[Symptoms of Accumulated Heat and Soreness] The four limbs are vexed and painful, sometimes feeling heavy and stagnant. The palms of the hands and soles of the feet may feel cold or hot at different times. There may be fever resembling malaria (Malaria-like fever), with periods of mental clarity alternating with fatigue. The body may feel heavy as if bearing a heavy load. Urine is yellow and red. Stool alternates between difficulty and ease. These are the symptoms of accumulated heat and soreness.
[Etiology of Accumulated Heat and Soreness] This condition arises from the accumulation of heat due to a diet rich in fatty and sweet foods, where damp-heat accumulates severely within the intestines and stomach. It can also result from sexual intercourse after drinking alcohol, causing heat qi to gather in the Spleen and fail to dissipate. Alternatively, excessive consumption of tea and soups leads to long-term accumulation of water-dampness internally, thereby forming the syndrome of accumulated heat and soreness.
[Pulse of Accumulated Heat and Soreness] The pulse is predominantly wiry and rapid (Wiry pulse indicates Spleen disease; Rapid pulse indicates Spleen heat). A wiry and rapid pulse at the Right Guan position indicates accumulated heat in the intestines and stomach. A wiry and rapid pulse at the Left [position]... (Text incomplete).
[Treatment of Accumulated Heat and Soreness] For accumulated heat from rich food, use Zhilian Erchen Tang (Gardenia and Coptis Two-Chen Decoction) or Qingwei Tang (Stomach-Clearing Decoction). For heat qi gathering in the Spleen, use Zhilian Wuji Tang (Gardenia and Coptis Wu-Ji Decoction). For Wood-Fire invading the Spleen, use Longdan Xiegan Tang (Gentian Liver-Draining Decoction).

Soreness, flaccidity, and spasms involve the sinews and vessels. Medical texts attribute this to the Liver Meridian, but upon detailed analysis of the condition, it is often primarily related to the Yangming Meridian. This is because the Yangming Meridian is the convergence of all sinews, governing the binding of bones and the mobility of joints. Furthermore, the Yangming Meridian is the Sea of Water and Grain, where damp-heat originates. When damp-heat resides in the meridians without transforming into dryness-heat, the nature of dampness is soft, resulting in soreness. When damp-heat transforms into dryness-heat, the nature of dryness is rigid, damaging the blood vessels and causing stiff sinews and strong pulses. Although these two conditions share similar onset, their progression differs, necessitating different treatments. To treat flaccidity and spasms, one uses methods to moisten and clear the sinews. To treat soreness, one primarily uses methods to clear dryness from the sinews. The author's Pingwei San (Pingwei Powder), Cangdu Sanmiao Wan (Cangdu Sanmiao Pill), combined with methods from the Shenghui Fang (Shenghui Formulas), constitute the major principles for treating soreness.
