After the ulceration of sores and carbuncles, if there is insomnia accompanied by fever and irritability, this is due to Blood deficiency. Shengyu Tang (Cure for Deficiency Decoction) may be used. If spontaneous sweating persists with insomnia, this is due to Qi deficiency. Six Gentlemen Decoction (Sijunzi Tang) added with Astragali Radix (Huangqi) and Schisandrae Fructus (Wuweizi) may be used. If fever, irritability, muscle twitching, and tendon tremors occur, this is due to Blood deficiency; Bahuang Tang (Eight Treasures Decoction) may be used. If there is extreme thirst, flushed face, and a flooding and floating pulse, this is Yin deficiency generating internal heat; Danggui Buxue Tang (Donggui Tonifying Blood Decoction) may be used. If there is slight heat in the limbs, irritability, flushed face, and a deep and minute pulse, this is agitation caused by excess Yin; Six Gentlemen Decoction (Sijunzi Tang) added with Zingiberis Rhizoma Recens (Shengjiang) and Aconiti Lateralis Radix Praeparata (Fuzi) may be used.
Bleeding from sores and carbuncles occurs because the Qi of the Five Viscera is depleted, causing deficient fire to stir and blood to flow recklessly outside its vessels. One must identify the affected meridian, examine the cause, and treat accordingly. If liver heat causes reckless bleeding, Four Substances Decoction (Siwu Tang) added with Gardeniae Fructus Preparatus (Chao Zhizi), Moutan Cortex (Danpi), Poria (Fuling), and Atractylodis Macrocephalae Rhizoma (Baizhu) may be used. If liver deficiency fails to store blood, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) may be used. If heart deficiency fails to generate blood, Four Substances Decoction (Siwu Tang) added with Coptidis Rhizoma Preparatum (Chao Lian), Moutan Cortex (Danpi), Poria (Fuling), and Atractylodis Macrocephalae Rhizoma (Baizhu) may be used. If spleen deficiency with heat fails to control blood, Six Gentlemen Decoction (Sijunzi Tang) added with Gardeniae Fructus Preparatus (Chao Zhizi) and Moutan Cortex (Danpi) may be used. If there is stagnation in the Spleen Meridian, Guipi Tang (Restore the Spleen Decoction) added with Schisandrae Fructus (Wuweizi) may be used. If there is Qi deficiency of the Spleen and Lung, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Schisandrae Fructus (Wuweizi) may be used. If both Qi and Blood are deficient, Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction) may be used. If deficient fire stirs, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) added with Schisandrae Fructus (Wuweizi) may be used. Generally, when excessive blood loss presents with symptoms such as vexation, heat, and thirst, regardless of the pulse, one should urgently use Danshen Tang (Single Ginseng Decoction) to tonify Qi. The Classics state: "Blood is born from Qi." Unless sweet-warm formulas containing Ginseng Radix et Rhizoma (Renshen), Astragali Radix (Huangqi), Angelicae Sinensis Radix (Danggui), and Atractylodis Macrocephalae Rhizoma (Baizhu) are used to generate the blood of the Heart and Liver, recovery is impossible. If there is fever with a large pulse, the condition is incurable. For all blood disorders, Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction) should be the primary formula. Li Dongyuan stated: "If both constitutional Qi and pathogenic Qi are sufficient, purgation is indicated rather than tonification; if both constitutional Qi and pathogenic Qi are insufficient, tonification is indicated rather than purgation."
Generally, after the ulceration of carbuncles, tendons and flesh rot, and pus and blood are massively discharged. Yang dissipates along with Yin. This may result in tendon constriction and urgency, aversion to cold, and tremors or convulsions. In severe cases, it presents with a stiff tongue, locked jaw, opisthotonos (arching of the back and neck), abundant phlegm and saliva, constipation with sweating, and intermittent fever. This is a transmission of disease due to dual deficiency of Qi and Blood. Although this resembles wind-stroke (tetanus), the primary treatment method should be massive tonification of Qi and Blood. Even if it is indeed a wind disorder, the foundation must still be massive tonification of Qi and Blood, supplemented by herbs that dispel wind. If one fails to distinguish the nature of the disease and prescribes indiscriminately, errors will occur. Gu Shicheng said: "For men, women, and children, whether the lesion is on the forehead, skull, eyebrows, fontanelle, ears, or neck, and whether it is a sore or a boil, after ulceration, ointments must be applied to seal the wound. Exposure to wind and cold must be avoided. If carelessness occurs and swelling develops on the head and face, Hongshengdan Gao (Red Ascendant Paste) should be applied externally to draw out toxins, while Jingjie (Schizonepetae Herba), Fangfeng (Saposhnikoviae Radix), Jiangcan (Bombyx Batryticatus), Tianma (Gastrodiae Rhizoma), and Baizhi (Angelicae Dahuricae Radix) should be taken internally to disperse the pathogen, leading to natural recovery."
