(The treatment for warm-heat disease, winter-warm disease, warm-toxin disease, and warm-epidemic disease is the same.)
When warm-heat disease lingers in the Yangming stage for a long time, whether purgatives have already been used or not, if symptoms such as fever, facial redness, dry mouth and tongue, and even black teeth and cracked lips appear, this indicates that the pathogenic heat is gradually damaging the Kidney Water of Shaoyin. If the pulse is deep and solid, purgatives may still be used. If the pulse is deficient and large, and the heat in the palms and soles is more severe than in the dorsum of hands and feet, it indicates that there is less pathogenic heat and more deficiency heat; Jiajian Fumai Decoction (Modified Pulse-Recovering Decoction) should be administered as the primary treatment. If warm-heat disease is mistakenly treated with diaphoretic methods, leading to palpitations, stiff tongue, and coma, it signifies damage to Heart Qi and depletion of body fluids. Fumai Decoction (Pulse-Recovering Decoction) is appropriate. (Recovery of saliva on the tongue indicates a favorable prognosis.)
If deafness occurs after six or seven days of warm-heat disease, it is due to blazing Yin fire within, indicating the disease is in Shaoyin. Fumai Decoction (Pulse-Recovering Decoction) is suitable. (Mistaking this for a Shaoyang syndrome and administering Chaihu Tang [Bupleurum Decoction] will inevitably lead to death.)
For internal injury due to overexertion combined with warm-heat disease that remains unresolved after six or seven days, Fumai Decoction (Pulse-Recovering Decoction) is indicated. If there is no fever but extreme fatigue, add Ren Shen (Ginseng).
If sweat has been induced without perspiration, or purgatives have been used without reduction of fever, and after six or seven days the pulse remains rapid and vigorous, this represents the struggle between pathogenic factors and healthy qi. Heavy doses of Fumai Decoction (Pulse-Recovering Decoction) should be given.
If warm-heat disease is mistakenly treated with ascending and dispersing agents, resulting in knotted and intermittent pulses, or even one beat per two breaths, the method requires urgent rescue of the interior, known as "preserving life before treating disease." Heavy doses of Fumai Decoction (Pulse-Recovering Decoction) should be used. After sweating and purging in warm-heat disease, if dry mouth and throat, mental fatigue, drowsiness, red tongue, and old coating appear, it indicates deficiency of Shaoyin fluids. Administer Fumai Decoction (Pulse-Recovering Decoction).
Jiajian Fumai Decoction (Modified Pulse-Recovering Decoction) [Based on modification of classic formulas]:
Zhi Gan Cao (Honey-fried Licorice Root, 3 qian), Gan Di Huang (Prepared Rehmannia Root, 3 qian), Sheng Bai Shao (Raw White Peony Root, 3 qian), Mai Dong (Ophiopogon Tuber, 2.5 qian, retaining the heart), E Jiao (Donkey-hide Gelatin, 1.5 qian), Ma Ren (Hemp Seed, 1.5 qian).
Decoct in water and take orally. For severe cases, increase Zhi Gan Cao (Licorice Root) to 5 qian, Gan Di Huang (Rehmannia Root) and Bai Shao (White Peony Root) to 4 qian each, and Mai Dong (Ophiopogon Tuber) to 3.5 qian.
If warm-heat disease is mistakenly treated with diaphoretics, leading to stiff tongue, coma, spontaneous sweating, and loss of mental control, it signifies severe damage to Heart Qi and body fluids. Jiuniu Tang (Rescue Counteraction Decoction) is the primary treatment.
Jiuniu Tang (Rescue Counteraction Decoction):
Take Jiajian Fumai Decoction (Modified Pulse-Recovering Decoction), remove Ma Ren (Hemp Seed), and add Sheng Long Gu (Raw Dragon Bone, 2 qian) and Sheng Mu Li (Raw Oyster Shell, 4 qian). If the pulse is deficient, large, and dissipating, add Ren Shen (Ginseng, 1 qian). If diarrhea is severe after purgation, occurring three to four times daily, with a rapid pulse, it indicates inherent deficiency of True Yang with uncleared interior heat. Yi Jia Jian (Single Shell Decoction) is the primary treatment. After one or two days of administration, if stool normalizes, Yi Jia Fumai Tang (Single Shell Pulse-Recovering Decoction) may be given.
In lower-jiao warm-heat disease, whenever loose stools occur, immediately administer Yi Jia Fumai Tang (Single Shell Pulse-Recovering Decoction) [This formula belongs to yin-nourishing formulas, containing the intent of securing yin].
Yi Jia Jian (Single Shell Decoction):
Sheng Mu Li (Raw Oyster Shell, 1 liang, finely ground).
Decoct in water and take warm.
Yi Jia Fumai Tang (Single Shell Pulse-Recovering Decoction):
Take Jiajian Fumai Decoction (Modified Pulse-Recovering Decoction), remove Ma Ren (Hemp Seed), and add Mu Li (Oyster Shell, 5 qian).
When pathogenic heat penetrates deeply into the lower jiao, presenting with deep and rapid pulse, dry tongue, black teeth, and twitching fingers, it indicates deficiency of True Water failing to nourish Wood. Urgently prevent spasms and convulsions. Er Xiang Fumai Tang (Two Shells Pulse-Recovering Decoction) is the primary treatment.
Er Xiang Fumai Tang (Two Shells Pulse-Recovering Decoction):
Take Jiajian Fumai Decoction (Modified Pulse-Recovering Decoction), add Sheng Mu Li (Raw Oyster Shell, 2.5 qian) and Sheng Bie Jia (Raw Softshell Turtle Shell, 4 qian).
In lower-jiao warm-heat disease, if heat is deep with severe cold extremities, pulse is fine and hurried, with severe palpitations or even chest pain, it indicates Kidney deficiency failing to assist Liver Wood, rampant Liver Wind, and loss of the Heart Master's support. San Jia Fumai Tang (Three Shells Pulse-Recovering Decoction) is the primary treatment.
San Jia Fumai Tang (Three Shells Pulse-Recovering Decoction):
Take Er Xiang Fumai Tang (Two Shells Pulse-Recovering Decoction), add Sheng Gui Ban (Raw Tortoise Plastron, 5 qian).
In Shaoyin warm-heat disease, if there is irritability and inability to sleep, it indicates exhaustion of True Yin and resurgence of vigorous fire. Huanglian Ejiao Tang (Coptis and Donkey-hide Gelatin Decoction) is the primary treatment.
Huanglian Ejiao Tang (Coptis and Donkey-hide Gelatin Decoction):
Huang Lian (Coptis Rhizome, 4 qian), Huang Qin (Scutellaria Root, 1 qian), E Jiao (Donkey-hide Gelatin, 3 qian), Bai Shao (White Peony Root, 1 qian), Ji Zi Huang (Egg Yolk, 2 pieces).
Use eight cups of water. First decoct Huang Qin (Scutellaria Root), Huang Lian (Coptis Rhizome), and Bai Shao (White Peony Root) to obtain three cups, then strain. Dissolve E Jiao (Donkey-hide Gelatin) in the liquid. Add Ji Zi Huang (Egg Yolk), stir well, and divide into three doses.
Night fever and morning coolness, with no sweat upon fever resolution, indicate pathogenic heat deeply hidden in the Yin division, arising from Yin. Qinghao Biejia Tang (Artemisia Annuae and Softshell Turtle Decoction) is the primary treatment.
Qinghao Biejia Tang (Artemisia Annuae and Softshell Turtle Decoction):
Qing Hao (Artemisia Annua Herb, 2 qian), Bie Jia (Softshell Turtle Shell, 5 qian), Xi Sheng Di (Fine Prepared Rehmannia Root, 4 qian), Zhi Mu (Anemarrhena Rhizome, 2 qian), Dan Pi (Moutan Cortex, 3 qian).
Decoct in water and take orally.
Both cold extremities and hiccups (commonly known as hiccupping), with a fine and forceful pulse, indicate warm pathogenic factors occupying the lower jiao for a long time, consuming Liver fluids, and disturbing the Chong Mai (Penetrating Vessel). Xiao Ding Feng Zhu (Small Calming Wind Pearl) is the primary treatment.
Xiao Ding Feng Zhu (Small Calming Wind Pearl):
Ji Zi Huang (Egg Yolk, 1 piece, raw), Zhen E Jiao (Authentic Donkey-hide Gelatin, 2 qian), Sheng Gui Ban (Raw Tortoise Plastron, 6 qian), Tong Bian (Child's Urine, 1 cup), Dan Cai (Edible Mussel, 3 qian).
Use five cups of water. First decoct Sheng Gui Ban (Raw Tortoise Plastron) and Dan Cai (Edible Mussel) to obtain two cups, then strain. Add Zhen E Jiao (Authentic Donkey-hide Gelatin) and melt over fire. Add Ji Zi Huang (Egg Yolk), stir well, then mix with Tong Bian (Child's Urine) and take in one dose.
In lower-jiao warm-heat disease, if there is mental fatigue, convulsions, crimson tongue with scant coating, weak pulse energy, and frequent sensations of collapse, it indicates prolonged pathogenic heat consuming True Yin. Da Ding Feng Zhu (Great Calming Wind Pearl) is the primary treatment. (This condition often results from mistaken diaphoresis or purgation.)
Da Ding Feng Zhu (Great Calming Wind Pearl):
Sheng Bai Shao (Raw White Peony Root, 6 qian), E Jiao (Donkey-hide Gelatin, 3 qian), Sheng Gui Ban (Raw Tortoise Plastron, 4 qian), Gan Di Huang (Prepared Rehmannia Root, 6 qian), Ma Ren (Hemp Seed, 2 qian), Wu Wei Zi (Schisandra Berry, 2 qian), Sheng Mu Li (Raw Oyster Shell, 4 qian), Mai Dong (Ophiopogon Tuber, 6 qian, retaining the heart), Zhi Gan Cao (Honey-fried Licorice Root, 4 qian), Ji Zi Huang (Egg Yolk, 2 pieces, raw), Bie Jia (Softshell Turtle Shell, 4 qian, raw).
Use eight cups of water, decoct to three cups, strain. Add Ji Zi Huang (Egg Yolk), stir well, and divide into three doses. For panting, add Ren Shen (Ginseng). For spontaneous sweating, add Long Gu (Dragon Bone), Ren Shen (Ginseng), and Xiao Mai (Wheat). For palpitations, add Fu Shen (Poria with Hostwood), Ren Shen (Ginseng), and Xiao Mai (Wheat).
Whenever vigorous fire is still abundant, Ding Feng Zhu (Calming Wind Pearls) and Fumai Tang (Pulse-Recovering Decoctions) must not be used. When pathogenic factors are few and deficiency is predominant, Huanglian Ejiao Tang (Coptis and Donkey-hide Gelatin Decoction) must not be used. When yin deficiency leads to impending spasms, Qinghao Biejia Tang (Artemisia Annuae and Softshell Turtle Decoction) must not be used.
In lower-jiao warm-heat disease, if spasms, convulsions, coma, short tongue, irritability occur, along with large cun pulse, heavy breath, red eyes, malar flush, and high fever, it indicates uncleared pathogenic heat in the Hand-Shaoyin (Heart) channel. First administer Niuhuang Wan (Calculus Bovis Pill) or Zixue Dan (Purple Snow Elixir) to open orifices and search out pathogens. Then use Fumai Tang (Pulse-Recovering Decoction) to preserve yin, and San Jia (Three Shells) to anchor yang. Consult carefully during clinical practice to avoid confusion.
Niuhuang Wan (Calculus Bovis Pill), Zixue Dan (Purple Snow Elixir) [See previous], Fumai Tang (Pulse-Recovering Decoction) [See previous], San Jia Fumai Tang (Three Shells Pulse-Recovering Decoction) [See previous].
When warm pathogenic factors linger for a long time, causing skin roughness like fish scales, it may be because purgation cleared the interior and pathogens are about to溃散 (disperse); or because yin-preserving medicines were taken, increasing fluids and inducing sweat, but healthy qi is too weak to expel them immediately. The struggle between yin and yang causes shivering, indicating an impending battle-sweat. Administer Fumai Tang (Pulse-Recovering Decoction) hot. For severe deficiency, add Ren Shen (Ginseng) to assist sweat expulsion. If muscles are still full and skin is not rough, allow sweat to occur naturally without disturbance.
Frequent rinsing of the mouth without swallowing, and black but dry-free stools, indicate yin deficiency with blood stasis. Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) is the primary treatment.
Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction)
Night fever and daytime coolness, hardness and fullness in the lower abdomen, free urination, constipation, and deep solid pulse indicate excess pathogens and blood accumulation. Taoren Chengqi Tang (Peach Kernel and Magnolia Bark Decoction) is the primary treatment.
Taoren Chengqi Tang (Peach Kernel and Magnolia Bark Decoction):
Da Huang (Rhubarb Root, 2.5 qian), Mang Xiao (Mirabilite, 1 qian), Tao Ren (Peach Kernel, 1.5 qian), Dang Gui (Angelica Sinensis Root, 1.5 qian), Shao Yao (Peony Root, 1.5 qian), Dan Pi (Moutan Cortex, 1.5 qian).
Decoct in water and take orally.
After taking clearing and resolving medicines for warm-heat disease, if the pulse becomes soggy and small, with watery diarrhea or pus-and-blood stools, it indicates heat has retreated leaving interior deficiency. Taohua Tang (Peach Blossom Decoction) is the primary treatment.
Taohua Tang (Peach Blossom Decoction):
Chishi Zhi (Red Halloysite, 3 qian: 2 qian whole for decoction, 1 qian powdered for mixing), Pao Jiang (Processed Ginger, 1.5 qian), Bai Jing Mi (White Glutinous Rice, 3 qian). Use three cups of water, decoct to one and a half cups, strain, add Chishi Zhi powder, and take warm. For deficiency, add Ren Shen (Ginseng).
After seven or eight days of warm-heat disease, if there is fever, crimson tongue with scant coating, frequent diarrhea (dozens of times daily) with undigested food, and deficient rapid pulse, it indicates interior pathogens are exhausted and Spleen Yang has sunk. Taohua Zhou (Peach Blossom Porridge) is the primary treatment.
Taohua Zhou (Peach Blossom Porridge):
Ren Shen (Ginseng, 3 qian), Zhi Gan Cao (Honey-fried Licorice Root, 3 qian), Chishi Zhi (Red Halloysite, 6 qian, finely powdered), Bai Jing Mi (White Glutinous Rice, 2 he). Use ten cups of water, first decoct Ren Shen (Ginseng) and Zhi Gan Cao (Licorice Root) to obtain six cups, strain. Add Bai Jing Mi (Glutinous Rice) and cook to obtain three cups. Take one cup mixed with 2 qian of Chishi Zhi powder in one dose. Stop subsequent doses if diarrhea ceases. If it continues, take the second cup, mixing 2 qian of Chishi Zhi powder each time. If caused by prior overuse of cold-cooling herbs, resulting in non-rapid pulse and no fever, add Gan Jiang (Dried Ginger, 3 qian).
In women with warm-heat disease, if menstruation coincides with onset, presenting with rapid pulse, fever, worse heat at night, or delirium, it is Heat Entering the Blood Chamber (Xueshi). The method should be acrid-cool to reduce fever, combined with clearing the blood level. Qingre Yangrong Tang (Clear Heat and Nourish Ying Decoction) is the primary treatment.
Qingre Yangrong Tang (Clear Heat and Nourish Ying Decoction):
Qing Hao (Artemisia Annua Herb, 2 qian), Sang Ye (Mulberry Leaf, 1.5 qian), Zhi Mu (Anemarrhena Rhizome, 1 qian), Bie Jia (Softshell Turtle Shell, 3 qian), Chi Shao (Red Peony Root, 1.5 qian), Sheng Di (Prepared Rehmannia Root, 2 qian), Dan Pi (Moutan Cortex, 1.5 qian), Dan Shen (Salvia Miltiorrhiza Root, 2 qian).
Decoct in water and take orally. If the patient is robust and symptoms persist, add Chai Hu (Bupleurum Root, 1.5 qian).
In women with warm-heat disease, if menstruation coincides with onset, presenting with rapid pulse, deafness, dry retching, irritability, thirst, unresolved for over ten days, and heat sinking inward causing spasms, it is a combined disease of Qi and Blood. Zhuye Yunü Jian (Bamboo Leaf Young Woman Decoction) is the primary treatment.
Zhuye Yunü Jian (Bamboo Leaf Young Woman Decoction):
Sheng Shi Gao (Raw Gypsum Fibrosum, 3 qian), Gan Di Huang (Prepared Rehmannia Root, 2 qian), Mai Dong (Ophiopogon Tuber, 2 qian), Zhi Mu (Anemarrhena Rhizome, 1 qian), Niu Xi (Achyranthes Root, 1 qian), Zhu Ye (Bamboo Leaf, 1.5 qian).
Use four cups of water, first decoct Sheng Shi Gao (Gypsum) and Gan Di Huang (Rehmannia) to obtain two cups. Add the remaining four herbs and decoct to one cup. Take warm.
For Heat Entering the Blood Chamber, after taking medicines clearing both Qi and Blood levels, if half the pathogens are gone but residual pathogens remain with a still rapid pulse, Huyang Heyin Tang (Protect Yang and Harmonize Yin Decoction) is the primary treatment.
Huyang Heyin Tang (Protect Yang and Harmonize Yin Decoction):
Bai Shao (White Peony Root, 2.5 qian), Zhi Gan Cao (Honey-fried Licorice Root, 1 qian), Ren Shen (Ginseng, 1 qian), Mai Dong (Ophiopogon Tuber, 1 qian, fried with heart), Sheng Di (Prepared Rehmannia Root, 1.5 qian, fried).
Decoct in water and take orally.
For Heat Entering the Blood Chamber, if eight or nine-tenths of pathogens are gone, with slight evening chills and fever, and a deficient rapid right pulse, it indicates few pathogens and predominant deficiency, a combined disease of Qi and Blood. Jiashen Fumai Tang (Ginseng-Added Pulse-Recovering Decoction) is the primary treatment.
Jiashen Fumai Tang (Ginseng-Added Pulse-Recovering Decoction):
Take Fumai Tang (Pulse-Recovering Decoction) and add Ren Shen (Ginseng, 1.5 qian).
In women with febrile disease, if menstruation coincides with onset, unresolved for over ten days, with flaccid tongue, desire for cold drinks, irritability, fluctuating mental clarity, and a long right pulse with sunken left pulse, it indicates stasis-heat in the interior with blood accumulation. Jiajian Taoren Chengqi Tang (Modified Peach Kernel and Magnolia Bark Decoction) is the primary treatment.
Jiajian Taoren Chengqi Tang (Modified Peach Kernel and Magnolia Bark Decoction):
Da Huang (Rhubarb Root, 1.5 qian, processed), Tao Ren (Peach Kernel, 1.5 qian, fried), Xi Sheng Di (Fine Prepared Rehmannia Root, 3 qian), Dan Pi (Moutan Cortex, 2 qian), Ze Lan (Lygodium Spore, 1 qian), Renzhongbai (Human Ash, 1 qian).
Decoct in water and take orally.
