Sha (Toxic Heat Pathogen) and miscellaneous diseases often occur simultaneously. In treatment, priority must be given to treating Sha, while concurrently addressing the miscellaneous disease, because Sha is acute while miscellaneous diseases are chronic. Only during pregnancy and postpartum should Sha be treated with equal emphasis alongside other conditions, because pregnancy requires tonification while Sha requires resolution; postpartum requires warming while Sha requires cooling. Therefore, medication for Sha must be carefully considered.
For Sha on the skin and muscles, scraping with oil and salt prevents the Sha toxin from attacking inward. For Sha involving flesh and blood, observing cyanotic or purple veins and performing pricking therapy allows the Sha toxin to be drained. For Sha affecting the meridians of the intestines, stomach, spleen, liver, and kidneys (the Three Yin meridians), treatment must differentiate the meridian and Zang-Fu organs, distinguishing whether the pathogen is in the Qi level or Blood level. Thus, Sha attacking inward can be resolved, dispersed, or expelled, eliminating the root of the disease.
Sha is contraindicated with hot soup, hot wine, porridge, rice, and similar foods. Drinking hot liquids causes mild cases to worsen and severe cases to die immediately. Eating rice may cause Sha masses to form, which over time transform into bizarre diseases that are extremely difficult to treat. Even if one fortunately digests the food without dying, this cannot serve as a precedent.
When Sha symptoms slightly relax and hunger is felt in the chest, sudden intake of food will cause Sha distension to recur, potentially worsening the condition immediately. One must endure for one or two days as a rule to ensure complete safety.
Sha presenting with a chest protrusion like a steamed bun is incurable. Pain at a single point on the back indicates death. Opisthotonos (body bent backward like a bow) indicates death. Pain at a single point in the lower back/kidney region indicates death. Pain at a single point on either side of the chest is incurable. Hypochondriac pain is incurable. Swelling and pain in the four limbs are difficult to treat. A nose color resembling soot indicates death. Tongue retraction and testicular retraction indicate death. Blackening around the mouth indicates death. Head sweating like pearls with continuous panting indicates death. Coma with inability to awaken, failure to release Sha through pricking, and ineffectiveness of medication indicate death. Severe pain from Sha masses with ineffectiveness of medication indicates death.
All hundred diseases have momentum that severely harms the body. However, there are cases where danger does not immediately appear, allowing time to drag on, yet suddenly transforming into a critical, incurable state. Why? Among the hundred diseases, some contract the summer-heat epidemic qi due to illness; others are affected by foul odors from unclean bedding, combined with deep-seated latent toxins from daily life. Under these circumstances, Sha can opportunistically emerge. The value lies in the physician's insight: treat the Sha first, then manage the underlying disease. This exemplifies the principle of "treating the branch in acute conditions and the root in chronic conditions."
