping, indicates Cold invasion. Use Wuji San. Pulse floating and tight, Wind-Cold dual invasion. Use Wuji Jiajia San. If sweating, remove Ephedra, add Ginseng.
Cold with Food/Qi Stagnation: Renying and Qikou pulses both tight-full. Use Xingqi Xiangsan San.
Milk Engorgement: Breast swelling, hardness, pain. Have mother massage breasts to promote flow. Or decoct Akebia Stem, Tetrapanax Pith, Pig Trotters. For those without children, use slightly.
Generally, postpartum illness prioritizes securing Blood and Qi; other ailments are treated secondarily.
Eight Postpartum Miscellaneous Disorders:
Postpartum Blood Vertigo: Though categorized as Qi exhaustion, Excess Blood, or Deficient Blood, generally caused by deficient fire drawing blood upward. Cut Chive leaves finely, place in a spouted bottle, pour hot vinegar, seal quickly, insert spout into mother's nose to stop vertigo. Another prescription: Burn Deer Antler to ash, remove fire toxicity, grind ultra-fine, mix with neonatal urine and good wine, drink to wake.
Another Prescription:
Myrrh and Hematite in equal parts, named Dueming San. Decoct half sheng neonatal urine and half sheng good wine for one or two boils, mix 2 qian, take. Repeat later. Also treats madness from blood entering Heart Meridian.
Postpartum Facial Paralysis/Jaw Lock: Due to Blood deficiency allowing Wind to enter the mouth. Opisthotonos indicates Body deficiency allowing Wind to enter Yang meridians, causing back spasms. Siwu Tang plus Gentian Macrophylla Root, Notopterygium Root, Schizonepeta Spike (lightly fried to powder), 3 qian per dose, taken with Black Soybean soaked in wine. Another prescription: Angelica Sinensis, Schizonepeta Spike, each 1.5 qian, one sheng water, little wine, decoct to 0.7 sheng, administer. Effective upon swallowing. □□ may take Xiao Xuming Tang.
Postpartum Edema: Requires major tonification of Qi and Blood, with minor addition of Atractylodes Lancea, Poria. Water will naturally drain.
Postpartum Metrorrhagia: Ruilian, 1 liang (burnt to ash retaining nature), Palm Ash, Angelica Sinensis, each 1 liang; Official Cinnamon, 5 qian; Areca Nut, 2 pieces; Carp Scales (fried).
Postpartum Aphasia: Due to敗血 (foul blood) confusing the Heart. Siwu Tang plus Cinnabar, Safflower, Acorus Tatarinowii Rhizome, Ginseng.
